Treatment of replacement resorption with Emdogain--a prospective clinical study.

The present clinical study investigated the outcome of intentional replantation using Emdogain for periodontal healing following trauma-related ankylosis. Sixteen ankylosed teeth affected by replacement resorption were treated as follows: After tooth extraction, the root canal was obturated with a retrograde titanium post. Emdogain was applied to the root surface and into the extraction socket with subsequent replantation of the tooth. Evaluation parameters included horizontal and vertical Periotest scores, percussion sound and periapical radiographs. All findings were compared to those of the

Complicated Crown-root Fracture Management Using the 180-degree Rotation Method.

AIM: The aim of this article is to report a successful treatment of a complicated crown-root fracture. BACKGROUND: The management of complicated crown-root fracture in young patients is challenging as the fracture line is below the bone crest and the pulp is exposed. Treatment options include crown lengthening, orthodontic extrusion, and intentional replantation. CASE DESCRIPTION: Here, a case of complicated crown-root fracture of tooth number 11 in a ten-year-old boy is presented. Intentional replantation with 180° rotation and slight extrusion and fixation was performed. In addition, root

Infraposition of ankylosed permanent maxillary incisors after replantation related to age and sex.

The extent of infraposition of replanted and subsequently ankylosed permanent incisors was examined in a longitudinal study of 52 patients. Study cast models were made during the follow-up period. Infraposition was evaluated on frontal photos of the study cast models taken parallel with the occlusal plane. The extent of infraposition was defined as the difference between the position of the incisal edge of the replanted incisor and the adjacent non-injured incisor in apico-coronal direction, measured with a digital caliper at 50x magnification of the negatives. The precision of this measuring

Influence of clinical factors, IL4 and IL6 genes polymorphisms in functional healing in late replantation.

To investigate the genetic association in a sample of replanted teeth, it is necessary to observe the extreme phenotypes, such as, teeth that underwent functional healing and those extracted due to severe external root resorption. Thus, this study aimed to investigate the association of age of the patients, root development, storage media, and polymorphisms in the interleukin 4 (IL4) and interleukin 6 (IL6) genes with teeth that presented extreme outcomes, as functional healing or extraction, in a group whose replantation techniques did not follow the International Association of Dental Trauma

A Novel Approach for Orthodontic Extrusion Prior to Intentional Replantation: A Case Report.

AIM: This case report aimed to present a novel surgical technique involving orthodontics luxation of the offended tooth using miniscrew prior to performing intentional replantation (IR) to remove an extruded separated instrument that injured the inferior alveolar nerve (IAN). BACKGROUND: Intentional replantation is a dental procedure that involves extracting a tooth, managing the root canal, and then reinserting the tooth back into its socket. This procedure is typically used as a last resort when other treatment options have failed or are not possible. Intentional replantation can be success

Intentional Replantation of C-shaped Mandibular Second Premolar with Radicular Groove.

C-shaped roots with radicular grooves are uncommon in mandibular premolars. Bacteria residing in the radicular groove and associated accessory canals can contribute to persistent periradicular infections. Eradicating bacteria in these less accessible areas remains a significant challenge in endodontic procedures. This report describes the successful management of a left mandibular second premolar (#20) with a persistent lesion related to a lingual radicular groove through an intentional replantation procedure. A 31-year-old Chinese male complained of a gum swelling related to tooth #20 which w

Long-term management of avulsed "hopeless" teeth in the adult dentition.

OBJECTIVE: This case will demonstrate a thoughtful approach to the management of avulsed/replanted teeth in the adult dentition and their long-term maintenance. Often times these teeth are either not replanted, or extracted soon after replantation when resorptive lesions appear and the teeth are deemed "hopeless". The term "hopeless tooth" (HT) has become more popular since the advent of implants. Implants allowed for a simple solution to the HT by extraction and immediate replacement with a fixture and a restoration. However, now that we are realizing that implants do not last forever and als

Immediate self-replantation of an avulsed permanent mandibular incisor - a case report.

BACKGROUND: Approximately 50% of children under the age of 15 years are subjected to various kinds of injuries in the orofacial region. In the permanent dentition, the most severe dental injury affects the surrounding alveolar bone structure and leads to loss of the tooth. The current literature emphasizes that the awareness of appropriate triage procedures following dental trauma is unsatisfactory and that delay in treatment is the single most influential factor affecting prognosis. RESULTS: This case report presents the immediate self-replantation of a right lateral mandibular incisor of a

Evaluation of the topical effect of alendronate on the root surface of extracted and replanted teeth. Microscopic analysis on rats' teeth.

