Reciprocating supraventricular tachycardia in children: low rate at rest as a major factor related to propensity to syncope during exercise.

Reciprocating supraventricular tachycardia may have several clinical presentations, with symptoms often more severe during exercise or emotional stress. This study shows by using transesophageal atrial pacing, the factors related to syncope during exercise. Between May 1989 and June 1994, transesophageal atrial pacing was performed at rest and during exercise in 75 children aged > 6 years with suspected or documented episodes of paroxysmal supraventricular tachycardia. Reciprocating supraventricular tachycardia could be induced both at rest and during exercise in 22 patients (8 girls, 14 boys;

Electrophysiologic approach to diagnosis and ablation of patients with permanent junctional reciprocating tachycardia associated with complex anatomy and/or physiology.

INTRODUCTION: Permanent junctional reciprocating tachycardia (PJRT) is a rare supraventricular tachycardia (SVT), typically involving a single decremental posteroseptal accessory pathway (AP). METHODS: Four patients with long RP SVT underwent electrophysiology (EP) study and ablation. The cases were reviewed. RESULTS: Case 1 recurred despite 3 prior ablations at the site of earliest retrograde atrial activation during orthodromic reciprocating tachycardia (ORT). Mapping during a repeat EP study demonstrated a prepotential in the coronary sinus (CS). Ablation over the earliest atrial activati

Accessory pathway potential recording in a case of permanent junctional reciprocating tachycardia with decremental conduction localized on the atrial site.

Permanent junctional reciprocating tachycardia (PJRT) is an uncommon form of atrioventricular reentrant tachycardia due to the presence of an accessory pathway characterized by slow and decremental retrograde conduction. We report a case of PJRT where we demonstrated the possibility of recording a distinct accessory pathway potential. Decremental retrograde conduction was evident using ventricular extrastimuli and it was also adenosine-sensitive. Delivering ventricular extrastimuli a prolongation of the accessory pathway potential-atrium interval was seen demonstrating that decremental conduct

Atrioventricular block in the atypical form of junctional reciprocating tachycardia: evidence supporting the atrioventricular node as the site of reentry.

Serial electrophysiologic studies were performed in 19 patients with the atypical form of supraventricular tachycardia having a long RP and short PR interval. In all 19 patients, supraventricular tachycardia was found to have a 1:1 P-QRS relation during initial control electrophysiologic studies, and in all 19 patients, electrophysiologic studies suggested that junctional reentry was the mechanism of supraventricular tachycardia. Seven of the 19 patients developed atrioventricular (AV) block during initiation of supraventricular tachycardia or after induction of supraventricular tachycardia fo

[Idionodal reciprocal tachycardia irresponsive to pharmacological prophylaxis. Description of a case successfully treated with radio frequency].

The case of a 63-year old female suffering from recurrent paroxysmal supraventricular tachycardia for 30 years is reported. In the past she was treated unsuccessfully with multiple drugs. Electrophysiological study showed dual nodal pathways and the inducibility of a junctional reciprocating tachycardia with RR and V-A intervals respectively 360-410 and 40 msec. Radiofrequency was applied for 30" in the nodal region during induced reciprocating tachycardia. The arrhythmia was interrupted after a few seconds and a 1st degree A-V block persisted for a few minutes. Repeated electrophysiological s

Ablation of atrioventricular nodal "slow pathway" for simultaneous treatment of coexisting atrioventricular and nodal reciprocating tachycardias.

INTRODUCTION: We report the case of a 49-year-old male patient with recurrent palpitations and two different supraventricular reciprocating tachycardias due to atrioventricular (AV) nodal reentry and orthodromic AV reentry sustained by a left-sided, concealed AV accessory pathway (AP). METHODS AND RESULTS: During the baseline electrophysiological study, dual AV nodal conduction (90 ms jump) and non-decremental, eccentric, ventriculo-atrial conduction due to a left-sided, unidirectional, postero-septal AP were documented. Both typical AV nodal reentrant and orthodromic AV reentrant tachycardia

[Latent Clerc-Levy-Cristesco syndrome as an electrophysiologic basis for paroxysmal reciprocal atrioventricular nodal tachycardia].

An intracardiac electrophysiological investigation was performed in 15 patients with frequent episodes of supraventricular tachycardias (SVT) which are regarded, in terms of the present electrophysiological criteria, as episodes of reciprocal atrioventricular nodal tachycardia. It was demonstrated that accessory atrionodal junctions functioning only in the retrograde direction formed the basis for arrhythmias in 6 patients. This electrophysiological phenomenon was proposed to be termed the latent Clerc-Lery-Cristesco (CLC) syndrome. The main electrophysiological criteria for diagnosing the syn

First postpacing interval after tachycardia entrainment with correction for atrioventricular node delay: a simple maneuver for differential diagnosis of atrioventricular nodal reentrant tachycardias versus orthodromic reciprocating tachycardias.

BACKGROUND: The difference between the first postpacing interval (PPI) after tachycardia entrainment from the right ventricular apex and the tachycardia cycle length (TCL) can be used as an index of proximity to the circuit. OBJECTIVES: The purpose of this study was to determine whether the response to entrainment of tachycardia during ventricular stimulation with correction for AV node delay is a useful, simple maneuver for differentiating AV nodal reentrant tachycardia (AVNRT) from orthodromic reciprocating tachycardia (ORT) using a concealed accessory pathway. METHODS: The study consisted

Electrophysiological findings and long-term follow-up of patients with the permanent form of junctional reciprocating tachycardia treated by catheter ablation.

