Periodontal healing and bone regeneration in response to azithromycin.

Azithromycin, first synthesized in 1980, is a macrolide antibiotic related to erythromycin. It is widely used by the medical profession as a broad-spectrum antibiotic in the treatment of pneumonia, urinary tract infections and tonsillitis. In addition to its antibiotic properties, azithromycin has immune-modulating effects and is used for this reason in the management of cystic fibrosis and chronic obstructive pulmonary diseases. The drug is taken up by neutrophils, macrophages and fibroblasts, and is slowly released by these cells. Three diverse case reports are presented in which a single co

Radiographic bone fill following non-surgical therapy--a report of three cases.

PURPOSE: Non-surgical therapy, including scaling, root planing, and personal plaque control continues to be a powerful effective treatment modality for treating chronic periodontal diseases. Reports in the literature have suggested that adjunctive procedures such as systemic antibiotics and topical antiseptics may enhance the clinical outcomes of non-surgical treatment. MATERIAL AND METHODS: Three retrospective case reports spanning an evaluation period of 9-36 months are presented. The patients were treated in a private periodontal practice and had advanced periodontal disease. A significant

Subgingival polishing with a teflon-coated sonic scaler insert in comparison to conventional instruments as assessed on extracted teeth. (I) Residual deposits.

BACKGROUND: Recent studies have shown that endotoxins are located on the periodontally diseased root cementum and not within it. These studies led to the suggestion that the root surface could be treated less aggressively during periodontal therapy. Thus, we designed a teflon-tubed sonic scaler insert for subgingival polishing. It was our objective to assess to which extent this new instrument is capable of removing bacterial deposits in deep pockets in comparison with conventional scaling instruments. METHOD: We compared the extent to which plaque and calculus could be removed with a curette

Effects of combined topical metronidazole and mechanical treatment on the subgingival flora in deep periodontal pockets in cuspids and bicuspids.

The Effect on the subgingival microflora of a single topical administration of a 95% collagen and 5% metronidazole device in combination with debridement was investigated in 30 adult periodontitis patients in comparison with mechanical treatment alone. For each patient, plaque samples from test and control sites in cuspids and bicuspids were collected for culture and enumeration of total anaerobically cultivable bacteria (TA), black-pigmented anaerobes (BPA), and Actinobacillus actinomycetemcomitans (Aa). Spirochetes and fusiforms were quantified by direct microscopic examination after Giemsa

Campylobacter rectus in human periodontitis.

Campylobacter rectus (formerly Wolinella recta) in periodontitis lesions was studied relative to age and sex distribution, relationship to disease-active periodontitis, response to periodontal debridement and in vitro antimicrobial susceptibility. Subgingival C. rectus was collected with paper points, transported in VMGA III and plated onto nonselective enriched brucella blood agar and Hammond's selective medium for C. rectus, both incubated anaerobically. C. rectus was recovered from 80% of 1654 periodontitis patients. Although the organism showed similar age and sex occurrence, its proportio

[Adjuvant antibiotic therapy as a 2-step treatment concept in early-onset periodontitis: a strategy for eradication of bacteria that cause periodontitis].

BACKGROUND AND OBJECTIVE: The periodontal region is a source of gram-negative bacterial infection. The pathogens involved have recently also been demonstrated in atheromatous plaques. They may increase the risk of myocardial infarction. In this study a strategy for eradicating periodontal bacteria and thus healing in patients with periodontal pockets and advanced destruction of alveolar bone was examined. PATIENTS AND METHODS: Initial periodontal status was documented in 36 patients with periodontitis (24 women, 12 men) who were then randomly assigned to one of three groups. Those in two of t

Efficiency of nonsurgical periodontal therapy in moderate chronic periodontitis.

