Time course of blood pressure control prior to lacunar TIA and stroke: Population-based study.

OBJECTIVE: To determine the age-specific temporal trends in blood pressure (BP) before acute lacunar vs nonlacunar TIA and stroke. METHODS: In a population-based study of TIA/ischemic stroke (Oxford Vascular Study), we studied 15-year premorbid BP readings from primary care records in patients with lacunar vs nonlacunar events (Trial of Org 10172 in Acute Stroke Treatment [TOAST]) stratified by age (<65, ≥65 years). RESULTS: Of 2,085 patients (1,250 with stroke, 835 with TIA), 309 had lacunar events. In 493 patients <65 years of age, the prevalence of diagnosed hypertension did not differ be

Resistant Hypertension, Time-Updated Blood Pressure Values and Renal Outcome in Type 2 Diabetes Mellitus.

BACKGROUND: Apparent treatment resistant hypertension (aTRH) is highly prevalent in patients with type 2 diabetes mellitus (T2D) and entails worse cardiovascular prognosis. The impact of aTRH and long-term achievement of recommended blood pressure (BP) values on renal outcome remains largely unknown. We assessed the role of aTRH and BP on the development of chronic kidney disease in patients with T2D and hypertension in real-life clinical practice. METHODS AND RESULTS: Clinical records from a total of 29 923 patients with T2D and hypertension, with normal baseline estimated glomerular filtrat

The effect of time pressure on risky financial decisions from description and decisions from experience.

Time pressure has been found to impact decision making in various ways, but studies on the effects time pressure in risky financial gambles have been largely limited to description-based decision tasks and to the gain domain. We present two experiments that investigated the effect of time pressure on decisions from description and decisions from experience, across both gain and loss domains. In description-based choice, time pressure decreased risk seeking for losses, whereas for gains there was a trend in the opposite direction. In experience-based choice, no impact of time pressure was obser

Acute effects of night-time noise exposure on blood pressure in populations living near airports.

AIMS: Within the framework of the HYENA (hypertension and exposure to noise near airports) project we investigated the effect of short-term changes of transportation or indoor noise levels on blood pressure (BP) and heart rate (HR) during night-time sleep in 140 subjects living near four major European airports. METHODS AND RESULTS: Non-invasive ambulatory BP measurements at 15 min intervals were performed. Noise was measured during the night sleeping period and recorded digitally for the identification of the source of a noise event. Exposure variables included equivalent noise level over 1

Cell design for picosecond time-resolved infrared spectroscopy in high-pressure liquids and supercritical fluids.

The design of a new high-pressure infrared (IR) cell for carrying out picosecond time-resolved infrared (ps-TRIR) spectroscopy in supercritical fluids is described. We have employed thin (2 mm) MgF(2) windows in order to overcome possible undesirable nonlinear optical effects caused by the extremely high peak powers of ultrashort ultraviolet (UV)/visible pulses. The design of our cell allows for the study of systems at pressures of up to 5500 psi at temperatures of up to approximately 50 degrees C. The MgF(2) windows enable the excitation of samples with both UV and visible light pulses and th

Continuous positive airway pressure treatment for sleep apnoea: compliance increases with time in continuing users.

Continuous positive airway pressure (CPAP) remains the best treatment for sleep apnoea syndrome (SAS). In the 1990s, many authors reported on daily compliance, but all of the studies utilised relatively short periods of follow-up that did not exceed a few years. The mean annual rate of CPAP use in patients with SAS was prospectively recorded. In the current study, the results are presented along with compliance data from patients who started CPAP between 1991 and 1998 and were still using it by the end of 2003. The cohort was chosen in order to obtain >or=5 yrs of follow-up for each patient. I

Time poor: rushing decreases the accuracy and reliability of blood pressure measurement technique in pregnancy.

BACKGROUND: In pregnancy, absolute blood pressure (BP) limits define preeclampsia. Therefore, BP in pregnancy should be measured accurately and in accordance with accepted guidelines. Accuracy of BP readings determined by rate of cuff deflation was analyzed. This study also investigated the compliance of clinical staff at Royal Prince Alfred Hospital, Australia, to guidelines for BP measurement. METHODS: The study was an observational trial of 98 normotensive antenatal or recently postnatal patients. Two BP readings were taken, each with fast (>5 mm Hg/sec) and slow (<or=2 mm Hg/sec) descent

Time-dependent change in baroreflex control capacity of arterial pressure by pentobarbital anesthesia in rabbits.

