Radiographic evaluation of stillbirth: what does it contribute?

Despite advances in perinatal care, stillbirth is relatively common (1/160 births) and frequently remains unexplained. Most recent protocols for etiologic evaluation of stillbirth either omit radiography or reserve it for infants with obvious skeletal disproportion. Over the past 30 years, the Wisconsin Stillbirth Service Program has collected radiographic images from 2,032 stillbirths and second trimester losses, of which about 25% (517) showed abnormalities. Review of these images by a medical geneticist showed that radiographs yielded a diagnosis in 45% of the infants with abnormalities (11

Stillbirth: knowledge and practice among U.S. obstetrician-gynecologists.

OBJECTIVE: To determine knowledge of U.S. obstetrician-gynecologists (OBGYNs) and individual and institutional practices regarding stillbirth. STUDY DESIGN: We surveyed 1,000 members of the American College of Obstetricians and Gynecologists regarding their knowledge of risk factors and causes of stillbirth and self-rated performance in stillbirth management. RESULTS: Of the 499 who responded, 365 currently practiced obstetrics. Knowledge regarding epidemiology, risk factors, and effective interventions to reduce stillbirth was only fair. About 30% of respondents were unaware that preeclamps

Wharton's jelly absence: a possible cause of stillbirth.

The umbilical cord is a structure that provides vascular flow between the fetus and the placenta. It contains two arteries and one vein, which are surrounded and supported by gelatinous tissue known as Wharton's jelly. There are many umbilical cord abnormalities that are related to the prognosis of fetus survival and birth weight. The authors report a case of umbilical cord constriction due to the localized absence of Wharton's jelly, which was undiagnosed antenatally and had a fatal outcome. A review of the association between the absence of Wharton's jelly and an unfavorable pregnancy outcom

Stillbirth in cases of severe acute maternal morbidity.

OBJECTIVE: To determine the incidence and correlates of stillbirths among women with severe acute maternal morbidity (SAMM). METHODS: In an observational study of 728 women who had SAMM between January 2007 and December 2010 at a referral tertiary health facility in Benin, Nigeria, the incidence of stillbirth, and the clinical and demographic correlates of stillbirth were evaluated. RESULTS: The rate of stillbirth among women with SAMM was 210 per 1000 deliveries. The rate among women who had uterine rupture (643 per 1000 deliveries) far exceeded other cause-specific rates of stillbirth. Unb

The Stillbirth Collaborative Research Network neuropathologic examination protocol.

We describe the neuropathologic procedure utilized in the Stillbirth Collaborative Research Network (SCRN), focusing on the examination of central nervous system (CNS) in stillbirth (SB). The SCRN was organized to perform a case-control study to determine the scope and causes of SB. Pathologists at all the participating centers agreed on and used the same standardized neuropathologic techniques. Standardized sections were taken and detailed data were collected. Fresh brain tissue was saved for investigative purposes. A total of 663 women with SB were enrolled into the case-control study: 620 d

The stillbirth collaborative research network postmortem examination protocol.

After reviewing the state of knowledge about the scope and causes of stillbirth (SB) in a special workshop sponsored by the Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD), the participants determined that there is little guidance regarding the best use of postmortem examination (PM) to address the pathogenesis of stillbirth. In this report, we describe the PM procedure designed and used in the NICHD-supported Stillbirth Cooperative Research Network (SCRN). Perinatal pathologists, clinicians, epidemiologists, and biostatisticians at four tertiary care ce

IVF and stillbirth: a prospective follow-up study.

BACKGROUND: Previous studies have indicated that the risk of stillbirth is increased in singleton pregnancies achieved after assisted reproduction technology (ART). However, no previous study fully accounted for factors with potential influence on the risk of stillbirth. Further, whether fertility treatment, the possible reproductive pathology of the infertile couples or other characteristics related to being subfertile may explain a possible association with stillbirth remains unclear. This study compares the risk of stillbirth in women pregnant after fertility treatment (IVF/ICSI and non-IVF

Management of subsequent pregnancy after an unexplained stillbirth.

PURPOSE: To review the management of pregnancy after an unexplained stillbirth. EPIDEMIOLOGY: Approximately 1 in 200 pregnancies will end in stillbirth, of which about one-third will remain unexplained. Unexplained stillbirth is the largest single contributor to perinatal mortality. Subsequent pregnancies do not appear to have an increased risk of stillbirth, but are characterized by increased rates of intervention (induction of labor, elective cesarean section) and iatrogenic adverse outcomes (low birth weight, prematurity, emergency cesarean section and post-partum hemorrhage). CONCLUSIONS

[Stillbirth and tight umbilical cord knot: obviously guilty?].

