The traditional role of theophylline as a bronchodilator has been expanded by recent findings that suggest this drug has more than smooth muscle relaxant properties. Twelve healthy subjects received a single oral dose of theophylline, 5 mg/kg alone, and after 7 days of oral verapamil, 120 mg every 8 hours, diltiazem, 90 mg every 8 hours, and nifedipine, 20 mg every 8 hours, in randomized crossover fashion. In patients with acute hepatitis, cholestasis, and compensated or decompensated liver cirrhosis the kinetics and urinary excretion of theophylline and metabolites were investigated following intravenous administration (193 mg). Caffeine (1,3,7-trimethylxanthine) is a biotransformation product of theophylline (1,3-dimethylxanthine) in the human fetus. Liver explants, obtained from human fetuses with gestational ages of 12 to 20 weeks, were incubated with theophylline and produced caffeine and, in lesser amounts, 1,3-dimethyluric acid and 3-methylxanthine. These findings suggest that the predominant pathway in theophylline metabolism in the fetus and newborn infant is the methylation reaction producing caffeine. To the well known methods for prevention of neonatal apnea like stimulation, CPAP, and mechanical ventilation the treatment with theophylline has been added recently. We studied the efficiency of theophylline therapy in 10 premature infants with apnea. The initial dosage was 6 mg theophylline/kg body-weight, followed by a dosage of 2 mg/kg for the next 12 h. The frequency of apnea thereby was significantly reduced. Only in two cases the heart rate rose above 180/min as a side effect. In no case an additional therapy was necessary.

The effect of theophylline on brain monoamine metabolism was studied in rats. Single doses of theophylline caused a striking and dose-related increase in the levels of 3-methoxy-4-hydroxyphenylethylene glycol sulfate (MOPEG-SO4) and 5-hydroxyindoleacetic acid (5-HIAA) in the brain. The effects of various doses of theophylline on the plasma levels of calcium, inorganic phosphorus and magnesium were studied in sheep. The dose of 0.25 mg/kg/min. during 40 min. produced a hypocalcaemia, which was abolished by thyroidectomy. In this paper we describe the kinetics of formation of 1-methylxanthine (1-MX), 3-methylxanthine (3-MX) and 1,3-dimethyluric acid (1,3-DMU) from theophylline in human liver microsomal incubations and use the selective inhibitor approach to define the role of the individual cytochrome P450s (CYP) in each pathway.
