BACKGROUND: Intrathecal morphine for postoperative analgesia after caesarean section has been used in Europe and North America, but its use is not common in Japan.
METHODS: We randomized 40 parturients to two groups, given either intrathecal saline (control group) or intrathecal morphine 0.05 mg (morphine group) for caesarean section. To both groups, we gave a diclofenac suppository 50 mg every 8 hours after surgery.

A 22-year-old female was admitted to our hospital because of general fatigue and continuous high fever. Salmonella Typhi was detected from her blood. Her mother, 44-year-old female was admitted to our hospital a few days later because of high fever. She had nursed to her daughter and inserted suppositories to her daughter several times. S. Typhi was also detected from her blood. No other member of her family was infected with S. Typhi. From the study of the route of the infection, we are confident that S. Typhi was delivered from daughter to mother by insertion of suppositories.

The outcome of labor induced by use of a glyceride-based vaginal suppository of prostaglandin E2 (PGE2) inserted 3 hours before amniotomy, when the cervix is favorable, has been assessed. A 51 year old Yugoslavian patient was admitted to our hospital in reduced general condition with distinct hypocalcemia, osteomalacia, and with rectum stenosis. Our investigations led to the diagnosis of a malabsorption syndrome due to non-tropical sprue. The most likely cause of the rectum stenosis is an abuse of ergotamine-containing suppositories for several years. A gluten-free diet and the interruption of the use of the suppositories improved her general condition remarkably.
