The fate of preserved sphincter in rectal cancer patients.

PURPOSE: Sphincter-saving surgery is widely accepted operative modality to treat rectal cancer. It often requires temporary diverting stoma to avoid the complications of anastomotic failure. This study investigates the cumulative failure rate in sphincter preservation for rectal cancer and the risk factors associated with the permanent stoma. METHODS: A retrospective study on 358 patients diagnosed with primary rectal cancer from 2009 to 2013 was conducted at a single institute. Three hundred and thirty-one out of 358 patients with rectal cancer located within 12 cm from the anal verge, who u

The role of faecal diversion in low rectal cancer: a review of 1791 patients having rectal resection with anastomosis for cancer, with and without a proximal stoma.

AIM: The morbidity of anastomotic dehiscence may be mitigated by a defunctioning stoma, but it is unclear if it is required for most low rectal anastomoses. Preoperative risk factors leading to anastomotic complications and the indications for faecal diversion have yet to be clearly defined. METHOD: Using the American College of Surgeons-National Surgical Quality Improvement Project (ACS-NSQIP) participant-use file, patients were identified who underwent low anterior resection with anastomosis for cancer at the 211 participating hospitals in 2005-08. RESULTS: A total of 1791 patients underwe

Complications associated with loop ileostomy: analysis of risk factors.

BACKGROUND: Loop ileostomy is a common surgical procedure but is associated with complications such as outlet obstruction (OO), parastomal hernia (PH), and high-output stoma (HOS). This study aimed to identify risk factors for these complications, as well as their causal relationships. METHODS: The study included 188 consecutive patients who underwent loop ileostomy between April 2016 and September 2021. Clinical factors and postoperative stoma-related complications (OO, HOS, and PH) were analyzed retrospectively. Stoma-related factors were evaluated using specific measurements from computed

Concealed umbilical stoma: long-term evaluation of stomal stenosis.

PURPOSE: The technique of forming a concealed umbilical stoma has been described previously and includes a posterior umbilical flap for improved cosmesis and stenosis prevention. We assessed long-term stomal stenosis. MATERIALS AND METHODS: We reviewed retrospectively the charts of 46 patients (mean age at surgery 14 years) of whom 35 had undergone concealed umbilical stoma creation and 11 the Malone antegrade continence enema procedure for continent urinary diversion. Urinary stomas were created from appendix in 20 cases, ileum in 8, sigmoid colon in 5, bladder in 1 and stomach in 1. Malone

Psychosocial effects and quality of life after stoma surgery: systematic review and qualitative meta-synthesis.

BACKGROUND: Although stoma surgery is a life-saving intervention, it can lead to significant physical, emotional, and social changes that affect individuals’ psychosocial well-being and overall quality of life. This qualitative meta-synthesis aims to systematically investigate and synthesize the psychological and social impacts, as well as the quality-of-life experiences, of adult individuals living with a permanent stoma due to colorectal cancer. METHODS: This qualitative meta-synthesis was conducted via systematic review and thematic synthesis methods, following the ENTREQ, PRISMA, and JBI g

Standardizing Peristomal Hygiene: Reducing Variation and Complications in Ostomy Care.

Peristomal skin complications (PSCs) remain a common, costly, and underrecognized challenge in ostomy care, driven by inconsistent education, variable clinical practices, and the absence of standardized terminology and protocols. These complications affect a substantial proportion of people living with an ostomy and are associated with leakage, pain, psychological distress, reduced health-related quality of life (QoL), and preventable health care utilization. To address this global gap, an international panel of ostomy care experts convened as the Peristomal Hygiene Advancement in Skin Educati

Understanding the Impact of Ostomy Dejecta Constituents on Peristomal Skin Health and Models for Its Characterisation.

An ostomy, or stoma, is a surgically created percutaneous aperture from a hollow organ (e.g., small intestine) to the body's surface. Physicians may recommend an ostomy as a temporary or permanent solution to a range of disorders of the gastrointestinal tract, with up to 130 000 ostomies performed annually in the United States. An ostomy facilitates the expulsion of waste products, termed dejecta and circumvents the compromised organs. While an ostomy can be a lifesaving treatment, it is a disruption of regular digestive flow and has a number of associated complications including hernia, prola

[Enterostomy based on abdominal wall tension and fascial locking: a theory of preventing stoma complications and parahernia].

No consensus on standardized technique of enterostomy creation has been made meanwhile high heterogeneity of surgical procedure exists in 'stoma creation' chapters of textbooks or atlases of colorectal surgery. The present article reviews the anatomy of tendinous aponeurotic fibers which is crucial for abdominal wall tension and integrity. Through empirical practice we hypothesize a procedure of enterostomy creation basied on abdominal wall tension plus anchor suture for fascia fixation which could theoretically decrease short-term stoma complication rates and long-term parastomal hernia rates

Percutaneous trans-stomal jejunostomy: a new technique.

AIM: We propose a new technique for feeding a malnourished patient with a "high" double-barrel jejunostomy (at about 60 cm from the Treitz Ligament). The procedure aims to restore an adequate nutritional state maintaining a correct diet for 24 h a day, without complications and without interfering with the normal activity of the nurses caring for the stoma. METHOD: Using local anesthesia, we introduced a Reverdin needle through the efferent loop of jejunostomy and externalized it through the skin, medially from jejunostomy of about 10 cm. Using this guide, we inserted an enteral feeding tube

How to do a laparoscopic repair of a post-hysterectomy rectovaginal fistula?

The exact incidence of iatrogenic post-hysterectomy rectovaginal fistula is unknown. The overall surgical management for this group of patients is different from rectovaginal fistula in general. These are high fistula presenting with a diversion stoma in situ and are characterised well on a rectal contrast pelvic computed tomography (CT). We have described the laparoscopic repair of these fistulae.

