Toward trustworthy healthcare AI: designing academic research for translation readiness.

In controlled proof-of-concept studies, healthcare artificial intelligence (AI) systems now routinely match or exceed specialist-level performance. Despite these impressive technical achievements, most systems remain confined to research settings, highlighting an opportunity to better prepare academic AI research for subsequent clinical translation. Successful translation requires attention to technical, regulatory, organizational, and infrastructural factors, ideally from the earliest stages of research. This Perspective focuses on trustworthiness as a key factor that academic researchers can

Translating biotechnology to knowledge-based innovation, peace, and development? Deploy a Science Peace Corps--an open letter to world leaders.

Scholarship knows no geographical boundaries. This science diplomacy and biotechnology journalism article introduces an original concept and policy petition to innovate the global translational science, a Science Peace Corps. Service at the new Corps could entail volunteer work for a minimum of 6 weeks, and up to a maximum of 2 years, for translational research in any region of the world to build capacity manifestly for development and peace, instead of the narrow bench-to-bedside model of life science translation. Topics for translational research are envisioned to include all fields of life

Paediatric biobanking for health: The ethical, legal, and societal landscape.

Biobanks play a central role in pediatric translational research, which deals primarily with genetic data from sample-based research. However, participation of children in biobanking has received only limited attention in the literature, even though research in general and in clinical trials in particular have a long history in involving minors. So, we resolved to explore specific challenging ethical, legal, and societal issues (ELSI) in the current pediatric biobanking landscape to propose a way forward for biobanking with children as partners in research. Methodologically, we first establish

Extracellular vesicles in osteoarthritis: pathological mechanisms, regenerative therapies and clinical translation.

Osteoarthritis (OA) is a common chronic degenerative joint disease that has severe impacts on quality of life and creates a substantial socioeconomic burden. Conventional therapeutic strategies primarily focused on alleviating symptoms such as pain and functional impairment, with limited efficacy and an inability to reverse the disease process. Over the past two decades, regenerative medicine has been developed rapidly. Extracellular vesicles (EVs) have garnered widespread attention and there has been an upsurge of research interest in their immunomodulatory and regenerative potential in OA. H

Sepsis-associated endothelial dysfunction: clinical phenotypes, biomarkers, bedside evidence, and translational implications.

Sepsis is a systemic vascular disorder in which endothelial activation, dysfunction, and structural injury contribute to shock, microcirculatory failure, immunothrombosis, and multiorgan dysfunction. Sepsis-associated endotheliopathy is not a uniform process but a dynamic trajectory that evolves from adaptive endothelial activation toward functional disruption, structural injury, and organ-specific vascular failure. In this Review, we propose a phenotype- and trajectory-oriented framework that integrates clinical manifestations, molecular mechanisms, and translational assessment of endothelial

Aptamers in Osteoimmune Regulation: Scientific Rationale, Emerging Evidence and Translational Challenges.

Osteoimmune disorders are difficult to treat because pathogenic signals are spatially restricted, temporally dynamic, and distributed across immune, skeletal, vascular, and microbial compartments. Aptamers provide a programmable recognition platform that may address these limitations, although their application in osteoimmune systems remains largely exploratory. This critical narrative review defines the scientific rationale for aptamer-based osteoimmune regulation, maps the current state of aptamer research across biological axes closely associated with osteoimmune regulation, and highlights

Plasma-based strategies for systemic rejuvenation: critical perspectives on clinical translation.

Experimental models such as heterochronic parabiosis and heterochronic plasma transfer have profoundly advanced our understanding of systemic aging, demonstrating that circulating factors can influence brain, vascular, and immune aging through cell nonautonomous mechanisms. These preclinical models have revealed that both pro-geronic and anti-geronic signals in blood can modulate neuroinflammation, neurovascular health, and cognitive resilience. However, despite their experimental promise, the clinical translation of these findings, particularly through plasma-based interventions in humans, re

Macrophage Iron Metabolism Mediates Immunometabolic Reprogramming and Tissue Homeostasis: From Molecular Mechanisms to Clinical Translation.

BACKGROUND: Macrophages were long regarded as passive executors of erythrophagocytosis responsible for systemic iron recycling. However, increasing evidence has reframed them as immunometabolic hubs that sense diverse environmental cues to modulate systemic iron homeostasis. MAIN BODY: This review examines the molecular architecture underlying macrophage iron metabolism and outlines how iron metabolic processes are dynamically regulated across spatial and temporal scales through the integration of mechanotransductive, mitochondrial, and epigenetic signaling pathways. Across disease contexts,

Extracellular vesicles for abdominal aortic aneurysm: mechanisms, therapeutic potential, and translational challenges.

Abdominal aortic aneurysm (AAA) is a progressive and potentially fatal vascular disease for which no effective pharmacological therapy is currently available. While surgical repair remains the only definitive treatment for advanced aneurysms, patients with small AAAs are mainly managed by surveillance, highlighting the need for disease-modifying strategies. Extracellular vesicles (EVs) have emerged as promising cell-free therapeutic tools because of their biocompatibility, ability to transfer bioactive cargo, and capacity to regulate multiple pathological processes involved in AAA progression.

Translational perspectives of next-generation cytokine therapies for musculoskeletal disorders.

Musculoskeletal disorders (MSDs) are a leading cause of chronic pain and disability worldwide, yet available pharmacological treatments remain largely ineffective or palliative. Increasing evidence indicates that chronic inflammation in MSDs reflects not only excessive pro-inflammatory signaling but also impaired endogenous anti-inflammatory and pro-resolving mechanisms. In this context, cytokines IL-34 through IL-41 have recently emerged as important regulators of immune activation, tissue remodeling, and tissue regeneration. This review provides a mechanistically informed analysis of the rol

Targeting the Wnt signaling network in endometriosis: mechanistic insights and translational opportunities†.

Endometriosis is a chronic gynecological disorder defined by the growth of endometrial-like tissue outside the uterus, driven by estrogen-dependent inflammation and progressive fibrosis. According to available data, remodeling and persistent abnormal activation of the Wnt/β-catenin signaling network constitute one of the main molecular bases underlying its progression. Pathological characteristics of this disease include increased proliferation and anti-apoptotic potential of ectopic endometrial cells, as well as invasive phenotypes and progressive fibrosis. The Wnt/β-catenin pathway contribut