BACKGROUND: The permanent form of junctional reciprocating tachycardia (PJRT) commonly presents as recurrent drug-refractory, narrow-complex tachycardia. We studied the efficacy and safety of catheter ablation in treating these patients. METHODS AND RESULTS: Six patients with the diagnosis of PJRT were treated at our institution with direct-current catheter ablation. The study cohort comprised three men and three women with a mean age of 33.8 +/- 4.5 years. The mean time from onset of symptoms to ablation was 129 +/- 44.7 months. All failed multiple drug therapy (mean number of drugs failed w

Transjugular approach for radiofrequency ablation of permanent junctional reciprocal tachycardia in a newborn with bilateral femoral vein thrombosis.

Although radiofrequency ablation is the first line therapy in some children with supraventricular tachycardia, its application in small children is still limited. Herein, we presented a premature newborn diagnosed as multidrug-resistant permanent junctional reciprocal tachycardia, and treated by radiofrequency ablation via the jugular vein approach because of bilateral femoral vein thrombosis. We think that when there is limited vascular access, the transjugular route for radiofrequency ablation might be considered as an alternative treatment in newborns with multidrug-resistant supraventricul

A case of concealed WPW syndrome with three different reciprocal tachycardias due to triple AV nodal pathways.

Triple atrioventricular nodal pathways (TAVNP) occur occasionally, but it is rare for them to produce more than two different tachycardias. Here we report a case of concealed WPW syndrome with three different tachycardias. During electrophysiologic studies, three different reciprocal tachycardias were induced. Tachycardia #1 was characterized by a cycle length of 230 msec and an A'-H interval of 70 msec. For tachycardia #2, these parameters were 300 msec and 140 msec, while they were 370 msec and 200 msec for tachycardia #3. During all three tachycardias, the earliest atrial activity was obser

[Serial testing of anti-arrhythmia preparations using the intracardiac electrophysiological study of patients with reciprocal paroxysmal tachycardias].

Diagnostic investigation of 36 patients with reciprocal paroxysmal tachycardias, using intracardiac electrophysiologic tests, revealed supraventricular paroxysmal tachycardia in 29 patients, and ventricular paroxysmal tachycardia in 7. Serial trials of antiarrhythmic drugs in acute experiments with simultaneous electrophysiologic studies made possible efficient medication in 80% of the patients. A high correlation was demonstrated between the results of acute antiarrhythmic drug tests and subsequent long-term treatment. Ritmilen proved particularly effective in patients with paroxysmal tachyca

[Possibilities of the combined use of transesophageal electrostimulation of the heart and physical load in the diagnosis of paroxysmal reciprocal atrioventricular tachycardia].

Possibilities of using transesophageal electrostimulation of the heart in combination with a physical load were studied in 38 patients with paroxysms of reciprocal atrioventricular (AV) tachycardia. The study has shown that the physical load exerts a pronounced influence on the induction, persistence, and electrophysiological parameters of AV tachycardia, significantly increases the functional capacity of the anterograde conduction along the anomalous tract, and reduces the efficacy of vagal techniques in alleviating paroxysms of AV tachycardia.

[Clinical Case of Severe Heart Failure Treatment, Produced due to the Development of Arrhythmogenic Cardiomyopathy in a Patient With Permanent Junctional Reciprocating Tachycardia (Coumels Tachycardia) With Additional Left Lateral Accessory Pathway].

UNLABELLED: Thirty two years old man had history of atrioventricular re-entry tachycardia (AVRT) with concealed left-lateral accessory pathway (AP), 3-5episodes per year, 30-40 minutes duration without hemodynamic compromise. Two years ago patient underwent ablation of concealed left lateral AP. After that tachycardia became malignant (high-frequent 150 beats-min, incessant (lasting up to 18 h/day), resistant to AAD, and led to development of tachycardia-induced cardiomyopathy (EF was 16%, and 2 episodes of pulmonary oedema). During redo EPS we verified AVRT with conduction via decremental ret

Effect of atrioventricular interval during pacing or reciprocating tachycardia on atrial size, pressure, and refractory period. Contraction-excitation feedback in human atrium.

To determine whether a contraction-excitation feedback mechanism exists in human atrium, we investigated the effects of varying the atrioventricular (AV) interval from 0 to 360 msec during AV pacing at a cycle length of 400 msec on atrial pressure, size, and refractoriness in 10 patients (group 1, without supraventricular tachycardia). The same parameters were determined in another 10 patients (group 2, with different spontaneous AV relations) during AV reciprocating tachycardia or AV nodal reciprocating tachycardia and during high right atrial (RA) pacing at the tachycardia cycle length. Studies on the prevalence of complicated atrial arrhythmias, flutter, and fibrillation in patients with reciprocating supraventricular tachycardia before and after successful catheter ablation.

Atrial flutter and AF are complications in approximately 30% of cases of paroxysmal supraventricular tachycardia (PSVT)-indicated catheter ablation, and it is of interest to determine if therapeutic modification for PSVT would eliminate combined atrial tachyarrhythmia like atrial flutter and AF. The aim of this study was to determine the incidence and the risk of atrial tachyarrhythmias after catheter ablation of PSVT. A total of 152 patients (age range 12-74, mean 41 +/- 17 years) with accessory pathway (n = 106) and/or dual atrioventricular nodal conduction (n = 46) were enrolled in a 2-year

[Paroxysmal junctional tachycardia. Determination of the inferior point of junction of the reentry circuit. Dissociation of the intra-nodal reciprocal rhythms].

Stimulation of the bundle of His and of the uppermost portion of the interventricular septum gives us an opportunity to make a precise study of capture phenomena in patients with paroxysmal nodal tachycardia. According to whether the capture is correctly timed, delayed, or unusually premature, the inferior junction point of the reentry circuit can be located precisely by reference to the H wave and the onset of the R wave. Out of a series of 65 patients, only 30 of whom had a true WPW syndrome, it was shown that 43 cases had a bundle of Kent which ensured retrograde conduction during the tachy
