Fibrinolytic Agents in Thromboembolic Diseases: Historical Perspectives and Approved Indications.

Fibrinolytic agents catalyze the conversion of the inactive proenzyme plasminogen into the active protease plasmin, degrading fibrin within the thrombus and recanalizing occluded vessels. The history of these medications dates to the discovery of the first fibrinolytic compound, streptokinase, from bacterial cultures in 1933. Over time, researchers identified two other plasminogen activators in human samples, namely urokinase and tissue plasminogen activator (tPA). Subsequently, tPA was cloned using recombinant DNA methods to produce alteplase. Several additional derivatives of tPA, such as te

Evaluating Differential Item Functioning in EQ-5D-5L in Acute Ischemic Stroke: Evidence From the ACT Trial.

BACKGROUND: Health-related quality of life is a key secondary end point in stroke trials. Differential item functioning (DIF) occurs when individuals with the same underlying health-related quality of life interpret and respond differently to questionnaire items, potentially biasing treatment comparisons. This study evaluates DIF in the patient-reported 5-level EuroQOL questionnaire among patients with acute ischemic stroke across age, sex, and treatment groups. METHODS: Data were from the AcT trial (Alteplase Compared to Tenecteplase), a registry-based randomized comparison of alteplase and

Drug Treatment of STEMI in the Elderly: Focus on Fibrinolytic Therapy and Insights from the STREAM Trial.

Elderly patients constitute a large and growing proportion of ST-elevation myocardial infarction (STEMI) patients, yet they have been under-represented or even excluded from reperfusion trials. Despite evidence that fibrinolysis improves outcomes irrespective of age, many elderly STEMI patients still remain undertreated or subject to major delays to primary percutaneous coronary intervention (PCI). The fear of an excessive risk of intracranial hemorrhage (ICH) in these patients can lead to avoidance of potentially life-saving reperfusion treatment, despite the fact that current STEMI guideline

Time-Driven Activity-Based Costing Methodology for Cost Savings in the EMOTE-TNK Study.

OBJECTIVE: To calculate financial savings resulting from early mobilization of patients with acute ischemic stroke receiving tenecteplase (TNK) in the Early Mobilization Outcomes with Tenecteplase Treatment in Acute Ischemic Stroke (EMOTE-TNK) safety and tolerability study. PATIENTS AND METHODS: We used the Time-Driven Activity-Based Costing methodology to compare costs of 180 patients who had early mobilization, defined as occurring between 13 and 24 hours after thrombolysis, with those of patients who had conventional mobilization, defined as waiting 24 hours after thrombolysis. Patients we

Pharmacoinvasive Strategy Versus Primary Percutaneous Coronary Intervention in ST-Elevation Myocardial Infarction in Clinical Practice: Insights From the Vital Heart Response Registry.

BACKGROUND: Recent clinical trial data support a pharmacoinvasive strategy as an alternative to primary percutaneous coronary intervention (pPCI) in ST-segment elevation myocardial infarction. We evaluated whether this is true in a real-world prehospital ST-segment elevation myocardial infarction network using ECG assessment of reperfusion coupled with clinical outcomes within 1 year. METHODS: Of the 5583 ST-segment elevation myocardial infarction patients in the Alberta Vital Heart Response Program (Cohort 1 [2006-2011]: n=3593; Cohort 2 [2013-2016]: n=1990), we studied 3287 patients who rec

Anomalous left anterior descending artery: an uncommon cause of ST elevation myocardial infarction.

A healthy 32-year-old Asian man presented in emergency with a history of severe chest pain. ECG showed normal sinus rhythm with ST elevations in the anterior chest leads. He was diagnosed with anterior wall ST segment elevation myocardial infarction and was thrombolysed with tenecteplase. The cardiac enzymes and troponin level were significantly high and echocardiography demonstrated akinetic anterior segment of left ventricle. Coronary angiography exhibited anomalous left anterior descending artery originating from right sinus of Valsalva without atherosclerotic lesion.

Elevated Factor VIII in the Setting of Recurrent Ischemic Strokes.

We report a case of a 52-year-old female who first developed an acute lacunar infarction in the left thalamus. Two and a half years later, the patient presented with a new infarction due to high-grade stenosis of the mid basilar artery, which required treatment with tenecteplase (TNK) and angioplasty with stenting. Approximately one month following this, the patient developed a new pontine infarction despite a patent basilar artery stent. This prompted a hypercoagulable workup, which revealed an elevated factor VIII level.

Thrombolytic drugs for ischemic stroke: historical perspective, state of play, and future developments.

