Unilateral two-headed extensor digitorum longus muscle: atypical configuration and clinical implications.

This report describes a new configuration of the extensor digitorum longus (EDL) with two heads and two main tendons divided into four slips. During routine cadaver dissection, unilateral (right) EDL muscle belly and tendon variations were identified. The morphometric measurements of the EDL were conducted using Image J software. Two bellies and two tendons of the EDL muscle were observed in the right leg of an 87-year-old male cadaver. The second belly of the EDL muscle originates 2 cm below the first belly. The tendons originate at the ends of the muscle bellies. First and second tendons spl

Endoscopic Ganglionectomy and Internal Drainage of Hourglass Ganglion of the Tarsal Tunnel.

Tarsal tunnel syndrome is a common entrapment neuropathy that is sometimes elicited by ganglia in the tarsal tunnel. The tarsal ganglion may penetrate through the flexor retinaculum and have an hourglass appearance with components deep and superficial to the retinaculum. The purpose of this Technical Note is to describe technical details of endoscopic management of hourglass ganglion of the tarsal tunnel. This comprises excision of the part of the ganglion superficial to the flexor retinaculum and internal drainage of the deep part of the ganglion to the tendon sheath of the posterior tibial t

Dynamic stabilization of chronic scapholunate dissociation with palmaris longus transfer: a new technique.

This report presents a new procedure of palmaris longus tendon transfer to the scaphoid and lunate, with reconstruction of the scapholunate ligament in chronic scapholunate dissociation. From 1988 to 2006, 10 cases of dynamic stabilization of chronic scapholunate dissociation were treated by palmaris longus tendon transfer to the scaphoid and lunate. The tendon transfer to the scaphoid and the reconstructed scapholunate ligament appeared to create a resultant force that corrected the scaphoid drift. The transfer to the lunate neutralized its dorsal intercalary segment instability posture. A no

[A case of delayed radiation lumbo-sacral plexopathy].

We report a 47-year-old woman who developed a slowly progressive lumbosacral plexopathy with mixed sensorimotor losses in the lower extremities. The symptoms were apparent 8 years after x-ray irradiation for an ovarian carcinoma. Neurological examination showed mild weakness and absent deep tendon reflexes of bilateral lower extremities, and hypesthesia to all modalities in anterior aspects of bilateral lower thighs, in dorsum pedis and soles. Extensive investigations regarding the possibility of tumor recurrence were negative. Computed tomography of pelvis showed abnormal soft tissue densitie

A biomechanical in vitro comparison of open versus percutaneous repair of tendon Achilles.

A study was undertaken to compare the in vitro strengths of percutaneous Achilles tendon repair to open Achilles tendon repair. Ten fresh frozen cadaver below-the-knee specimens underwent Achilles tenotomy. The specimens were divided into two groups of five specimens each. Group I specimens underwent open Achilles repair using a Bunnell suture technique. Group II specimens underwent percutaneous repair. Each specimen was placed on an apparatus which produced progressive ankle dorsiflexion. Measurements of anastomotic gap were correlated with angle of dorsiflexion. Group I anastomoses were able

Truamatic ophthalmoplegia: a complication of fractures of the zygomaticomaxillary complex. Review of etiology with report of an unusual case.

Ocular muscle imbalance is a common complication in fractures of the zygomaticomaxillary complex. Involvement can be direct; caused by entrapment of either the muscle or tendon in the fracture; or indirect, produced by central or peripheral damage to the cranial nerves supplying the muscles, massive orbital edema or hemorrhage. A case of complete paralysis of the extraocular muscles caused by a critical reduction of the size of the orbit as a result of fracture of the zygomaticomaxillary complex is described.

Traumatic pathologies of the calcaneal peroneal tubercle.

The peroneal tubercle occurs in about one half of human feet. It size varies from a small nodule of less than 1mm height to a prominent spur up to 1cm in size. It had been noted to cause stenosing tenosynovitis of the peroneal tendons, especially in association with an accessory peroneal bone. The current manuscript describes two cases of symptomatic tenosynovitis developing after an ankle sprain with fracture of the peroneal tubercle. The tenosynovitis caused dysfunction of the peroneal tendons unresponsive to prolonged conservative therapy. Symptoms were resolved following surgical resection

Peroneus longus tears associated with pathology of the os peroneum.

