[Pigmented villonodular synovitis: apropos of 3 cases].

Three cases of pigmented villonodular synovitis treated in recent years are described. One case presents a malignant transformation and raises the question of the benign character of these lesions. The therapeutic options are considered as well as the need for a larger series of patients to define therapeutic strategy.

Silicone synovitis: MR imaging in five patients.

Silicone synovitis is a known complication of silicone implants used in orthopedic surgery for joint reconstruction. It has been studied with routine radiography; however, no report on magnetic resonance imaging abnormalities of this condition exist in the literature. This article reports on five patients with silicone synovitis studied by magnetic resonance imaging. All patients showed hypointense implants that were deformed, fragmented or subluxed, and intra- and periarticular silicone particles were evident on T1- and T2-weighted images.

Synovitis secondary to silicone elastomeric joint implant.

The authors present a case study of synovitis after an arthroplasty of the first metatarsophalangeal joint and placement of a hemisilicone joint implant. This was an unusual presentation because excision of the prosthesis was necessary 1 month after placement. Also, a review of the literature is presented on silicone synovitis and other complications that arise after silicone joint implantation.

Silicone synovitis: clinical, radiologic, and histologic features.

We have described a patient in whom an erosive arthropathy of the right wrist developed three years after insertion of a silicone lunate prosthesis. The clinical picture of silicone synovitis is being recognized more frequently after bone and joint implants, but the diagnosis may be delayed because initial suspicions focus on infection or crystal-induced arthropathy rather than foreign body reaction.

Pigmented villonodular synovitis. Diagnosis and differential diagnosis.

PVNS is a proliferative disorder that can affect joints, tendons and bursae. Its etiology is unknown. The most widely accepted theories attribute this disorder to a chronic inflammatory response or a benign neoplasm of fibrohistiocytic origin. On gross specimens, the synovial changes are characterized by villous and/or nodular hyperplasia. This disorder occurs most frequently in the third or fourth decades of life and has no sex predilection. As a joint problem, it favors the knee and hip. As a tendon abnormality, it favors the digits. Plain film findings include 1) soft tissue swelling, 2) in

Familial granulomatous synovitis, uveitis, and cranial neuropathies.

A family is presented that had what is believed to be a previously undescribed syndrome of granulomatous synovitis, bilateral recurrent uveitis, and cranial neuropathies. Affected members included the proband, his brother, father, and probably the decreased paternal grandmother. Disease onset was in childhood. Each had symmetric, boggy polysynovitis of the hands and wrists, resulting in nearly identical boutonniere deformities. Hand radiography in the proband and his brother revealed no erosions or joint destruction despite more than 20 years of disease. Synovectomy specimens in the proband an

Knee Tuberculosis Presenting As Pigmented villonodular Synovitis.

INTRODUCTION: Parsonage-Turner syndrome is a rare condition that is often misdiagnosed by physicians due to the overlapping symptoms with other conditions such as rotator cuff injury and cervical radiculopathy. The etiology of the Parsonage-Turner syndrome is unknown, but has been associated with an immune-mediated response to rheumatic disease, infection, surgery, and immunizations. CASE PRESENTATION: A 18-year-old female former tennis player with a history of complex regional pain syndrome (CRPS), Ehler-Danlos syndrome (EDS), and two prior right shoulder surgeries presented to the orthopaed

Post-arthrogram synovitis: MRI and histopathologic findings.

A 57-year-old patient developed severe, persistent pain following MR arthrography with iodinated contrast. MRI 1 week later showed synovitis which was new compared to the prior MRI. Arthroscopy showed severe synovitis. Histopathology showed synovitis characterized by lymphocytes, neutrophils, and necrosis. One out of 4 intraoperative cultures was positive, but ultimately believed to be due to contaminants. CRP normalized within 1 month. Repeat MRI 2 years later showed progressive degenerative findings, but no evidence of ongoing infection, or stigmata of previous infection. We believe this to

Psoriatic Synovitis: Singularity and Potential Clinical Implications.

Psoriatic arthritis (PsA) is an immuno-inflammatory disease with a heterogeneous clinical presentation as affects musculoskeletal tissues (arthritis, enthesitis, spondylitis), skin (psoriasis) and, less frequently, eye (uveitis) and bowel (inflammatory bowel disease). It has been suggested that distinct affected tissues could exhibit different immune-inflammatory pathways so complicating the understanding of the physiopathology of psoriatic disease as well as its treatment. Despite of the key pathogenic and clinical relevance that enthesitis has in PsA, peripheral arthritis is more easily perc

Diffuse Pigmented Villonodular Synovitis Around the Ankle.

BACKGROUND: Pigmented villonodular synovitis (PVNS) is a rare disorder around the ankle joint. The optimal treatment for diffuse-type PVNS is still controversial because of the high incidence of recurrence. We present the clinical features of our patients and review the current diagnostic and treatment modalities. METHODS: Five patients with PVNS located around the ankle were surgically treated. In three patients, diffuse PVNS arose from the ankle joint, and in the other two it arose from the calcaneocuboid and intercuneiform joints. The average follow-up time after surgery was 2.9 years (ran

Remitting seronegative symmetrical synovitis with pitting oedema.

Remitting seronegative symmetrical synovitis with pitting oedema (RS3PE) is a rare clinical entity often mimicking more common diseases like rheumatoid arthritis (RA) and polymyalgia rheumatica (PMR). Although the exact aetiology is not clear, yet it responds dramatically to low doses of steroids with excellent prognosis unlike RA and PMR. We report a case of 72 year old male agriculturist by profession presenting with acute polyarthritis and pitting oedema of both hands and feet as well as pretibial areas, diagnosed to be a case of RS3 PE.

Adult multifocal pigmented villonodular synovitis--clinical review.

Pigmented villonodular synovitis (PVNS) is a rare, benign proliferative disease of the synovial tissue that affects a single joint or a tendon sheath. Data from the literature present only a few cases of multifocal PVNS. This paper presents multifocal PVNS in the adult. This disease can affect bilateral shoulders, hips and knees. The diagnosis may be delayed by the slow evolution of the disease (up to ten years); some patients may be seen with late-stage degenerative joints, serious complications, painful and functionally uncompensated, with significant locomotion deficit. PVNS requires a radi

Sweet's syndrome associated with pigmented villonodular synovitis.

Sweet's syndrome (acute febrile neutrophilic dermatosis) was first described in 1964. The typical symptoms of Sweet's syndrome are high temperature, peripheral leucocytosis, painful cutaneous rashes (papules, plaques) and arthralgia. Sweet's syndrome has particularly been described in association with neoplastic, infectious and immunological diseases. The pathogenesis of Sweet's syndrome can be explained by a reaction to an antigenic structure with accumulation of immunological complexes and liberation of inflammatory mediators. For the first time we report on a patient with Sweet's syndrome a

Vascular changes in psoriatic knee joint synovitis.

OBJECTIVE: To evaluate the diagnostic utility of standard arthroscopy supported by a computerized image analysis system; and to examine and quantify the macroscopic appearance of blood vessels in selected anatomical areas, comparing 2 groups of patients with PsA and RA with refractory knee joint synovitis (KJS) for vascular marking (VM) features and VM scores, as well as for the relationship between respective VM scores and local and systemic KJS disease activity indices. METHODS: Standard arthroscopy was carried out on 39 knees (20 PsA, 19 RA). Videorecordings of the examination were reanaly
