Free vascularized joint transfers in acute complex hand injuries: case reports.

The loss of function of the metacarpophalangeal joint is a significant disability. Simultaneous reconstruction of the soft tissue, extensor mechanism, joint, and flexor tendon in a complex hand injury is difficult and challenging. Free vascularized autogenous toe joint transplantation is a useful technique that provides not only joint replacement but also the soft tissue, extensor mechanism, and flexor tendon in more severe complex hand injuries. Two patients underwent immediate, free vascularized metatarsophalangeal joint transfer of the second toe to replace the long and ring finger metacarp

Foot arch rigidity in walking: In vivo evidence for the contribution of metatarsophalangeal joint dorsiflexion.

Human foot rigidity is thought to provide a more effective lever with which to push against the ground. Tension of the plantar aponeurosis (PA) with increased metatarsophalangeal (MTP) joint dorsiflexion (i.e., the windlass mechanism) has been credited with providing some of this rigidity. However, there is growing debate on whether MTP joint dorsiflexion indeed increases arch rigidity. Further, the arch can be made more rigid independent of additional MTP joint dorsiflexion (e.g., when walking with added mass). The purpose of the present study was therefore to compare the influence of increas

The relationship between morphometric measurements and radiographic changes associated with distal interphalangeal joint flexural deformity in donkeys.

BACKGROUND: Flexural deformity of the distal interphalangeal (DIP) joint is a common condition in both working and non-working donkeys, arising from congenital or acquired causes, or as a secondary consequence of trauma or chronic lameness. The deformity results in abnormal weight distribution on the hoof. Although radiography is essential for identifying osseous and articular abnormalities, few studies have documented the morphometric and radiographic changes associated with DIP joint flexural deformity in donkeys. The objective of this study was to investigate the relationship between morpho

Assessment and Treatment of Sports Injuries to the First Metatarsophalangeal Joint.

Turf toe injuries have been increasing in numbers in recent years. Injury to the plantar restraints of the first metatarsophalangeal joint can lead to significant disability in athletes, affecting their push-off and ability to perform on the athletic field. Most turf toe injuries can be treated conservatively with rest, ice, compression, immobilization if needed, and a dedicated rehabilitation program; however, in some injuries, the plantar restraints are torn and the joint becomes unstable. If necessary, turf toe injury and its many variants can be surgically repaired with the expectation tha

A pilot biomechanical assessment of curling deliveries: is toe sliding more likely to cause knee injury than flatfoot sliding?

BACKGROUND: The aim of this study was to determine whether toe sliding is more likely to cause knee injuries than flatfoot sliding in curling. METHODS: Twelve curlers participated in the study, each delivering 12 stones. Six stones per volunteer were delivered using a flatfoot slide and six were delivered using a toe slide. The Pedar-X in-shoe pressure system recorded the plantar pressure during each of the slides, while a sagittal plane digital video recorded the body position of the curler. Measurements were taken from the video recordings using a software overlay program (MB Ruler), and th

Investigating the Effect of Motivational Music and Video and the Role of Cognitive Style on the Gait Kinematics of Elderly Individuals.

In the present study, we investigated the impact of motivational music and video on gait parameters in older adults with different cognitive styles. Participants (N = 24; M age = 68.83 years, SD = 2.61) were divided into two cognitive style groups: field-dependent and field-independent, based on a hidden figures test. Participants completed a treadmill walking task under three different conditions (motivational music, motivational video, and no intervention). We measured their step width and step length and their hip, knee, and ankle joint angles at heel strike and toe-off. Results showed a si

Aesthetic refinements in second toe-to-thumb transfer surgery.

BACKGROUND: Second toe-to-thumb transfer is a good alternative to using the great toe for reconstruction of the thumb. It achieves excellent function and reduces morbidity to the donor foot. However, cosmesis is often poor. The second toe has three unattractive features, a narrow "neck," a bulbous tip, and a short nail. METHODS: The authors describe a modified second toe transfer that addresses cosmesis in six patients. These include (1) harvesting a flap from the adjacent side of the great toe and insetting it into the volar aspect of the second toe to give more bulk, (2) making skin excisio

Operative Treatment for Fourth Curly Toe Deformity in Adults.

