[Anaphylactic shock after tourniquet removal in orthopedic surgery].

Case report of six patients who experienced symptoms of severe anaphylactic shock, after tourniquet release at the end of minor orthopaedic surgery of the limbs under regional anaesthesia. The immunological mechanism of the shock was confirmed by prick tests. The causative agent was rifamycin SV used for cleaning the surgical wound prior to its closure. In case of repetitive surgery and/or in allergic patients the use of another antibiotic or the simple cleaning of the wound with normal saline should be considered. In case of surgery using a tourniquet, an iv access should be maintained for 1

Perineural pressures under the pneumatic tourniquet in the upper extremity.

The literature indicates that tourniquet-induced neurological injuries are relatively common and frequently occur at a subclinical level. In order to evaluate the pressure transmitted to the major peripheral nerves of the arm by an externally applied pneumatic tourniquet, a fully implantable biomedical pressure transducer was placed adjacent the radial, median and ulnar nerves in six cadaver upper extremities of average dimensions. This sensor allowed accurate, reproducible measurements of perineural pressures without requiring significant disruption of the normal anatomical structures of the

[Strangulation of glans penis by hair or "penis tourniquet syndrome"].

The authors report 5 cases of strangulation of the penis with hair in children. This affection, more seen frequently within socially disadvantaged children, needs early diagnosis and treatment because it may be responsible for glans amputation. A better knowledge of this accident by parents and physicians might prevent this barbaric aggression.

Prehospital tourniquet use and associated outcomes in a low-resource conflict setting: prospective observational casualty data and tourniquet classification system from the Sahel.

INTRODUCTION: Tourniquet (TQ) application is a key component of extremity haemorrhage control in high-resource military and civilian trauma systems. In settings with prolonged evacuation times, TQs can cause complications including limb loss, rhabdomyolysis, or death, yet limited data exist on the scope and impact of this problem. METHODS: We conducted a single-centre prospective observational analysis at the largest military hospital in Burkina Faso to describe injury epidemiology, TQ application practices and early outcomes among casualties with conflict-related injuries who underwent preho

Tourniquet with Intravenous Tranexamic Acid versus Tourniquet with Vaginal Misoprostol for Blood Loss Reduction during Open Myomectomy: A Double-Blind Randomised Trial.

BACKGROUND: Myomectomy has remained one of the main surgical techniques for managing uterine fibroids, especially in sub-Saharan Africa, where women desire to preserve their uterus for fertility and other cultural reasons. However, blood loss remains a major complication during myomectomy. The study's objective was to compare tourniquet with intravenous tranexamic acid and tourniquet with vaginal misoprostol in blood loss reduction during open myomectomy. METHODOLOGY: A double-blind randomised controlled trial with Pan-African Clinical Trials Registry approval number PACTR202406615201194 was

Co-Selection of Antibiotic and Disinfectant Resistance in Bacteria Isolated from Reusable Blood Collection Tourniquets: Implications for Infection Control.

Background: Reusable tourniquets are widely used across clinical settings, yet their role as reservoirs of microbial contamination and antimicrobial resistance remains poorly characterized. Methods: In this cross-sectional study, 53 polyester-elastane tourniquets were collected from an Emergency Department (SR) and Operating Theater (SBO) over a 28-day period to assess bacterial burden and resistome composition. A 180-target qPCR panel targeting antibiotic and disinfectant resistance determinants was used. qPCR analysis identified 112 distinct resistance genes across all samples, with SR tourn

WAFER Trial: A randomized controlled trial comparing Wide-awake Local Anesthesia No Tourniquet (WALANT) to General and Regional Anesthesia with tourniquet for Flexor Tendon Repair.

BACKGROUND: Wide-awake local anesthesia no tourniquet (WALANT) is increasingly used for flexor tendon repair, but comparative data versus general/regional anesthesia(GA/RA) are limited. The aims of the WAFER trial were to generate high quality comparative data on WALANT versus GA/RA for flexor tendon repair and assess the feasibility of performing a large multicenter trial of this nature. METHODS: WAFER was a multicenter, assessor-blinded, parallel-group RCT with embedded qualitative and health economic components across four major hand units. Patients were randomized on a 1:1 basis to have f

Assessing Evidence Bias for Prehospital Tourniquet Use: A Scoping Review.