Thrombolytic agents are serine proteases or related enzymes that facilitate blood clot breakdown via a process called thrombolysis. They work either indirectly, via activation of plasminogen into plasmin-a potent fibrin-degrading enzyme-or by direct dissolution of fibrin within clots. Numerous clinical trials investigating thrombolytic candidates for major thrombotic conditions, such as heart attack and acute ischaemic stroke (AIS), have led to the development of several potential therapies. However, while some drugs show clinical benefits, most still possess substantial limitations, especiall

[Thrombus entrapped in a patent foramen ovale : About a case revealed by a pulmonary embolism at the University Hospital Center of Libreville].

The patent foramen ovale is common in the general population. It is often encountered during paradoxical embolisms. During a pulmonary embolism, its presence is a poor prognosis. The discovery of an enclosed thrombus in this area is rare. We report a case of an enclaved thrombus in the patent foramen ovale discovered during a transthoracic echocardiography indicated in the presence of a suspected pulmonary embolism in a pregnant woman. This patient presented with hemodynamic instability, the evolution of which was favorable after thrombolysis with tenecteplase. In the absence of consensus on t

Innovations in Acute Stroke Reperfusion Strategies.

Vascular neurology is witnessing unprecedented innovations in the management of acute ischemic stroke, especially in reperfusion strategies. The emergence of mechanical thrombectomy with new generation devices has revolutionized the treatment of acute ischemic stroke with large vessel occlusion. The reperfusion strategies are evolving with the extension of the window period for thrombolysis and endovascular therapy through the concept of "tissue clock" in addition to the established "time clock." The newer generation of thrombolytic drugs like tenecteplase are promising exciting times ahead in

Acute ST-segment elevation myocardial infarction: American College of Chest Physicians Evidence-Based Clinical Practice Guidelines (8th Edition).

This chapter about fibrinolytic, antiplatelet, and antithrombin treatment for acute ST-segment elevation (STE) myocardial infarction (MI) is part of the American College of Chest Physicians Evidence-Based Clinical Practice Guidelines (8th Edition). Grade 1 recommendations are strong and indicate that the benefits do, or do not, outweigh risks, burden, and costs. Grade 2 suggests that individual patient values may lead to different choices (for a full understanding of the grading see the chapter by Guyatt et al, CHEST 2008; 133[suppl]:123S-131S). Among the key recommendations in this chapter ar

Concurrent Cardio-Cerebral Infarction - A Case Report and Literature Review.

Concurrent cardio-cerebral infarction (CCI) describes the simultaneous occurrence of an acute myocardial infarction and an acute ischemic stroke. It is a rare phenomenon, and no consensus yet exists on how to best treat it. CCI patients present with variable clinical scenarios and complications which makes the establishment of a treatment guideline difficult. We present here a case of a 67-year-old male with concurrent acute ST-elevation myocardial infarction and acute ischemic stroke due to right middle cerebral artery occlusion who was successfully treated with Tenecteplase and mechanical th

Worldwide Survey on Approach to Thrombolysis in Acute Ischemic Stroke With Large Vessel Occlusion.

BACKGROUND AND OBJECTIVES: With recent trials suggesting that endovascular thrombectomy (EVT) alone may be noninferior to combined intravenous thrombolysis (IVT) with alteplase and EVT and that tenecteplase is non-inferior to alteplase in treating acute ischemic stroke, we sought to understand current practices around the world for treating acute ischemic stroke with large vessel occlusion (LVO) depending on the center of practice (IVT-capable vs IVT and EVT-capable stroke center). METHODS: The electronic survey launched by the Practice Current section of Neurology: Clinical Practice included

Thrombolysis in the Delayed Treatment Window for Acute Ischemic Stroke: Updated Systematic Review and Meta-Analysis.

BACKGROUND: Intravenous thrombolysis is a standard treatment for acute ischemic stroke but has traditionally been limited to 4.5 hours after symptom onset. This time-based restriction may exclude patients with salvageable brain tissue, especially those with wake-up or unknown-onset strokes. Multimodal imaging allows for the identification of viable penumbra beyond traditional treatment windows. This study compiles randomized evidence evaluating intravenous thrombolysis in imaging-selected patients treated after 4.5 hours. METHODS: We conducted a systematic review and meta-analysis in accordan

Sequential Systemic Thrombolysis for Clot-In-Transit after Thrombolytic Failure in Massive Pulmonary Embolism.

INTRODUCTION: A clot-in-transit (CIT) is a mobile thrombus within the right heart associated with acute pulmonary embolism (PE) and high mortality. Although systemic thrombolysis remains the standard treatment for massive or high-risk PE, management of thrombolytic failure and associated CIT remains challenging, particularly when surgical embolectomy and catheter-directed thrombectomy are unavailable. Sequential systemic thrombolysis represents a practical therapeutic option in these high-risk scenarios. CASE DESCRIPTION: A 54-year-old woman with hypertension presented with progressive dyspno