BACKGROUND: Curly toe deformity, a relatively common condition, occurs because of contracture of the flexor digitorum longus and flexor digitorum brevis. Numerous operative techniques have been introduced for treatment. However, there are no reports on fixed, longstanding deformity in adults. METHODS: Data on 32 toes in 24 consecutive patients treated from 2003 to 2013 who underwent dorsolateral closing wedge-shaped resection arthroplasty for fourth curly toe deformity with a minimum 1-year follow-up were reviewed retrospectively. The mean follow-up period was 49 months. All patients presente

Flexor tendon transfer for metatarsophalangeal instability of the second toe.

Flexor to extensor transfer was used to treat painful second metatarsophalangeal joint instability in thirteen feet in eleven patients. All patients had their pain reproduced with vertical stress motion of 50% to 100% at the metatarsophalangeal joint. Seven feet had concomitant hallux valgus correction, two feet had no hallux valgus, and four feet underwent no correction for asymptomatic hallux valgus. Results at an average of 33.4 months followup showed that all patients had substantial pain relief, with eight patients becoming pain-free, and five patients experiencing mild pain. All but one

Hemiarthroplasty Versus Total Joint Arthroplasty for Hallux Rigidus: A Systematic Review and Meta-analysis.

BACKGROUND: When surgery is indicated for hallux rigidus, toe arthroplasty is an alternative procedure to arthrodesis for patients who wish to preserve toe range of motion. Our study investigated midterm outcomes of first metatarsophalangeal joint (MTPJ) arthroplasty in an effort to discern whether or not partial or total joint replacement confers benefit in these patients. METHODS: A systematic review of MTPJ arthroplasty was performed for the years 2000 to 2017. A Forest plot was created comparing preoperative and postoperative American Orthopedic Foot and Ankle Score (AOFAS), Visual Analog

An individualized approach to surgical reconstruction for lateral polydactyly of the foot with an emphasis on collateral ligament reconstruction.

BACKGROUND: Most previous studies concerning the treatment of polydactyly of the foot have focused on surgical techniques for aesthetic improvement rather than functional alignment of the reconstructed toe. The purpose of this study was to present the authors' surgical procedures for preventing postoperative malalignment of the reconstructed toe and evaluate whether those procedures were useful for the treatment of polydactyly of the foot. METHODS: The study included 30 feet from 26 patients, four male and 22 female, with four bilateral cases. The average age of the patients at surgery was 20

A new bone-ligament-bone autograft from the plantar plates of the toes and its potential use in scapholunate reconstruction: an anatomical study.

The study was performed to investigate a new bone-ligament-bone autograft from the plantar plate of the toes. The anatomic properties of the plantar ligaments and the technical feasibility to harvest a bone-ligament-bone graft were examined to evaluate the potential use of this graft for a suitable reconstruction of the scapholunate (SL) interosseous ligament. The plantar plate of the metatarsophalangeal joints of the second to fifth toe and the proximal interphalangeal joints of the second to fourth toe were examined in 20 cadaver feet (15 fresh and 5 embalmed cadavers) and measurements such

Synergic control of the minimum toe clearance in young and older adults during foot swing on treadmill walking in different speeds.

BACKGROUND: The vertical toe position at minimum toe clearance (MTC) in the swing phase is critical for walking safety. Consequently, the joints involved should be strictly controlled and coordinated to stabilize the foot at MTC. The uncontrolled manifold (UCM) hypothesis framework has been used to determine the existence of synergies that stabilize relevant performance variables during walking. However, no study investigated the presence of a multi-joint synergy stabilizing the foot position at MTC and the effects of age and walking speed on this synergy. RESEARCH QUESTIONS: Is there a multi
