Gastric volvulus. More common than previously thought?

Gastric volvulus is torsion of the stomach, which can compromise the gastric orifices and result in either acute or chronic and intermittent symptoms. We believe that gastric volvulus, especially when partial or intermittent, may be more common than has been previously thought. The condition should be suspected in any patient who has a history of retching or vomiting and has a paraesophageal hiatus hernia or eventration of the diaphragm. The classic triad of retching, severe and constant epigastric pain, and difficulty in passing a nasogastric tube should suggest the presence of acute gastric

A Rare Case of Emphysematous Gastritis Secondary to Organo-Axial Gastric Volvulus Associated With Sarcina ventriculi.

Gastric volvulus is a potentially life-threatening condition that can compromise the blood supply to the stomach predisposing to ischemia, eventually necrosis and cell death associated with gas-forming bacteria. Sarcina ventriculi has been associated with emphysematous gastritis also. We report a case of emphysematous gastritis associated with S. ventriculi in the setting of organo-axial gastric volvulus.

Acute Gastric Volvulus and Atrial Fibrillation with RVR: A Coincidence or Association.

Gastric volvulus is a rare and life-threatening condition that involves the abnormal rotation of the stomach around its axis by more than 180°. The association between acute gastric volvulus and atrial fibrillation with rapid ventricular response is rare with only few cases that have been reported. Our patient was an 86-year-old female who presented with upper abdominal pain, distension, nausea, and shortness of breath. Clinical and laboratory workup revealed acute gastric volvulus with diaphragmatic hernia. On presentation, she was also in atrial fibrillation with rapid ventricular response.

Acute Mesentero-Axial Gastric Volvulus in the Setting of a Paraesophageal Hernia: A Rare Case Report.

Acute gastric volvulus is a surgical emergency that requires urgent intervention to prevent gastric ischemia and necrosis. Gastric volvulus manifests as an abnormal rotation or torsion of the stomach and may be associated with gastric outlet obstruction. This pathology can be classified as either mesentero-axial or organo-axial volvulus, depending on the axis of rotation. Similarly, it can be categorized as primary or secondary, depending on the etiology. We describe a case of a 63-year-old female with a history of peptic ulcer disease who presented with severe epigastric pain and vomiting of

An unusual case of gastric volvulus through Bochdalek hernia A case report and review of the recent literature.

INTRODUCTION: Bochdalek Hernias are one of the most common types of diaphragmatic hernia, a congenital pathology diagnosed during the neonatal period. In adulthood, diagnosis of a Bochdalek hernia is rare and its complications may be fatal. CASE REPORT: We report an unusual case of a 60-year-old woman who presented with upper gastrointestinal bleeding due to a Bochdalek hernia with herniation of stomach and spleen into the chest with a gastric volvulus. Endoscopy showed a strong suspicion of ischemic suffering of the stomach walls, therefore the patient was subjected to urgent laparotomy. The

[Gastric emphysema associated with gastric volvulus].

The gastric volvulus is an infrequent entity requiring surgical treatment in both the acute and chronic cases. The case of an 81-year old female patient attended for an episode of gastric obstruction caused by a volvulus and whose simple stomach radiography showed an image of gastric emphysema is reported. The possible causes of gastric emphysema and its differential diagnosis with emphysematous gastritis are discussed.

A Case of Intermittent Organo-Axial Gastric Volvulus.

Gastric volvulus is defined as twisting of the stomach along its long or short axis, a rare clinical entity that can lead to gastric outlet obstruction. Symptoms are typically acute and include retching, abdominal pain, and vomiting. Chronic intermittent manifestations of this entity present a diagnostic challenge as conclusive imaging findings are only apparent during symptomatic periods. Given the disorder's intermittent nature, a volvulus may resolve before imaging studies can be conducted. We present a rare case of an intermittent organo-axial gastric volvulus that responded to conservativ

Acute gastric volvulus: an uncommon complication of a hiatus hernia.

An 80-year-old male patient with a history of a hiatus hernia presented with acute abdominal pain and vomiting. CT of his abdomen revealed extraluminal free gas consistent with a perforation. He had a large hiatus hernia. The subdiaphragmatic portion of the stomach was distended and adopted a more transverse lie. The radiological findings were in keeping with acute gastric volvulus with secondary ischaemic complications. Acute gastric volvulus is an abnormal rotation of the stomach resulting in complete obstruction. It is a surgical emergency and does not always present in its classical form.

Acute Gastric Volvulus: A Rare Complication of Hiatal Hernia.

Gastric volvulus, characterized by stomach rotation, is a rare condition arising from congenital or acquired factors. Predominantly affecting pediatric and elderly populations, it necessitates a high index of suspicion for timely diagnosis. Delayed recognition may precipitate severe complications such as ischemia, strangulation, and septic shock, often culminating in fatal outcomes. We present a case of a 71-year-old male initially admitted for suspected gastroenteritis, subsequently developing acute gastric volvulus during hospitalization, necessitating immediate surgical intervention. Emergent laparoscopic reduction of acute gastric volvulus with anterior gastropexy.

Gastric volvulus is characterized by abnormal rotation of the stomach around an axis made by two fixed portions. Symptoms of gastric volvulus range from anemia and weight loss to severe epigastric or chest pain associated with nonproductive vomiting or upper gastrointestinal bleeding. Ischemia, necrosis, and perforation will occur if this condition remains untreated. We report a case of a 92-year-old patient with acute gastric volvulus treated with laparoscopic reduction and anterior gastropexy. We suggest that the laparoscopic approach to gastric volvulus is safe and feasible and should be co

Pediatric gastric volvulus--experience with 7 cases.

