Transcranial magnetic stimulation in patients with Rett syndrome: preliminary results.

Transcranial magnetic stimulation was used to investigate Rett patients (n = 31, age 2-38 years). The results were compared to healthy control persons (n = 112, age 1 month-35 years). The peripheral conduction time showed no difference between the patients and control persons. The mean central conduction time, however, was shorter in the younger patients with Rett syndrome, and this is particularly evident in the youngest. This phenomenon could be interpreted as cortical and/or spinal hyperexcitability which occurs during the early stages of Rett syndrome.

[Motor evoked potentials of transcranial magnetic stimulation for Parkinson's disease].

We studied motor evoked potential (MEP) elicited by transcranial magnetic stimulation (TMS) in 37 patients with Parkinson's disease (PD). TMS was used at high energy capacity (800 Volts) in all patients and normal subjects under resting position. MEPs were recorded from the abductor pollicis brevis and the abductor hallucis. Magnetic stimulator equipped with an 8-shaped coil and a flat type coil was used for TMS. The data were statistically evaluated using a paired Student t test. The latency of MEPs were slightly shorter in PD patients than that in normal subjects. There were significant diff

[Analysis of cortical excitation and inhibition using transcranial magnetic stimulation].

The method of magnetic transcranial stimulation of the motor cortex produces both excitatory and inhibitory effect. Definition of excitability and inhibitory threshold represents clinically useful indicator. In order to reduce variability of the thresholds, it is important to notice mental activity, posture, type of magnetic stimulator, involuntary movement, muscle tonus and age. Excitability threshold was elevated in patients with stroke and reduced in patients with epilepsy. Enhancement of motor excitability by subthreshold magnetic stimulation of the motor cortex prior to voluntary movement

Reading with the right hemisphere: evidence from transcranial magnetic stimulation.

Although many patients with dominant hemisphere damage retain some reading ability, the anatomic substrate of the preserved reading remains controversial. We tested the hypothesis that the right hemisphere mediates the reading of some patients with acquired dyslexia by using Transcranial Magnetic Stimulation (TMS) to assess the effect of transient cortical stimulation on reading. A patient with (partially recovered) pure alexia was asked to read aloud briefly presented words, half of which were shown in association with TMS of the right or left hemisphere. Consistent with the right hemisphere

Evaluation of proximal facial nerve conduction by transcranial magnetic stimulation.

A magnetic stimulator was used for direct transcutaneous stimulation of the intracranial portion of the facial nerve in 15 normal subjects and in patients with Bell's palsy, demyelinating neuropathy, traumatic facial palsy and pontine glioma. Compound muscle action potentials (CMAPs) thus elicited in the orbicularis oris muscle of controls were of similar amplitude but longer latency (1.3 SD 0.15 ms) compared with CMAPs produced by conventional electrical stimulation at the stylomastoid foramen. No response to magnetic stimulation could be recorded from the affected side in 15 of 16 patients w

[Assessment of motor cranial nerve functions using transcranial magnetic stimulation].

Transcranial magnetic stimulation applied over one cerebral hemisphere evokes bilateral responses in the muscles supplied by cranial nerves (masseter, mentalis, sternomastoid, genioglossus) (Tabl. 1). The relatively long latencies of these responses and the influence that preactivation of the muscle has on them, suggests that they are cortically evoked. Shorter latency and exclusively ipsilateral responses can be obtained if the position of the stimulating coil over the head is more lateral. These responses are uninfluenced by pre-activation of the muscle which suggests that they arise from st

Neurophysiological Signatures of Negative Symptom Improvement After High-Definition Transcranial Direct Current Stimulation: A Transcranial Magnetic Stimulation-Electroencephalography Study.

BACKGROUND: Schizophrenia remains a major health challenge, partly because conventional antipsychotics show limited efficacy for negative symptoms. High-definition transcranial direct current stimulation (HD-tDCS) may improve these symptoms, but its neurophysiological mechanisms remain unclear. METHODS: This study included two phases. First, 103 patients with schizophrenia and 76 healthy controls underwent transcranial magnetic stimulation combined with electroencephalography (TMS-EEG) to assess cortical excitation and inhibition; patients also completed PANSS assessments. Second, patients we

Influence of Concurrent Theta-Gamma Transcranial Alternating Current Stimulation and Repetitive Transcranial Magnetic Stimulation on Primary Motor Cortex Plasticity.

