This study presents a rare case of plastic bronchitis (PB) in a 57-year-old adult post-lung lobectomy, a condition predominantly found in post-Fontan children. The patient exhibited progressive dyspnea and complete atelectasis on the surgical side, revealing a gray rubbery bronchial cast obstructing the right main bronchus. Treatment involved repeated bronchoscopies, glucocorticoids, acetylcysteine, physiotherapy, and a low-fat diet, leading to gradual improvement. No similar cases have been reported, highlighting PB's diagnostic challenge. This underscores the need to consider rare conditions

OBJECTIVE: In this study, we aimed to compare the results of patients who underwent surgery by thoracotomy and Video-assisted thoracoscopic surgery (VATS) in mediastinal neurogenic tumors.
MATERIALS AND METHODS: Twenty-six consecutive cases (12 males and 14 females; mean age 39.4 ± 22.3 years; range 1-72 years) who were histopathologically diagnosed as having mediastinal neurogenic tumors between January 2000 and August 2020 were included in a single-center, retrospective study.

The association of an accessory hemidiaphragm and scimitar syndrome is extremely rare and occurs exclusively in the right hemithorax. We present the case of a 27-year-old female, who underwent complete resection of the accessory hemidiaphragm and direct anastomosis of the scimitar vein to the left atrium through a lateral right thoracotomy. This surgical approach offered easy access to both anomalies and allowed single-stage concomitant repair; this approach avoided postoperative respiratory complications and possible torsion or stenosis of the scimitar vein.

Supraventricular arrhythmias are common rhythm disturbances following pulmonary surgery. The overall incidence varies between 3.2% and 30% in the literature, while atrial fibrillation is the most common form. A best evidence topic in cardiac surgery was written according to a structured protocol. The question addressed was: is sternotomy approach superior to a thoracotomy approach for a modified Blalock-Taussig shunt procedure? Reoperations were fulfilled on 71 patients (2.73%) after 2576 thoracotomies for diseases and injuries of the chest, rethoracotomies were fulfilled on 34 patients, in 37 patients the interventions were fulfilled endoscopically. The indication for thoracoscopy were continuing intrapleural bleeding, fragmented pleurisy due to pleural empyema, not arrested chylothorax, foreign body (drainage tube). Nine patients died (24.3%). Lethality was one third less as compared with rethoracotomy.

Intrathoracic dislocation of the scapula is extremely rare. We present the case of a 64 year old man who underwent a lung transplant for emphysema via a standard posterior thoracotomy approach. Four weeks later, following a bronchoscopy, he experienced severe pain and restriction of movement in his shoulder. CT scans revealed intrathoracic dislocation of the inferior angle of the scapula. Two manipulations under anaesthesia were unsuccessful and formal exploration required with closure of the intercostal defect. We describe our surgical technique.

Surgical treatment of spontaneous pneumothorax can be done through a thoracotomy or a video-thoracoscopic approach. Although the videothoracoscopic technique is currently popular it is not obviously superior to a more traditional axillary thoracotomy approach. Stevens-Johnson syndrome (SJS) is an uncommon eruptive disorder of the skin and mucous membranes with systemic manifestation. It is extremely unusual for patients with a past history of SJS to present with indications for surgery necessitating thoracotomy. We describe herein the perioperative management of a patient with SJS who underwent surgery for a spontaneous pneumothorax.

Since 1991 ten patients, 9 male and 1 female, with aortic arch aneurysm underwent surgical therapy with trapdoor thoracotomy. The mean age was 67.4 +/- 9.1 (standard deviation) years at operation. In addition to the aortic arch repair, we also performed seven descending aortic replacements. We performed one partial arch replacement, one total arch replacement, and one aneurysmorrhaphy and wrapping of the aortic arch aneurysm. We experienced 1 case of paraplegia, no hospital death, and no long-term mortality.

In a patient with pectus excavatum, coronary artery bypass grafting with the internal mammary artery is difficult or even impossible through a median sternotomy. We suggest and describe an anterolateral thoracotomy that, in this particular case, not only allowed taking the internal mammary artery but also provided an excellent exposure of the anterolateral aspect of the heart.
