Coma from worsening spontaneous intracranial hypotension after subdural hematoma evacuation.

BACKGROUND: Low cerebrospinal fluid volume is typically diagnosed in patients presenting with positional headaches. However, severe intracranial hypotension and brain sagging may cause orthostatic coma. We present a case that illustrates this uncommon presentation. METHOD: Case report. RESULTS: A 50-year-old man presented with orthostatic headaches and then developed bilateral subdural hematomas. Following unilateral subdural hematoma evacuation, the patient became gradually drowsier and more confused. Upon transfer to our hospital, he would become comatose each time he was placed in the upr

Ultrasonography for Serial Monitoring and Management of Cerebrospinal Fluid Dynamic Disorders After Decompressive Craniectomy.

OBJECTIVE: Decompressive craniectomy (DC) is widely used to treat intracranial hypertension following severe head injury. However, impairments of cerebrospinal fluid (CSF) hydrodynamics such as hydrocephalus and subdural effusion are common complications that occur after DC. Therefore, monitoring of intracranial pressure is a staple of neurocritical care post-DC. The aim of this study was to assess the usefulness of transcranial duplex sonography (TDS) for serial monitoring and management of CSF disorders after DC. METHODS: A total of 100 patients who underwent DC between June 2016 and May 20

Subdural fluid accumulation with two meningiomas in a cat.

A case of two separate meningiomas with concurrent subdural fluid accumulation is presented. A 13-year-old, male neutered domestic shorthair cat presented with behavior change and progressive neurologic signs localizing to the forebrain. Two extra-axial masses were seen along with a large subdural fluid collection on advanced imaging of the brain. Histology of the lesions was consistent with multiple meningiomas, while analysis of the subdural fluid was consistent with cerebrospinal fluid. A case of multiple feline meningiomas with peritumoral subdural fluid accumulation has not previously bee

Spontaneous intracranial hypotension: clinical and neuroimaging findings in six cases with literature review.

Spontaneous intracranial hypotension (SIH) is an uncommon but increasingly recognized syndrome. Orthostatic headache with typical findings on magnetic resonance imaging (MRI) are the key to diagnosis. Delayed diagnosis of this condition may subject patients to unnecessary procedures and prolong morbidity. We describe six patients with SIH and outline the important clinical and neuroimaging findings. They were all relatively young, 20-54 years old, with clearly orthostatic headache, minimal neurological signs (only abducent nerve paresis in two) and diffuse pachymeningeal gadolinium enhancement

Delayed symptoms of cerebrospinal fluid leak following lumbar decompression.

Numerous reports in the literature describe cerebrospinal fluid (CSF) leaks resulting from dural tears; however, most of these reports document symptoms of incidental durotomy occurring ,48 hours postoperatively. This case report describes patients with symptoms of CSF leak occurring a few days to a few weeks after lumbar spine surgery who required additional surgery to repair the dura and alleviate their symptoms. Records for patients who had lumbar spine surgery performed by 2 spine surgeons at a single institution between 1990 and 2005 were reviewed. After additional surgery to repair what

Blood patch for spontaneous intracranial hypotension caused by cerebrospinal fluid leak at C1-2.

A case of spontaneous intracranial hypotension (SIH) caused by a cerebrospinal fluid (CSF) leak at C1-2 is described. The patient, a 46-year-old gentleman, presented to the emergency department with a severe, orthostatic neck pain and occipital headache of sudden onset. He was diagnosed with SIH and admitted, but failed to respond to conservative management. Imaging studies suggested that C1-2 was the spinal level responsible for the CSF leak, and he underwent a blood patch therapy delivered via an epidural catheter inserted from C6-7. His neck pain disappeared a day after the procedure, and h

Chronic subdural hematoma: What precisely are we treating?

Subdural hematoma (SDH) refers to the collection of blood between the dura matter and the arachnoid membrane. Advancements in imaging technology have enabled the categorization of SDH based on specific imaging characteristics, causative factors, and the onset of symptoms. Given that the prognosis of SDHs varies significantly and is contingent upon the size and chronicity of the hemorrhage, a comprehensive understanding of its subtypes may carry crucial treatment implications. For example, an acute SDH classically results from severe traumatic brain injury and appears as a homogenous, crescent-

[Role of toxoplasmosis in the development of central nervous system lesions in children].

Because of the high degree of infestation in women capable of bearing children in Europe (70 to 80%) with Toxoplasma gondii the authors have studied all babies admitted for clinical treatment at the institute of Paediatrics in Lublin for clinical and serological indications of a connatal toxoplasmosis. In the period between January 1977 and December 1978, 133 babies showed positive findings. The most frequently occurring findings were lesions of the central nervous system (hydrocephalus 21 X, microcephaly 8 X, porencephaly 18 X, convulsions and psychomotor retardation 11 X, subdural hygroma an

Cerebrospinal Fluid Management in the Setting of Decompressive Hemicraniectomy.

