The use of vancomycin powder reduces surgical reoperation in posterior instrumented and noninstrumented spinal surgery.

BACKGROUND: Surgical site infections can complicate posterior spine surgery. Multiple hospital admissions may be required to adequately treat a surgical site infection, which is associated with increased costs and lower patient satisfaction. The objective of this study was to evaluate the efficacy of prophylactic intra-wound vancomycin powder in reducing the incidence of repeat surgery for infections after posterior instrumented and noninstrumented spine surgery. METHODS: A series of consecutive patients who underwent instrumented or noninstrumented posterior spine surgery for any indication

Posterior lumbar interbody fusion and posterior instrumentation in the surgical management of lumbar tuberculous spondylitis.

There are few reports of the treatment of lumbar tuberculous spondylitis using the posterior approach. Between January 1999 and February 2004, 16 patients underwent posterior lumbar interbody fusion with autogenous iliac-bone grafting and pedicle screw instrumentation. Their mean age at surgery was 51 years (28 to 66). The mean follow-up period was 33 months (24 to 48). The clinical outcome was assessed using the Frankel neurological classification and the Kirkaldy-Willis criteria. On the Frankel classification, one patient improved by two grades (C to E), seven by one grade, and eight showed

Patients' experience of pain and discomfort during instrumentation in the diagnosis and non-surgical treatment of periodontitis.

BACKGROUND: The present multicenter study aimed to survey the patients' experiences in relation to the diagnosis and non-surgical treatment of periodontitis according to current treatment routine. METHODS: Patients (N = 268) treated for plaque-related periodontitis were enrolled in two groups: group 1 received primary probing of pocket depths (PD 1) and/or primary scaling and root planing (SRP 1); group 2 consisted of patients who were scheduled in the different centers for recall PD/scaling/subgingival instrumentation. Data collection included a full periodontal status, anesthesia employed,

Rapid Fire Hair Implanter Carousel. A new surgical instrument for the automation of hair transplantation.

BACKGROUND: As hair transplantation evolved into a procedure in which large numbers of very small grafts are moved in a single session, new problems have emerged. These include greater staffing requirements, longer operating time, additional technical difficulties, and increasing problems with quality control. OBJECTIVE: To introduce a new surgical instrument, the Rapid Fire Hair Implanter Carousel (Carousel), which can automate the most labor-intensive parts of the hair transplantation process, site creation and implant placement, by combining them into a single step and delivering them in r

[Finger-free speech following total laryngectomy. Instrumentation and technic of surgical voice rehabilitation].

Surgical voice restoration during and after total laryngectomy using Blom-Singer puncture and the glottoplasty techniques provide good results. There is no doubt that the proportion of patients with a good voice is higher after surgical voice restoration than with the use of the esophageal speech or electronic speaking aids. Nevertheless there are problems due to the shunt or the necessary daily changing of the prosthesis with its risks, and failures due to the tracheostomy, the prosthesis and the pharynx. In order to reduce these surgical problems we developed new instruments, a personal surg

TD-SAM: Temporal and Distance-Guided Adaptations of SAM for Accurate Surgical Instrument Segmentation.

Accurate automatic surgical instrument segmentation plays a crucial role in robot-assisted surgery, but analyzing surgical videos remains challenging due to factors such as rapid instrument movements, high inter-category similarity, and frequent object occlusions. Current surgical instrument segmentation models struggle to capture both inter-frame variations and intra-frame details in complex surgical scenarios. The Segment Anything Model (SAM) has shown significant potential in various segmentation tasks. However, it has not fully addressed the unique challenges posed by surgical videos. To t

Surgical Site Infections Following Spinal Instrumentation: A Review of Risk Factors and Treatment Options.

Postoperative infection of the instrumented spine carries serious morbidity, great cost, and even failure of the original objective. In the present review, we discuss the predisposing risk factors, analyze the effectiveness of preventive measures, and evaluate evidence-based treatment options for surgical site infections (SSIs) in spinal instrumentation procedures. Several risk factors are identified. Preoperative factors include concomitant infections, immunocompromised host conditions, advanced age, obesity, malnutrition, uncontrolled diabetes mellitus, tobacco use, and previous spinal surge

Surgical outcomes after instrumented lumbar surgery in patients of eighty years of age and older.

BACKGROUND: In Taiwan, the life expectancy of an 80-year-old man is 88.4 years and the life expectancy of an 80-year-old woman is 89.8 years. Some of these people will develop symptomatic degenerative lumbar diseases that interfere with an active lifestyle. These older surgical candidates usually ask the surgeon whether it would be safe to undergo surgery. However, there is no literature assessing the outcomes of laminectomy, fusion and posterior fixation for degenerative lumbar diseases in patients older than 80 years. The purpose of this study was to report the surgical outcomes of patients

Postoperative deep surgical-site infection after instrumented spinal surgery: a multicenter study.

A retrospective survey revealed 37 cases (1.1%) of deep surgical-site infection (SSI) among 3,462 instrumented spinal surgeries between 2004 and 2008. Excluding 8 patients who were unclassifiable, we categorized 29 patients into 3 groups of similar backgrounds-thoracolumbar degenerative disease (the DEG group; n = 15), osteoporotic vertebral collapse (the OVC group; n = 10), and cervical disorders (the cervical group; n = 4)-and investigated the key to implant salvage. Final respective implant retention rates for the groups were 40, 0, and 100%, with the OVC group having the worst rate (p < 0.