OBJECTIVES: Transcranial alternating current stimulation (tACS) with a combined theta-gamma (TG) waveform can boost human motor performance, but its effects on primary motor cortex (M1) plasticity remain unclear. This study investigated the effects of concurrent TG tACS and repetitive paired-pulse transcranial magnetic stimulation (rppTMS) on M1 excitability and plasticity. MATERIALS AND METHODS: A total of 22 healthy young adults completed four experimental sessions involving concurrent application of tACS and rppTMS over M1. Fifteen minutes of rppTMS (1.5 ms interstimulus interval, [ISI]) w

Transcranial magnetic stimulation-based neuroplasticity in the treatment of amblyopia.

BACKGROUND: In recent years, repetitive transcranial magnetic stimulation (rTMS) has shown positive clinical effects in improving neuroplasticity by modulating cortical neural activity, particularly the functional connectivity of visual-related brain regions. This research was aimed to investigate the effects of rTMS on visual function in adult amblyopia and to assess changes in brain neuronal activity before and following remedy using resting-state functional magnetic resonance imaging (rs-fMRI). NEW METHOD: A total of 148 patients with anisometropic amblyopia were enrolled and randomly divi

A meta-analysis of transcranial magnetic stimulation in Tourette syndrome.

There is growing enthusiasm for the treatment of neuropsychiatric disorders using neuromodulation. While some reports claim that transcranial magnetic stimulation (TMS) can be used to treat Tourette Syndrome (TS), little research exists to support this assertion. This meta-analysis examined the efficacy of TMS to reduce tic severity in patients with TS. Additionally, it explored the effect of TMS to reduce premonitory urge severity-the primary mechanism implicated in the frontline evidence-based treatment of TS. Five treatment comparisons were selected using PRISMA guidelines. All studies incl

Applications of Transcranial Magnetic Stimulation for Understanding and Treating Dystonia.

Transcranial magnetic stimulation (TMS)-based studies have led to an advanced understanding of the pathophysiology of dystonia. This narrative review summarizes the TMS data contributed to the literature so far. Many studies have shown that increased motor cortex excitability, excessive sensorimotor plasticity, and abnormal sensorimotor integration are the core pathophysiological substrates for dystonia. However, an increasing body of evidence supports a more widespread network dysfunction involving many other brain regions. Repetitive TMS pulses (rTMS) in dystonia have therapeutic potential a

A comprehensive review of transcranial magnetic stimulation in secondary dementia.

Although primary degenerative diseases are the main cause of dementia, a non-negligible proportion of patients is affected by a secondary and potentially treatable cognitive disorder. Therefore, diagnostic tools able to early identify and monitor them and to predict the response to treatment are needed. Transcranial magnetic stimulation (TMS) is a non-invasive neurophysiological technique capable of evaluating in vivo and in "real time" the motor areas, the cortico-spinal tract, and the neurotransmission pathways in several neurological and neuropsychiatric disorders, including cognitive impai

[Pupils reactions to transcranial magnetic stimulation in patients with glaucoma].

A comparative study of visual reactions was performed in 38 patients with primary open-angle glaucoma and in 53 healthy individuals. Pupillary sizes were recorded before, during, and after transcranial magnetic stimulation (TMS) of the primary visual cortex. A significant increase in papillary sizes was found during TMS in the patients with glaucoma, which was more pronounced then in the healthy individuals. After TMS, the sizes of pupils were significantly decreased in the patients with glaucoma; however, they were significantly larger than those before stimulation. In some patients, dysfunct

Transcranial magnetic stimulation for differential diagnostics in patients with parkinsonism.

OBJECTIVE: We investigated transcranial magnetic stimulation (TMS) parameters in patients with parkinsonism, particularly in the early stages of the disease. SUBJECTS AND METHODS: We performed TMS in 48 patients with PD, progressive supranuclear palsy (PSP) and multiple system atrophy (MSA). We measured motor threshold (MT), latency (L), motor-evoked potential amplitude and central motor conduction time (CMCT) and cortical silent period (CSP). Furthermore, we selected and compared 27 patients with a disease duration of less than 3 years. RESULTS: CMCT, MT, L and CSP were different among the

Therapeutic and neurophysiologic aspects of transcranial magnetic stimulation in schizophrenia.

The use of repetitive transcranial magnetic stimulation (rTMS) in psychiatry provides the therapeutic field with a new tool. Since its introduction in the mid 1980s, the vast majority of studies have focussed on depression. A growing body of evidence suggests that rTMS is effective in the treatment of depression if dorsolateral prefrontal cortex is stimulated. Less is known about its efficacy in schizophrenia. Neuroimaging investigations in schizophrenia suggest abnormalities in the prefrontal and temporoparietal cortex (TPC), which are correlated with psychopathological dimensions. Based on i
