A more persistent tolerance to islet allografts through bone marrow transplantation in minimal nonmyeloablative conditioning therapy.

INTRODUCTION: Islet transplantation is a therapeutic approach to prevent diabetes complications. However, the side effects of the required lifelong immunosuppressive regimens to prevent graft rejection restrict the impact of type 1 diabetes. One strategy to overcome these limitations is tolerance induction and graft acceptance through hematopoietic chimerism. In this study we investigated whether tolerance to major histocompatibility complex (MHC) and minor-disparate islet allografts could be induced by minimal nonmyeloablative conditioning and whether more persistent donor-specific islet allo

Allogeneic hematopoietic stem-cell transplantation, mixed chimerism, and tolerance in living related donor renal allograft recipients.

OBJECTIVE: We designed a prospective, randomized, and controlled clinical trial to evaluate the efficacy and safety of achieving a mixed chimerism-associated tolerance protocol for recipients of living related donor (LRD) renal allografts. PATIENTS AND METHODS: Sixty-six consecutive patients were divided into two equal groups of 33 patients with end-stage renal disease. They were enrolled for transplantation after negative lymphocytotoxicity cross-matching (LCM). Both groups (treated [Tn] and control [Cn]) showed similar clinical and laboratory parameters and donor HLA match profiles. The Tn

Vascularized thymic lobe transplantation in miniature swine: thymopoiesis and tolerance induction across fully MHC-mismatched barriers.

As the major site of self-nonself discrimination in the immune system, the thymus, if successfully transplanted, could potentially carry with it the induction of central tolerance to any other organ or tissue from the same donor. We have recently developed a technique for transplantation of an intact, vascularized thymic lobe (VTL) in miniature swine. In the present study, we have examined the ability of such VTL allografts to support thymopoiesis and induce transplantation tolerance across fully MHC-mismatched barriers. Six miniature swine recipients received fully MHC-mismatched VTL grafts w

Simultaneous donor bone marrow and cardiac transplantation: can tolerance be induced with the development of chimerism?

Mixed bone marrow chimerism (mixed chimerism) is defined by the coexistence of two genetically different bone marrow stem cells in an individual. The chimeric immune system recognizes donor antigen as self, yet is capable of mounting a normal response to third-party antigens. In animal models, mixed chimerism confers donor-specific tolerance for solid-organ and cellular grafts: tissue from the bone marrow donor is permanently accepted by mixed chimeras in the absence of conventional immunosuppressive agents. Clinical application of mixed chimerism to induce transplantation tolerance requires n

Innovative approaches targeting innate immune cells to promote organ transplant tolerance.

Achieving long-term allograft survival while minimizing systemic immunosuppression (IS) remains a critical unmet need in transplantation. While adaptive immunity has traditionally been the primary focus of IS therapy, innate immune cells-that include neutrophils, monocytes, macrophages, dendritic cells, myeloid-derived suppressor cells, innate lymphoid cells (ILCs), natural killer (NK) cells, and gamma delta (γδ) T cells act as key upstream orchestrators of allograft rejection and tolerance. Recent advances in single-cell RNA sequencing and spatial transcriptomics have unveiled the profound he

Exploring the Interplay Between Depression, Anxiety, and Immune Tolerance in Liver Transplant Recipients.

OBJECTIVE: Liver transplantation requires lifelong immunosuppression, which has significant psychological and physiological implications. While depression and anxiety are prevalent among transplant recipients, their potential relationship with immune tolerance remains unclear. This study aims to evaluate the association between depression, anxiety, and immune tolerance in liver transplant (LT) recipients. METHODS: This cross-sectional study included 179 adult LT recipients, categorized as tolerant ( n =49) and nontolerant ( n =130) based on immunosuppressive status. Depression and anxiety lev

Faecal microbiota transplant in Parkinson's disease: pilot study to establish safety & tolerability.

Emerging evidence suggests gut microbiota differences in Parkinson's Disease (PD) may impact disease progression and treatment. Faecal Microbiota Transplantation (FMT) offers a potential therapeutic approach. We conducted an open-label pilot study to assess the safety, tolerability, and symptom impact of FMT in 12 patients with mild to moderate PD, administered via enema for 6 months. FMT was safe and well tolerated, causing only mild, transient gastrointestinal symptoms. While no significant motor symptom changes were observed, there was a trend toward reduced daily OFF time at 2 months. Whil

Efficacy and tolerance of immune checkpoint inhibitors in transplant patients with cancer: A systematic review.

Solid organ transplant (SOT) is frequently complicated by cancers, which render immunosuppression challenging. Immune checkpoint inhibitors have emerged as treatments for many cancers. Data are lacking regarding efficacy and rejection risk in the SOT population. We conducted a systematic literature review and analyzed 83 cases of immune checkpoint inhibitor use for cancer in SOT. Two thirds of these patients received anti-programmed death ligand 1 therapy, 15.7% received anti-cytotoxic T lymphocyte-associated protein 4 therapy, and 10.8% received a combination. Allograft rejection occurred in

Nonchimeric HLA-Identical Renal Transplant Tolerance: Regulatory Immunophenotypic/Genomic Biomarkers.