BACKGROUND: There is a range of different types of tears and pathology of the peroneal tendons. One of the least common types is the tear of the peroneus longus associated with fracture, enlargement, or entrapment at the cuboid tunnel of the os peroneum. The purpose of this study was to evaluate the pathologic patterns of these uncommon peroneal tendon tears, to review the treatment, and to report the patient outcomes following treatment with excision of the os peroneum, debridement, and tenodesis of the peroneus longus to the peroneus brevis. METHODS: A 5-year retrospective review of all pat

Carpal tunnel syndrome secondary to intratendinous infiltration by tophaceous gout.

Carpal tunnel syndrome caused by gout is rare. We describe a case of entrapment of the median nerve secondary to intratendinous infiltration by gouty tophi in a 54-year-old man. The tophus was excised from the profundus tendon but the superficialis was too infiltrated and destroyed to be saved. The patient made an uneventful recovery with relief of his symptoms. This case illustrates the potential consequences this could have for hand function.

Internal impingement syndromes.

Internal causes of impingement are secondary to rotator cuff and capsular dysfunction and are categorized by the location of the impingement and the underlying pathophysiological or mechanical cause of the impingement. These include posterosuperior impingement, anterosuperior impingement, anterior impingement, and entrapment of the long head of the biceps tendon. The objective of this article is to review magnetic resonance imaging findings of each of the 4 types of internal impingement syndromes and discuss the pathophysiology behind the impingement.

[Hindfoot pain].

The hindfoot is the part of the foot which is proximal to the midtarsal joint. The obvious causes of pain are not considered (post-traumatic etiologies, sprains and fractures but also cutaneous lesions). The main etiologies on the subject are successively exposed by following the localization of the pain. Diffuse pains (ankle arthritis tarsal osteoarthritis, algodystrophy, calcaneo-navicular synostosis but also bone diseases like stress fractures, Paget disease or tumors). Magnetic resonance imaging of the wrist and hand.

The anatomy of the wrist and hand is complex and contains numerous small structures. Magnetic resonance imaging (MRI) is often an ideal imaging modality in the assessment of various traumatic and pathologic conditions of this region, and it is frequently performed after initial radiographs. In this manuscript we describe the normal anatomy, imaging techniques, and MRI findings of various traumatic and pathologic conditions of the wrist and hand including occult fractures, osteonecrosis, ligamentous and tendon injuries, and entrapment neuropathies.

Long-term results of surgical release of de Quervain's stenosing tenosynovitis.

The management of de Quervain's disease (DD) is nonoperative in the first instance, but surgery should be considered if conservative measures fail. We present the long-term results of operative treatment of DD. From July 1988 to July 1998, 94 consecutive patients with DD were treated operatively by a single surgeon. There were 80 women and 14 men. Average age at the time of operation was 47.4 years (range 22-76). The right wrist was involved in 43 cases, the left in 51 cases. All operations were done under tourniquet control with local infiltration anaesthesia using a longitudinal incision and

Inveterate Section of Deep Flexor in Pediatric Hand, Periosteum Clues.

Diagnostic delay in flexor tendon injuries is common in pediatric patients, and age influences both recovery and postoperative management. We report the case of a three-year-old girl who presented with the inability to flex the distal interphalangeal joint of the left third finger, two months after the digit became entrapped in a chair. Ultrasonography revealed cortical ossification of the middle phalanx compressing the flexor tendon, without clear evidence of discontinuity. Surgical exploration was undertaken due to a strong clinical suspicion of flexor digitorum profundus rupture, revealing

Cubital tunnel syndrome due to giant cell tumour of tendon sheaths.

Cubital tunnel syndrome is one of the most common entrapment neuropathies in adults. It is mainly caused by the depression of ulnar nerve from normal structures at the elbow area. Despite the fact that several pathgological entities can be potential mechanisms of the syndrome, the pathogenesis due to benign or malignant neoplasms is extremely rare. In the present report we describe the first case of cubital tunnel syndrome due to giant cell tumour of the tendon sheaths.