Systematic Review and Meta-Analysis of Extraperitoneal Versus Transperitoneal Colostomy for Preventing Parastomal Hernia.

BACKGROUND: Parastomal hernia remains a frequent problem after constructing a colostomy. Current research mainly focuses on prophylactic mesh placement as an addition to transperitoneal colostomies. However, for constructing a colostomy, either an extraperitoneal or transperitoneal route can be chosen. OBJECTIVE: The aim of this meta-analysis was to investigate which technique results in lower parastomal hernia rates in patients undergoing end colostomy. DATA SOURCES: A meta-analysis was conducted according to Preferred Items for Reporting of Systematic Reviews and Meta-Analyses and Meta-Ana

Emergency Airway Management in a Patient With Tracheoesophageal Fistula and Prior Laryngectomy.

BACKGROUND: Airway management of the patient with a history of total laryngectomy is complicated by iatrogenic changes to the patient's upper airway. Securing the airway through the pharynx is impossible because of surgical closure, and front-of-neck surgical airway is not feasible due to the existing tracheostomy. The only viable option is via the existing tracheal stoma. Further complicating this scenario, patients who have had an insertion of a tracheoesophageal prosthesis (TEP), inherently have an iatrogenic tracheoesophageal fistula (TEF). We present a case of airway management in a patie

The Use of an Aspirating/Resecting Device to Reduce Stoma Closure Following Endoscopic Third Ventriculostomy for Aqueductal Stenosis.

BACKGROUND: Endoscopic third ventriculostomy (ETV) is commonly used to treat obstructive hydrocephalus. Closure of the stoma can be associated with symptom recurrence and need for further surgical intervention. OBJECTIVE: To describe the use of a side-cutting aspiration device for treatment of aqueductal stenosis in patients undergoing ETV. METHODS: A retrospective review of 30 consecutive adults with aqueductal stenosis treated with ETV using an adjunct side-cutting aspiration device between 2011 and 2013 was performed. Patients included in the study ranged from 35 to 64 years of age. ETV s

Pitfalls and technical errors in the first approach to neonates with anorectal malformations in a non-specialist context: can we do any better? A review from three Eastern African Centres.

INTRODUCTION: In sub-Saharan Africa, Anorectal malformations (ARM) are the most frequent cause of neonatal obstruction. Referral to a Pediatric Surgeon is frequently delayed. The first treatment is often delivered at not specialist level and mismanagement may result. AIM: To study ARM patients referred beyond neonatal period and managed at a non-specialist level. MATERIALS AND METHODS: One hundred and thirty patients were included (M/F ratio 63/67) among 144 admitted to three Eastern African Hospitals with Pediatric Surgical facilities. Demographics, type of anomaly, delay on referral, previ

Incidence and impact of peristomal candidiasis on outcomes after intestinal stoma closure: A prospective study.

An intestinal stoma is often created to protect a distal anastomosis. Factors affecting morbidity and mortality of stoma closure have been well studied; however, the role of fungal infection in peristomal skin has not. This prospective, observational study was conducted between August 2022 and July 2024 to determine the incidence of fungal infection in peristomal skin, in patients undergoing stoma closure, and its association with postoperative complications such as surgical site infection (SSI), anastomotic-leak, bowel obstruction/ileus and mortality. The incidence of paralytic ileus, bowel o

Early postoperative parastomal evisceration after explorative laparotomy: case report of a rare and potentially life-threatening surgical complication.

BACKGROUND: Parastomal evisceration represents a preventable surgical complication that should not occur with appropriate technical diligence and surgical skills. While late parastomal hernias are well described in the literature, there is a paucity of reports on the early postoperative occurrence of parastomal intestinal evisceration. CASE PRESENTATION: An urgent laparotomy was performed on a 58-year-old female patient for an acute cecal perforation with generalized peritonitis related to underlying colon cancer. Intraoperative revelations necessitated a carcinologic right colectomy and the

Prevention and management of anastomotic leakage after colorectal resection: evolving trends among Italian surgeons. A SICCR survey.

Anastomotic leakage (AL) remains one of the most feared complications after colorectal resection. Although several strategies have been introduced in the fields of AL prevention, diagnosis and management, their application in clinical practice is still to be assessed. A 26-item-questionnaire was administered between December 2023 and March 2024 to residents in general surgery and surgeons to analyse the actual clinical practice in Italy, in general and with respect of surgical experience in colorectal surgery. A total of 414 responses were analyzed. Most respondents were general surgeons/resid

Colonic stenting as a bridge to surgery in malignant large-bowel obstruction: a report from two large multinational registries.

OBJECTIVES: To date, this is the largest prospective series in patients with malignant colorectal obstruction to evaluate the effectiveness and safety of colonic self-expanding metal stents (SEMSs) as an alternative to emergency surgery. SEMSs allow restoration of bowel transit and careful tumor staging in preparation for elective surgery, hence avoiding the high morbidity and mortality associated with emergency surgery and stoma creation. METHODS: This report is on the SEMS bridge-to-surgery subset enrolled in two multicenter international registries. Patients were treated per standard of pr

Feasibility and outcomes of robotic sphincter-preserving surgery for rectal cancer after neoadjuvant treatment in patients with preexisting colostomy.

BACKGROUND: Diverting colostomy followed by neoadjuvant treatment is a treatment of choice for obstructive rectal cancer. Such patients may be treated via a robotic approach with several advantages over conventional laparoscopic surgery. Conversely, the existing stoma may interfere with the optimal trocar position and thus affect the quality of robotic surgery. Moreover, the console surgeon does not face the patient, which may endanger the stoma. METHODS: Patients with rectal cancer who underwent sphincter-preserving surgery were retrospectively investigated using a robotic platform after neo