Decompressive hemicraniectomy can lead to cerebrospinal fluid (CSF)-related complications including hydrocephalus, hygroma formation, and CSF fistulas. The formation of CSF hygromas is often self-limiting and most resolves spontaneously or following cranioplasty, but refractory or symptomatic collections may require subdural drains or shunting. Hydrocephalus in a patient with a decompressive hemicraniectomy must be differentiated from ventriculomegaly that may be a result of the original insult with shunting being the primary treatment of hydrocephalus. In this review, pathogenesis, management

External hydrocephalus in the adult population with acute brain injury. A narrative review.

External hydrocephalus is a frequent and poorly recognized condition in the adult population with acute brain injury. It refers to cerebrospinal fluid flow impairment, with enlargement of the subarachnoid space in the context of raised intracranial pressure. This manuscript reviews the current evidence on epidemiology, pathophysiology, neuroimaging, diagnostic criteria and treatment strategies of acute external hydrocephalus in adult patients with acute brain injury. A total of 15 studies were included, mainly retrospective and case series, with a limited methodological quality. Intracranial b

Bedside transfontanelle drain placement reduces the need for operative intervention in infants with subdural fluid collections.

OBJECTIVE: Subdural fluid collection is a common neurosurgical condition in the pediatric population. Patients requiring surgical intervention have historically been managed with subdural drains, subdural-subgaleal shunting, subdural-peritoneal shunting, and mini-craniotomies. An alternative procedure for patients with an open anterior fontanelle is bedside transfontanelle drainage. This study aims to evaluate the efficacy of the transfontanelle angiocatheter drain placement including complications and the need for any further surgery. METHODS: We performed a single-center retrospective revie

[Role of Ommaya reservoir in the management of neonates with post-hemorrhagic hydrocephalus].

OBJECTIVE: Intra-ventricular hemorrhage (IVH) is one of the most serious complications of preterm infants. Significant numbers of the surviving infants with severe IVH go on to develop post-hemorrhagic hydrocephalus (PHH). The management of PHH remains a very challenging problem for both neonatologists and pediatric neurosurgeons. This study aimed to evaluate the efficacy and safety of the use of Ommaya reservoirs and serial cerebrospinal fluid (CSF) drainage in the management of a series of neonates with PHH. METHOD: Between January 1, 2003 and December 30, 2005, 15 consecutive newborn infan

Timing Matters: A Comprehensive Meta-Analysis on the Optimal Period for Cranioplasty After Severe Traumatic Brain Injury.

BACKGROUND AND OBJECTIVES: Craniectomy is a commonly performed procedure in severe traumatic brain injury (TBI) to control intracranial hypertension and reduce mortality. The optimal timing for cranioplasty after craniectomy remains a topic of debate. The aim of this study was to investigate the ideal timing for cranioplasty after severe TBI, focusing on complications associated with temporal aspects and materials used in cranioplasty. METHODS: Multiple databases, including PubMed, Embase, Cochrane, and Web of Science, were searched for studies reporting on the subject. Inclusion criteria inv

[Intracranial pneumocephalus under pressure. A possible cause of postoperative deterioration in neurosurgery. 10 cases].

10 patients in whom an early post operative alteration of the level of consciousness or of the neurological status could be related to a tension pneumocephalus are reviewed. Four underwent a surgical procedure in the sitting position. Among these, 2 had been previously shunted for hydrocephalus. A 5th patient underwent a IIId ventriculocisternostomy for tumorous occlusion of the aqueduct. The other five patients were involved with supra-tentorial conditions (hemispheric glioma: 1 case, meningioma: 1 case, chronic subdural hematoma: 3 cases). The post-operative deterioration clinically suggeste

Divergence paresis without positional headache: an unusual presentation of cerebrospinal fluid hypovolemia after spinal anesthesia.

We report a rare complication of spinal anesthesia-divergence paresis-which is characterized by an acquired horizontal diplopia at distance without evidence for abducens palsy. A 64-yr-old man underwent prostatectomy under spinal anesthesia with 2.5 mL of dibucaine hydrochloride 0.3% injected through a 20-gauge cutting-tip spinal needle. Seventeen days after the operation, the patient noticed horizontal diplopia for distant objects. Although cranial magnetic resonance imaging demonstrated diffuse pachymeningeal gadolinium enhancement and subdural effusion, characteristic findings of cerebrospi

Primary low grade B-cell lymphoma of the dura in an immunocompetent patient.

Primary central nervous system lymphomas (PCNSL) represent only 1% of all non-Hodgkin's lymphomas (NHLs). A 66 year-old woman was hospitalized due to multiple episodes of syncope and seizures which occurred the week before admission to the hospital. A computerized tomography (CT) scan of the brain showed a right parietal lesion suggesting a subdural effusion. The patient was operated and a dural lesion extending to the epidural space and cerebral cortex was excised. Histologic findings suggested diagnosis of a low-grade lymphoma of the mucosa associated lymphoid tissue (MALT) type. The patient