INTRODUCTION: Chronic periodontitis is defined as an inflammatory disease of the supporting tissues of teeth caused by microorganisms in the dental biofilm, resulting in progressive destruction of the periodontal ligament and alveolar bone with pocket formation and gingival recession. Treatment of chronic periodontitis aims at arresting the inflammation and stopping the loss of attachment by removal and control of the supra- and subgingival biofilm and establishing a local environment and microflora compatible with periodontal health. The AIM of this study was to evaluate the effectiveness of

Immunomodulators, associated or not with systemic antibiotics, to treat periodontitis: A 1-year multicenter, placebo-controlled, double-blind, randomized clinical trial.

BACKGROUND: Immunomodulatory agents such as omega-3 (ω-3) and low-dose aspirin (ASA) have been proposed as effective adjuncts in the treatment of periodontitis. However, they still lack long-term studies and head-to-head evaluation with metronidazole (MTZ) and amoxicillin (AMX), the adjunct protocol with the strongest scientific evidence of clinical benefit. This study aimed to evaluate the clinical effects of (ω)-3 and ASA, alone or combined with MTZ + AMX, to treat periodontitis. METHODS: Adults with stage III/IV periodontitis were randomly allocated to receive: (i) subgingival instrumentat

Clinical and microbiological effects of ozone nano-bubble water irrigation as an adjunct to mechanical subgingival debridement in periodontitis patients in a randomized controlled trial.

AIM: Ozone nano-bubble water (NBW3) seems to be suitable as an adjunct to periodontal treatment owing to its potent antimicrobial effects, high level of safety, and long storage stability. The aim of the present study was to evaluate the clinical and microbiological effects of NBW3 irrigation as an adjunct to subgingival debridement for periodontal treatment. METHODS: Twenty-two subjects were randomly assigned to one of the two treatment groups: full-mouth mechanical debridement with tap water (WATER) or full-mouth mechanical debridement with NBW3 (NBW3). Persistence patterns of Porphyromonas gingivalis, Prevotella intermedia/nigrescens, and Actinobacillus actinomyetemcomitans after mechanical therapy of periodontal disease.

BACKGROUND: The aim of this study was to determine the distribution patterns of Porphyromonas gingivalis, Prevotella intermedia/nigrescens, and Actinobacillus actinomycetemcomitans in periodontitis patients after standard mechanical periodontal therapy, and to determine factors increasing the odds to detect these target organisms in treated sites. METHODS: Eight hundred fifty-two (852) separate subgingival microbial samples were taken from the mesial and distal aspects of every tooth in 17 patients. Target organisms were identified culturally. RESULTS: The 3 microorganisms showed different p

The use of a triclosan/copolymer dentifrice may retard the progression of periodontitis.

The present investigation was performed to examine if triclosan and a copolymer, incorporated in a dentifrice and used by periodontitis-susceptible subjects could influence clinical symptoms characteristic of recurrent periodontitis. 60 subjects, 34 to 67 years of age, were recruited for the study. They were randomly selected from a group of patients previously treated for advanced periodontal disease. This treatment had included oral hygiene instruction, subgingival debridement, but no surgical therapy. The patients had, during a 3-5 year period following active therapy, been enrolled in a ma

Dental calculus: recent insights into occurrence, formation, prevention, removal and oral health effects of supragingival and subgingival deposits.

Dental calculus, both supra- and subgingival occurs in the majority of adults worldwide. Dental calculus is calcified dental plaque, composed primarily of calcium phosphate mineral salts deposited between and within remnants of formerly viable microorganisms. A viable dental plaque covers mineralized calculus deposits. Levels of calculus and location of formation are population specific and are affected by oral hygiene habits, access to professional care, diet, age, ethnic origin, time since last dental cleaning, systemic disease and the use of prescription medications. In populations that pra

Radiographic bone fill of peri-implantitis defects following nonsurgical therapy: report of three cases.

It is reported that peri-implantitis does not heal favorably following nonsurgical therapy. In this case study, three consecutive peri-implantitis patients with concomitant history of periodontitis were treated with nonsurgical therapy and reassessed up to 12 months following treatment. All treated peri-implantitis sites showed a considerable reduction in probing pocket depth, attachment loss, and bleeding on probing. This was also associated with bone fill of the vertical bony defects around the previously exposed implant threads. This case report shows that a degree of clinical resolution an