A healthy woman, gravida 1, para 1, spontaneously delivered a stillborn at 40 weeks of gestation, presenting with a tight umbilical cord knot. Fetopathological examination confirmed the main but not exclusive role of the knot in this intra-uterine fetal death (IUFD). Knot of the umbilical cord is a rare occurrence, which is usually an incidental finding at delivery. However, it is associated with an increase in perinatal mortality. Only a complete fetopathological examination can state causality between knot and IUFD, exclude another fetal or placental cause of death and consequently reassure

Stillbirth rates in Peruvian populations at high altitude.

OBJECTIVE: To compare the stillbirth rates in 4 cities at different altitudes in Peru. METHODS: The stillbirth rates from a sample of 22,662 births recorded in the Perinatal Information System database were analyzed between 2005 and 2006 for the cities of Lima (150 m), Huancayo (3280 m), Cuzco (3430 m), and Puno (3850 m). RESULTS: Stillbirths were higher at high altitude (>3000 m) (OR 4.82; CI, 3.05-7.72) compared with low altitude. The effect of altitude was observed after controlling for socioeconomic (OR 4.29; CI, 2.61-6.93), demographic and biological (OR 4.18; CI, 2.65-6.77), maternal h

Stillbirth: tissue findings with environmental and genetic links.

This article discusses stillbirth from a pathologist's perspective. Stillbirth may be caused by many different mechanisms. There are specific maternal and fetal disorders leading to stillbirth, and some of these have genetic and environmental associations. Frequently, an autopsy examination may determine the specific cause of the fetal death. We illustrate tissue findings in stillbirth to put individual faces on this condition.

Gestational-age-specific stillbirth risk among Australian Aborigines.

BACKGROUND: Previous studies have found that, for preterm babies, the risk of stillbirth among Australian Aborigines is similar to the risk for whites. In contrast, at full term, the risk for Aborigines is more than twice the risk for whites. However, these studies (like most other analyses of stillbirths) used the number of births at each gestational age to calculate the risk. For stillbirths, the risk is more appropriately estimated using the number of babies in utero. This paper presents the first comparison of stillbirth risk for Australian Aborigines and whites using the more appropriate

[Stillbirth--an analysis of 354 cases 1966-88].

A study was conducted on the collection of stillbirths occurring during the last 23 years in the Gynaecological Hospital in Bamberg. The data were taken from the birth reports and the recorded course of each pregnancy. The results of the first 22 years were divided into 2 groups, the first 11 years and the subsequent 11 years, to enable better judgement of trend changes. A total number of 354 children died antepartum or subpartum, the larger group (249) being that in the first 11 years of the observation period. The number of stillborn babies decreased from 40 per year (15%) to 3 per year (1.1

Risk of unexplained stillbirth at different gestational ages.

In 40,635 deliveries in 1978-85, unexplained stillbirths were an important component (nearly a quarter) of all perinatal deaths. The rate of unexplained stillbirth (unexplained stillbirths divided by total births) was highest among preterm deliveries, fell to a minimum at 39-40 weeks' gestation, then rose at 41-42 weeks. Rate is generally accepted as measuring risk, but since it is the population of undelivered, not delivered, infants that is at risk of intrauterine death, stillbirth risk would be better measured as the number of impending stillbirths divided by the total number of undelivered

Modeling and Exploring Stillbirth Risks in Northern Pakistan.

Background: The World Health Organization (WHO) defines stillbirth as the loss of a fetus after 28 weeks of gestation. Annually, approximately 2 million stillbirths occur worldwide. Projections indicate that by 2030, this figure could rise to nearly 15.9 million, with half of these stillbirths expected to take place in Sub-Saharan Africa. In the global literature, causes include placental complications, birth defects, and maternal health issues, though often the cause is unknown. Stillbirths have significant emotional and financial impacts on families. Methods: The process involves using chi-s

Call for Improved Fourth Trimester Care After Stillbirth.

Women who experience stillbirths are at increased risk for severe maternal morbidity and mortality, which makes the postpartum period a critical time in which to address health conditions and prevent complications. However, research on the health care needs of women who experience stillbirths is scarce, and these women are often excluded from research on the postpartum period. Therefore, the purpose of this commentary is to identify gaps in the research on postpartum care after stillbirth, explain why current fourth trimester care guidelines in the United States are inadequate, and advocate fo
