Distal Transradial Approach in Mechanical Thrombectomy: Technical Note.

OBJECTIVE: We report a case of mechanical thrombectomy (MT) via the distal transradial approach (dTRA) and technical tips. CASE PRESENTATION: An 89-year-old woman was transferred to our hospital due to back pain after a fall and sudden-onset left hemiparesis. We performed MT because three-dimensional computed tomography angiography (3D-CTA) revealed right middle cerebral artery (MCA) occlusion. The access route was Type 3 aortic arch. The abdominal aorta and common iliac artery were tortuous and partially dissected, and she had a lumbar vertebra fracture. We selected dTRA in consideration of

Acute Hemispheric Stroke: Full Remission Following Surgical Thrombectomy.

INTRODUCTION: Carotid occlusion because of embolisation or as a distal extension of thrombus formation in an ulcerated plaque can be the cause of a devastating stroke, caused by sudden occlusion of the internal carotid artery (ICA). Often, invasive treatments are not an option because of the limited time frame. In rare situations of acute stroke onset and admission to therapy within six hours however, aggressive recanalisation may be considered. This technical note demonstrates surgical transcatheter embolectomy of intra-extra cranial ICA by reducing inflow by placing a clamp on the common car

Percutaneous Vacuum-Assisted Thrombectomy Using AngioVac Aspiration System.

BACKGROUND: Surgical embolectomy and thrombolytic therapy are two common approaches for the treatment of large intra-cardiac or intravascular thrombi to prevent new or worsening pulmonary embolism (PE). Considering high operative mortality with surgical embolectomy and high bleeding risk with thrombolytic therapy, patients who are poor candidates for these treatments may benefit from percutaneous aspiration thrombectomy/Vacuum-assisted thrombectomy (VAT). AngioVac aspiration system was granted 510(k) clearance by the United States Food and Drug Administration (FDA) in April 2009. We present a

Right Atrial Vacuum-Assisted Thrombectomy: Single-Center Experience.

BACKGROUND: Right heart thrombus in the absence of structural heart disease, atrial fibrillation, or intracardiac catheter is rare. It typically represents a thrombus migrating from the venous system to the lung, known as thrombi-in-transit, and can lead to a life-threatening pulmonary embolism. The optimal therapy for thrombi-in-transit remains controversial. We report our experience using percutaneous removal of right heart thrombus using vacuum aspiration. METHODS: Patients with right atrial mass who were hemodynamically stable and underwent vacuum thombectomy using the AngioVac system (An

Angiojet rheolytic thrombectomy in infants following cardiac surgery.

OBJECTIVES: To present our experience with the Angiojet thrombectomy system for percutaneous thrombectomy in infants following cardiac surgery. BACKGROUND: Arterial and venous thrombosis after pediatric cardiac surgery is life threatening, particularly in infants. The best method for treating these patients has not been determined. Angiojet rheolytic thrombectomy (Possis Medical, Minneapolis, MN) is a percutaneous method of thrombus removal with proven safety and efficacy in adults. METHODS: We reviewed the medical records and angiograms of all procedures in which Angiojet thrombectomy was u

Management of left atrial endocardium after extensive thrombectomy.

Thrombosis at the left atrium is a common phenomenon in patients with chronic mitral valve disease and atrial fibrillation. When thrombus organizes and evolves into chronic phase, clean thrombectomy can become a challenge during heart surgery because of dense adhesions and the lack of clean cleavage plane. Leaving residual thrombotic material or roughened endocardial surface after thrombectomy could be a potential source for further thrombosis and a nidus for thromboembolism. We recently managed such a patient successfully using extensive thrombectomy and endocardial coverage with a fresh auto

[Iatrogenic carotid-cavernous fistulas, complications of carotid thrombectomy].

The authors report four cases of carotid cavernous fistula at the C5 segment of the internal carotid artery following embolectomy by a 3F Fogarty Catheter. All patients were operated on a carotid bifurcation stenosis (pre-occlusive) and a thrombectomy with a 3F Fogarty Catheter was carried out in the same time in 3 patients and during re-operation in 1 patient who had post operative thrombosis. The carotid-cavernous fistula occurred the day after surgery in 2 cases, 1 week after in 1 case, and 2 months later in 1 case. Spontaneous recovery occurred in two patients (1 occlusion--1 re-stenosis),

Infection complications after thrombectomy in deep venous thrombosis.

In acute iliofemoral vein thrombosis thrombectomy with a temporary AV-fistula is an alternative treatment to induced fibrinolysis and anticoagulation. Reviewing our results we have found an extremely high incidence of wound infections. In 36 patients thrombectomy with temporary AV-fistula has been performed. 31% of these had a postoperative wound infection. The mean hospital stay for the infected patients was 25 days compared to 10 days for the non-infected patients. In 26 patients a second, elective operation with closure of the AV-fistula was performed. 15% of these patients developed postop

Control-matched octo/nonagenarian outcomes in acute basilar artery occlusions treated with mechanical thrombectomy-analysis of the PC-SEARCH thrombectomy registry.

IntroductionAge is often used as a predictor in determining outcomes in large vessel occlusions treated with mechanical thrombectomy. However, limited data exist for octo/nonagenarian outcomes compared to younger individuals in acute basilar artery occlusions treated with thrombectomy.MethodsPatient data were obtained from the PC-SEARCH Thrombectomy Registry which consists of 444 acute basilar artery occlusions treated with mechanical thrombectomy. Individuals were dichotomized based on age (>80 and ≤80 years old). Primary outcome was defined as modified Rankin Scale of 0-3 at 90 days. Logisti

Comparison between transradial and transfemoral mechanical thrombectomy for ICA and M1 occlusions: insights from the Stroke Thrombectomy and Aneurysm Registry (STAR).