The treatment of choice for tooth avulsion is replantation. The ideal replantation should be realized as quickly as possible, or at least, the avulsed tooth should be kept in an adequate solution to preserve the periodontal ligament attached to the root. If that is not possible, treatment of the radicular surface should be done in order to prevent radicular resorption. The purpose of this study was to test sodium alendronate as a substance for topical treatment of the radicular surface of avulsed teeth in an attempt to prevent the occurrence of dental resorptions. Fifty-four rat maxillary righ

Re-inventing intentional replantation: a modification of the technique.

Intentional Replantation (IR) has traditionally been considered a treatment of last resort due to complications during extraction, handling, and replantation. Root resorption and failure often resulted from drying of the root surface during extraoral manipulations. Replantation techniques have been recently modified to prevent desiccation. This article presents a modified IR technique with continuous immersion of the tooth in a tissue culture solution. This allows extension of the extraoral time period to permit inspection of the root surfaces, precise root-end resections and preparations, and

Histopathological evaluation of the effects of variable extraoral dry times and enamel matrix proteins (enamel matrix derivatives) application on replanted dogs' teeth.

The extra-alveolar dry period and storage medium in which the tooth was kept prior to replantation remain the critical factors affecting the survival and regeneration of the damaged periodontium. When the replantation is delayed, replacement root resorption is the most common complication following replantation of an avulsed tooth. The aim of this histological study was to evaluate the periodontal healing of replanted dogs' teeth after 20 min (short) and 60 min (long) extraoral dry time with and without the application of enamel matrix proteins. Eighty mature premolar roots (40 teeth) maxillar

Clinical management of the avulsed tooth.

Treatment outside the dental office: Replant immediately after gentle washing if practical. If replantation is not practical, store the tooth in the best medium available. Storage media in order of preference are Hank's Balanced Salt Solution (HBSS), milk, saline, and saliva (buccal vestibule). Water is the least desirable storage medium. Treatment in the office: Emergency visit; Place tooth in HBSS while exam is conducted and history is taken. Prepare socket for gentle repositioning of the tooth. Prepare the root. Extraoral dry time < 20 minutes: Closed apex--replant immediately after gentle

Treatment opinions for dens invaginatus: A case series.

Dens invaginatus (DI) is a rare congenital dental malformation characterized by enamel or cementum folded into dentine. Such teeth are susceptible to caries, pulp infection or necrosis and periradicular lesion. The complex anatomy of this disease results in difficult treatment and a high rate of therapeutic failure. Therapeutic options, such as debriding and filling invagination, root canal treatment (RCT) and intentional replantation, vary according to the morphology and infection of the involved tooth. The present study reports five cases of DI with chronic apical periodontitis. The treatmen

Management of Crown-root Fracture with 180-degree Rotation Replantation: a Report of 2 Cases.

Intentional replantation involves a combination of periodontics, endodontics, prosthodontics and oral surgery. Crown-root fracture management is still complicated nowadays. A fracture line extending longitudinally to the subgingival area and intruding bioogical width could affect infection control, gingival health and crown restoration. In the present study, we present two cases. Case 1 involved a 23-year-old man who presented at our hospital with crown-root fracture of the maxillary left central incisor. A radiographic image of the tooth revealed a fracture line under the alveolar crest. The

An evidence-based assessment of the clinical guidelines for replanted avulsed teeth. Part II: prescription of systemic antibiotics.

BACKGROUND: Current clinical guidelines recommend prescribing systemic antibiotic therapy (SAT) for patients having an avulsed permanent tooth replanted. The principles of evidence-based dentistry can be used to assess whether this is the best approach based on currently-available evidence. The objective of this study was to use the principles of evidence-based dentistry to answer the PICO question: (P) for a replanted avulsed permanent tooth, (I) is prescribing SAT, (C) compared with not prescribing SAT, (O) associated with an increased likelihood of successful periodontal healing after tooth

Protocol for treating the avulsed tooth.

Avulsion of a tooth requires decisive action by the dental practitioner. Replantation of the tooth at the earliest possible time, along with the avoidance of further periodontal damage, is paramount to achieving a successful result. The accident site, time the tooth is out of the socket, storage media and endodontic protocol influence the prognosis. Appropriate endodontic management and tooth restoration will ensure long-term success.

Replantation of an avulsed permanent maxillary incisor with an immature apex: report of a case.

A case is reported of the replantation of a maxillary incisor with an immature apex following a traumatic avulsion. A 14-month follow-up clinical examination revealed the patient to be asymptomatic, the tooth to be still functional, and a recall radiograph showed no evidence of renewed periradicular breakdown. The indications for, and limitations of, replantation of an avulsed permanent incisor with an immature apex are discussed.