The present study is designed to investigate the time-dependent effect of pentobarbital anesthesia on the baroreflex arterial pressure (AP) control system in rabbits. The overall AP control capacity of the baroreflex system was assessed with mean arterial pressure (MAP) responses to the rapid mild hemorrhage (2 ml/kg body weight) and an overall open-loop gain (G) of the system. The G value was determined by means of the following formula: G = delta API/delta APS-1, where delta APl is an immediate MAP fall and delta APS a steady-state fall after the rapid hemorrhage. Prior to the experiment, tw

Effects of varying inspiratory flow waveform and time in intermittent positive pressure ventilation: pulmonary oedema.

Greyhound dogs were given oleic acid i.v. to induce controlled pulmonary oedema. These animals were then studied using intermittent positive pressure ventilation (IPPV) with four different inspiratory flow waveforms, each at three different inspiratory times, in a fixed respiratory cycle of 4 s at a constant tidal volume. Although there were statistically significant differences in airway and oesophageal pressures between the different waveforms and times, there was little variation in the other physiological parameters studied except for arterial carbon dioxide tension. (PaCO2) which showed s

Association of Systolic Blood Pressure Time in Target Range With Cardiovascular Events Among PRECISION Participants.

Blood pressure (BP) is a dynamic vital sign with variability. Novel metrics that account for BP variability and longitudinal control are gaining interest, such as time in target range (TTR) assessments. TTR is the percentage of time a patient's BP is within a desired range. We sought to determine if systolic BP TTR was associated with major adverse cardiovascular events (MACE) among participants in the PRECISION (Prospective Randomized Evaluation of Celecoxib Integrated Safety versus Ibuprofen or Naproxen) trial. PRECISION was a 24 081-participant cardiovascular (CV) outcomes trial comparing c

Inspiratory pressure waveform influences time to failure, respiratory muscle fatigue, and metabolism during resistive breathing.

Increased ventilatory work beyond working capacity of the respiratory muscles can induce fatigue, resulting in limited respiratory muscle endurance (Tlim ). Previous resistive breathing investigations all applied square wave inspiratory pressure as fatigue-inducing pattern. Spontaneous breathing pressure pattern more closely approximate a triangle waveform. This study aimed at comparing Tlim , maximal inspiratory pressure (PImax ), and metabolism between square and triangle wave breathing. Eight healthy subjects (Wei = 76 ± 10 kg, H = 181 ± 7.9 cm, age = 33.5 ± 4.8 years, sex [F/M] = 1/7) comp

Time point of nocturnal trough systolic blood pressure as an independent predictor of cardiovascular events.

Nocturnal trough systolic blood pressure (NTSBP) and Time Point of Nocturnal Trough Systolic Blood Pressure (T-NTSBP) were important parameters of nocturnal blood pressure, the predictive values of which are unclear for stroke outcome. This study aimed to examine the relationship between NTSBP/T-NTSBP and stroke outcome. The authors used data from a nationwide ambulatory blood pressure monitoring cohort study conducted in China, which recruited 2348 ischemic stroke and transient ischemic attack (TIA) patients. NTSBP was defined as the lowest SBP during nighttime (22:00-6:00), and T-NTSBP was d

Pulmonary artery acceleration time accuracy for systolic pulmonary artery pressure estimation in critically ill patients.

BACKGROUND: Estimation of pulmonary pressures is of key importance in acute cardiovascular and respiratory failure. Pulmonary artery acceleration time (PAAT) has emerged as reliable parameter for the estimation of systolic pulmonary artery pressure (sPAP) in cardiological population with preserved right ventricular function. We sought to find whether PAAT correlates with sPAP in critically ill patients with and without right ventricular (RV) systolic dysfunction. METHODS: Observational study. We measured sPAP using continuous-wave Doppler analysis of tricuspid regurgitation velocity peak meth