BACKGROUND: The role for the transradial approach for mechanical thrombectomy is controversial. We sought to compare transradial and transfemoral mechanical thrombectomy in a large multicenter database of acute ischemic stroke. METHODS: The prospectively maintained Stroke Thrombectomy and Aneurysm Registry (STAR) was reviewed for patients who underwent mechanical thrombectomy for an internal carotid artery (ICA) or middle cerebral artery M1 occlusion. Multivariate regression analyses were performed to assess outcomes including reperfusion time, symptomatic intracerebral hemorrhage (ICH), dist

Single-session mechanical thrombectomy for iliofemoral deep vein thrombosis using a dual mechanism of action device combining basket and rotational thrombectomy.

OBJECTIVE: Interventional treatments for acute iliofemoral deep vein thrombosis (DVT) remain controversial after publication of the Acute Venous Thrombosis: Thrombus Removal with Adjunctive Catheter-Directed Thrombolysis (ATTRACT) study. Interventions have been shown to reduce post-thrombotic syndrome severity and improve quality of life in DVT patients, but have been accompanied by risk of major bleeding from thrombolytics. We describe thrombus removal using a novel combined basket-rotational thrombectomy device that minimizes the need for thrombolytics or repeat procedures. METHODS: The aim

SWAM Mechanical Thrombectomy System Versus AngioJet Rheolytic Thrombectomy System for Acute Lower Extremity Deep Vein Thrombosis: A Prospective, Randomized, Single-Center Study.

BACKGROUND: Acute lower extremity deep vein thrombosis (DVT) poses a significant clinical burden due to its association with post-thrombotic syndrome and pulmonary embolism (PE). While the AngioJet rheolytic thrombectomy system has established clinical utility, device-related hemolysis and embolic complications remain concerns. The Zhongtian SWAM (Synchronized Wave Aspiration and Maceration) system represents an innovative mechanical thrombectomy technology designed to minimize these adverse effects. OBJECTIVE: To compare the clinical efficacy and safety profile of the Zhongtian SWAM mechanic

Safety first: A comparative study of mechanical thrombectomy techniques for M2 occlusions - stent-retriever thrombectomy (SRT) vs. first stenting without retrieval (FRESH).

BACKGROUND: The most appropriate method for mechanical thrombectomy (MT) in patients with acute ischemic stroke (AIS) due to M2 segment middle cerebral artery (MCA) occlusion remains unclear. Hence, this study aimed to investigate the benefits of first stenting without retrieval (FRESH) vs. stent-retriever thrombectomy (SRT) for treating patients with M2 occlusion-associated AIS. METHODS: The data of patients who underwent MT for AIS at our institution between January 2019 and December 2023 were reviewed. Treatment variables, clinical outcomes, and complications were analyzed. RESULTS: Thirt

Direct thrombectomy versus bridging thrombolysis with mechanical thrombectomy in middle cerebral artery stroke: a real-world analysis through National Inpatient Sample data.

OBJECTIVE: A paradigm shift in the management of acute ischemic stroke (AIS) due to large-vessel occlusion (LVO) occurred after 2015 when 7 randomized controlled trials demonstrated better outcomes using second-generation thrombectomy devices combined with best medical management than did stand-alone intravenous thrombolysis (IVT) with tissue plasminogen activator (tPA). All recently published landmark trials were designed to study the outcome of mechanical thrombectomy (MT); therefore, the majority of the patients enrolled in these trials received intravenous tPA. Currently, initiating IVT be

Comparison of stent retriever thrombectomy using 3-dimensional patient-specific models of intracranial circulation with actual middle cerebral artery occlusion thrombectomy cases.

BACKGROUND AND PURPOSE: Stent retriever (SR) thrombectomy is commonly used for the treatment of emergent large vessel occlusion (ELVO) in acute ischemic stroke. Clot imaging parameters such as clot length, diameter, distance to the internal carotid artery terminus, and vessel angle where the SR is deployed may predict the likelihood of achieving first pass effect (FPE). Most of the proposed factors that seem to affect recanalization success have been studied individually, and conflicting data derived from clinical versus in vitro studies using 3-dimensional printed models of intracranial circu

Bacterial endocarditis presenting with unilateral renal artery occlusion treated with the Rinspirator Thrombus Removal System rinsing and thrombectomy device and suction thrombectomy.

We report a 36-year-old man who presented with unilateral flank pain caused by renal artery occlusion with ischemia and infarction from septic emboli secondary to bacterial endocarditis. We treated the occlusion with a novel rinsing and aspiration device, the Rinspirator Thrombus Removal System (ev3, Plymouth, Minn) and suction thrombectomy, which resulted in significant revascularization of the kidney and relief of symptoms. Postprocedural imaging demonstrated marked improvement in renal vascularization, with only small areas of infarction. This technique may be useful in patients where the e

Technical considerations in endovascular thrombectomy for distal vessel occlusions.

The role of endovascular thrombectomy (EVT) in acute ischemic stroke due to distal vessel occlusions (DVOs) remains to be clarified. Anatomical complexities in the distal vasculature, such as tortuosity and vessel fragility, render technique selection particularly paramount during EVT. Herein, we present two illustrative cases of anatomically similar DVOs treated with two different techniques at a single institution to frame a discussion of important procedural considerations during EVT for DVO.