Gastric volvulus, organoaxial or mesenterioaxial, is a rare condition in infancy and childhood. We experienced 7 cases of pediatric gastric volvulus, consisting of 3 cases of secondary gastric volvulus due to left diaphragmatic eventration or paraesophageal hernia and 4 cases of idiopathic gastric volvulus. Of 7 cases, five were organoaxial in type and two were mesenterioaxial. The main symptoms of secondary gastric volvulus were vomiting and respiratory difficulty whereas those of idiopathic gastric volvulus were abdominal distension and weight loss with or without failure to thrive. It may b

Gastric volvulus after sleeve gastrectomy for morbid obesity.

BACKGROUND: Laparoscopic sleeve gastrectomy in morbid obesity has proved to be a safe and reproducible technique. Sleeve gastrectomy, however, is not free of complications. On the other hand, gastric volvulus is reported in those subjects where, either because of laxity of the gastric anatomical fixations or incorrect position of the stomach, rotation or turning is facilitated. CASE: We report the case of a patient with morbid obesity (Bone mass index / BMI 63 Kg/m2), who in the post-operative period immediately following a sleeve gastrectomy, presented early symptoms of upper gastrointestina

Trauma-Induced Delayed Presentation of Diaphragmatic Hernia with Gastric Volvulus: A Rare Presentation.

A 19-year-old male presented with a three-day history of worsening epigastric pain, persistent vomiting, abdominal distension, and obstipation. His medical history included high-fall trauma in 2022, resulting in a parietal bone fracture, scalp swelling, and an epidural hematoma. Physical examination revealed epigastric tenderness, abdominal distension, and diminished bowel sounds. Imaging revealed a 7 cm left diaphragmatic defect with herniation of the stomach, colon, and mesentery into the thoracic cavity, confirming a diagnosis of gastric volvulus with a diaphragmatic hernia. An exploratory

Diagnosis of chronic gastric instability using computed tomography in a Great Dane that progressed to gastric dilatation and volvulus: a literature review and case report.

Chronic gastric instability (CGI) is part of the gastric dilatation and volvulus (GDV) complex and contributes to the pathophysiologic mechanism of volvulus. Clinical signs can be attributed to a flaccid and mobile stomach. This manuscript reviews the literature regarding CGI and presents a case where computerized tomography is utilized in combination with abdominal radiography to diagnose CGI in an 8 year old spayed female Great Dane. The patient presented for nonspecific clinical signs including hypersalivation, anorexia, and regurgitation, and was euthanized after failing medical management

Chronic Gastric Volvulus with Laparoscopic Gastropexy after Endoscopic Reduction: A Case Report.

Gastric volvulus is an uncommon clinical entity. There are three types of gastric volvulus; organoaxial, mesenteroaxial and combined type. This condition can lead to a closed-loop obstruction or strangulation. Traditional surgical therapy for gastric volvulus is based on an open approach. Here we report a successful case of a patient with chronic gastric volvulus with a laparoscopic treatment. A 79-year-old woman came to the emergency department with epigastric pain accompanied by nausea for 2 weeks. Abdominal computed tomography revealed markedly distended stomach with transposition of gastro

Pediatric gastric volvulus: diagnostic and clinical approach.

Gastric volvulus is a significant, rare cause of non-bilious vomiting and consists of a pathological rotation of the stomach of more than 180° around the axis without obstruction of the gastrointestinal tract. A definitive diagnosis is made with upper radiological gastrointestinal studies. Treatment may be conservative or surgical with anterior and fundal gastropexy in patients with ingravescent symptoms. We describe the case of a 16-month-old female admitted to our hospital for recurrent and postprandial vomiting episodes which had started at 11 months of age. A history of gastroesophageal re

Endoscopic Reduction of an Acute Gastric Volvulus.

We are reporting a case of gastric volvulus in a 52-year-old man in the setting of a paraesophageal hernia initially identified on computerized tomography (CT). CT of the abdomen showed a large paraesophageal hernia with intra-thoracic herniation of the distal stomach and gastroduodenal junction, resulting in mesenteroaxial rotation consistent with acute gastric volvulus. Esophagogastroduodenoscopy (EGD) confirmed the presence of the gastric volvulus, which was initially temporized with endoscopic detorsion. He subsequently had nasogastric tube placement and ultimately underwent a laparoscopic

Acute Gastric Volvulus With Wandering Spleen in a Two-Year-Old Child: A Rare Association.

Gastric volvulus is a rare clinical condition characterized by a pathological rotation of the stomach greater than 180º around its axis. The wandering spleen is also an exceptional clinical entity characterized by the absence or laxity of splenic ligaments which lead to splenic mobility in the abdominal cavity from its normal anatomical site. Wandering spleen and gastric volvulus association is unusual. Both can be life-threatening if left untreated. We herein present a rare, unusual association of mesenteroaxial gastric volvulus with wandering spleen in a two-year-old child and interpret the

Gastric remnant volvulus following gastric sleeve conversion to Roux-en-Y gastric bypass: a case report.

Gastric remnant volvulus following Roux-en-Y gastric bypass (RYGB) surgery is rare, with only two previously reported cases. Herein, we present the first case of gastric remnant volvulus following gastric sleeve conversion to RYGB in a 32-year-old female. Management for gastric remnant volvulus has not been clearly described in the literature due to the rarity of cases; however, previously documented cases of gastric remnant volvulus following RYGB were managed with gastropexy or resection of the gastric remnant. Due to anatomical limitations, gastropexy was not an option for our patient, and