We previously described early results of a nonchimeric operational tolerance protocol in human leukocyte antigen (HLA)-identical living donor renal transplants and now update these results. Recipients given alemtuzumab, tacrolimus/MPA with early sirolimus conversion were multiply infused with donor hematopoietic CD34(+) stem cells. Immunosuppression was withdrawn by 24 months. Twelve months later, operational tolerance was confirmed by rejection-free transplant biopsies. Five of the first eight enrollees were initially tolerant 1 year off immunosuppression. Biopsies of three others after total

The dual role of T cells in solid organ transplant rejection and immune tolerance.

Solid organ transplantation, a major breakthrough in modern medicine, has saved countless patients with end-stage organ failure. However, immune rejection remains the primary obstacle to transplant success. As the central effector cells of adaptive immunity, T cells drive acute rejection by directly recognizing donor alloantigens (such as MHC molecules) or indirectly recognizing processed donor antigens presented by antigen-presenting cells. Recent studies have revealed the dual roles of distinct T cell subsets in rejection or tolerance: pro-inflammatory Th1, Th2, Th17, and CD8+ cytotoxic T ce

Optimizing the in vitro production of immunomodulatory cells for the induction of tolerance in solid organ transplantation.

BACKGROUND: Cell therapy can be utilized to induce operational tolerance following solid organ transplantation. In thi study, donor-specific immunomodulatory cells (DSIMC) are generated by co-culturing recipient peripheral blood mononuclear cells (PBMC) with irradiated donor PBMC in the presence of belatacept, a CTLA-4-IgG1 fusion protein. DSIMC promote a regulatory response to donor cells. Reinfusion of these cells into the recipient may induce donor-specific tolerance, enabling weaning or complete cessation of immunosuppression (IS). AIM: This study aims to determine optimal culture conditi

Tolerability and clinical efficacy of letermovir for primary cytomegalovirus prophylaxis after heart transplantation.

BACKGROUND: Letermovir (LET) is a newer antiviral that has successfully been used for primary cytomegalovirus (CMV) prophylaxis in kidney transplant recipients. Valganciclovir (VGC), the current first-line CMV antiviral, is effective at prophylaxis but carries a significant risk of myelotoxicity with related downstream consequences. We studied the tolerability and clinical effectiveness of LET for primary CMV prophylaxis after heart transplant (HT) and compared rates of neutropenia and CMV disease to a historical HT cohort. METHODS: All single-organ, first-time HTs at our center who gave info

Targeting Regulatory T Cells for Transplant Tolerance: New Insights and Future Perspectives.

BACKGROUND: Organ transplantation is considered the ultimate therapy for end-stage organ disease. While pharmacologic immunosuppression is the mainstay of therapeutic strategies to prolong the survival of the graft, long-term use of immunosuppressive medications carries the risk of organ toxicity, malignancies, serious opportunistic infections, and diabetes. Therapies that promote recipient tolerance in solid organ transplantation are able to improve patient outcomes by eliminating the need for long-term immunosuppression. SUMMARY: Establishing tolerance to an allograft has become an area of

Tempering allorecognition to induce transplant tolerance with chemically modified apoptotic donor cells.

The development of organ transplantation as a therapy for end-stage organ failure is among the most significant achievements of 20th century medicine, but chronic rejection remains a barrier to achieving long-term success. Current therapeutic regimens consist of immunosuppressive drugs that are efficient at delaying rejection but are associated with significant risks such as opportunistic infections, toxicity, and malignancy. Thus, the induction of specific immune tolerance to transplant antigens is the coveted aim of researchers. The use of 1-ethyl-3-(3-dimethylaminopropyl)carbodiimide (ECDI)

Heart allograft tolerance induced and maintained by vascularized hind-limb transplant in rats.

Organ/tissue transplantation has become an effective therapy for end-stage diseases. However, immunosuppression after transplantation may cause severe side effects. Donor-specific transplant tolerance was proposed to solve this problem. In this study, we report a novel method for inducing and maintaining heart allograft tolerance rats. First, we induced indefinite vascularized hind-limb allograft survival with a short-term antilymphocyte serum + Cyclosporine A treatment. Peripheral blood chimerism disappeared 6-7 weeks after immunosuppression was withdrawn. Then the recipients accepted seconda

Induction of tolerance to an allogeneic skin flap transplant in a preclinical large animal model.

Clinical composite tissue allotransplantation can adequately reconstruct defects that are not possible by other means. However, immunosuppressant toxicity limits the use of these techniques. Clinical attempts to reduce the amount of immunosuppression required by induction of an immunologically permissive state have so far been unsuccessful. The aim of this study was to induce tolerance in a preclinical large animal model. Donor hematopoietic stem cell (HSC) engraftment was induced by T-cell depletion, irradiation, and a short course of cyclosporine administered to the recipient, along with a h

Minimal Development of Liver Fibrosis in Adult Tolerant Liver Transplant Recipients Late After Immunosuppressive Drug Weaning and Transplantation.

BACKGROUND: Operationally tolerant liver transplant (LTx)-recipients can be weaned off immunosuppressive (IS) drugs without development of graft rejection. However, it is feared that liver fibrosis might develop after complete IS weaning. The purpose of this small single-center study was to assess liver fibrosis in adult tolerant LTx recipients long after LTx and IS weaning. METHODS: Liver fibrosis was assessed in adult tolerant LTx-recipients (n = 9) using noninvasive transient elastography and measurements of multiple pro- and antifibrotic serum markers associated with liver fibrosis.