BACKGROUND: The available evidence for the prehospital use of tourniquets is largely based on the military experience of high-resource countries over the last 30 years. Though the lifesaving benefits of an appropriately applied tourniquet to stop catastrophic bleeding are undeniable, the potential consequences of tourniquet application differ greatly within different environments. In many low resource settings, both the time until initial treatment and the evacuation time to a qualified medical care can range from hours to days. The global burden of trauma disproportionally affects low- and mi

Strangulated Intestinal Obstruction due to Appendiceal Tourniquet: A Case Report.

Intestinal obstruction is a common surgical emergency. It has many causes, which may be either benign or malignant. However, appendicitis is not a common cause of intestinal obstruction, hence the challenge in making a diagnosis. Radiological and clinical diagnoses of this could be difficult; hence, diagnosis is mostly intraoperative. This case study involved a 54-year-old male, who presented at Edward Francis Small Teaching Hospital with features of acute intestinal obstruction for 5 days. During surgery, a gangrene of a loop of the bowel was found with an inflamed and partly gangrenous appen

Management of Tourniquet-Related Nerve Injury (TRNI): A Systematic Review.

Tourniquet-related nerve injuries (TRNIs) are a rare but feared complication of operative tourniquet use. While the literature contains multiple discussions regarding tourniquet use as well as reported cases of its complications, there does not exist a consensus guideline for a safe tourniquet pressure, application time, or management of TRNI. This paper conducts a comprehensive review of the available literature for cases of TRNI with a specific focus on analyzing the management of cases of TRNI and their functional recovery. One hundred nine articles were retrieved in a search of medical lit

Management of a Pediatric Snake Envenomation After Presentation With a Tight Tourniquet.

We describe an illustrative case of pediatric snake envenomation presenting with a tightly wound tourniquet. A 10-year-old boy presented after a snake bite to the right calf. A tourniquet was in place just below the right knee. The species of snake was unknown. The patient was hemodynamically stable, but the entirety of the right leg distal to the tourniquet was discolored. Over concern for a potential venom bolus effect upon tourniquet removal, the decision was made to start a crotaline Fab antivenom infusion and gradually loosen the tourniquet. The patient tolerated the infusion and removal

Pulmonary Embolism After Application of a Sterile Elastic Exsanguination Tourniquet.

Sterile elastic exsanguination tourniquets (HemaClear; OHK Medical Devices, Haifa, Israel) are relatively new on the market but are widely used because of the ease and speed of their application. The sterile elastic exsanguination tourniquet consists of a silicon ring wrapped in a stockinet sleeve with pull straps. The physician places the ring on the patient's fingers or toes and then pulls the straps proximally. The silicon ring rolls up the limb, and the stockinet sleeve unrolls onto the limb. During proximal rolling, the device displaces blood out of the limb (exsanguination). When the ela

Tourniquet time affects postoperative complications after knee arthroplasty.

PURPOSE: Pneumatic tourniquets are frequently used in knee arthroplasty surgery. However, there is a lack of evidence to define safe tourniquet time in lower limb surgery. The primary aim of this study was to investigate whether tourniquet time influences the risk of postoperative complications after primary and secondary knee arthroplasty. METHODS: This study was a prospective register study. Since we wanted dispersion in tourniquet time, we included a consecutive series of 577 primary knee arthroplasties, 46 revision knee arthroplasties, and 18 patellar supplementing knee arthroplasties fro

Preventing tourniquet effect when dressing finger wounds in children.

Hand and finger injuries are common injuries in children seen in emergency departments. Many of these are soft tissue injuries, which are often caused by household items, such as doors and exercise equipment. Once these injuries are repaired, usually they are covered with a simple dressing using a material such as Coban (3M, Minneapolis, MN). This dressing often is applied by winding it circumferentially around the digit. However, with very little manipulation, this dressing can be lifted and rolled up the digit in a distal direction, creating a tourniquet effect, which can cause hypoxia and t
