Light aircraft crash--a case analysis of injuries.

BACKGROUND: As air travel increases and the number of commercial and non-commercial flights rises, so does the number of aircraft accidents. The improved safety standards of the aviation industry result in a growing number of survivors of aircraft crashes, but there are no management guidelines for the treatment of these survivors. OBJECTIVES: To present our experience in treating five survivors of a light aircraft crash that occurred in August 1995 near Jerusalem. RESULTS: All five survivors sustained vertebral column injuries, which was the only injury in most of the survivors. We discuss

[Clinical-radiologic diagnostics of cranio-cerebral injuries in polytrauma].

A retrospective analysis of clinical-radial examination was made in 1946 patients with polytrauma. In 82% of them severe cranio-cerebral traumas and extracranial injuries were revealed. The diagnosis of fractures of the skull bones on the basis of survey radiography was possible but in 2% of the patients. Cranio-cerebral and craniofacial traumas appeared much more often than injuries of other anatomical areas, reaching 79.4%. Spiral computed tomography was the main method of early diagnosing polytrauma which allowed detecting the character, localization and volume of traumatic injuries of the

One-day surgery in multiple trauma patients.

The aim of the authors was evaluation of influence of progress in MT procedures, particularly on final results of treatment, with special attention to aggressive one-day surgery, including definitive orthopaedic operations. The study is based on own material from the 2-nd Department of Surgery of the Jagellonian University. From almost 3000 victims of accidents with multitrauma (MT), who were treated between 1968 and 1997, 932 patients were selected. They were treated in consecutive 2-year periods in a 5-year cycle, starting from 70/71 up to 95/96. From 1987, the patient material has been pros

Computerised tomographic patterns in patients with head injury at the university of Benin teaching hospital.

BACKGROUND: Head injuries rank high among morbidities due to trauma. Computerised tomography is an important modality in the investigation of these cases. However, literature on this subject in the south-south geopolitical zone of Nigeria is sparse. This study therefore aimed to document the computerized tomographic features of patients with head injury managed at the University of Benin Teaching Hospital (UBTH). MATERIALS AND METHODS: A prospective study involving patients with head injury referred for CT scan from the Accident and Emergency Unit of UBTH over a 12-month period. A total of 10

Management of traumatic hemipelvectomy: an institutional experience on four consecutive cases.

BACKGROUND AND OBJECTIVE: The incidence of traumatic hemipelvectomy is rare, but it is a devastating injury. Recently, an increasing number of patients with traumatic hemipelvectomy are admitted to trauma centers alive due to improvements of the pre-hospital care. Successful management requires prompt recognition of the nature of this injury and meticulous surgical technique. We present our successful experiences on four cases of traumatic hemipelvectomy in the past nine years. PATIENTS AND METHODS: Four cases with traumatic hemipelvectomy were admited to our hospital from June 21, 2002 to Se

Clinical criteria for obtaining maxillofacial computed tomographic scans in trauma patients.

BACKGROUND: Over 150,000 patients present with maxillofacial trauma annually to emergency rooms in the United States. Although maxillofacial computed tomography is a sensitive screening tool for identifying facial fractures, indiscriminate use leads to unnecessary radiation exposure and substantial costs. A decision instrument is needed to ensure computed tomographic evaluation of patients at high risk for facial fracture and limit computed tomography use in low-risk patients. METHODS: A retrospective review was conducted of all patients evaluated at a Level I trauma center over a 3-year peri

Evaluation of the field relevance of several injury risk functions.

An evaluation of the four injury risk curves proposed in the NHTSA NCAP for estimating the risk of AIS>= 3 injuries to the head, neck, chest and AIS>=2 injury to the Knee-Thigh-Hip (KTH) complex has been conducted. The predicted injury risk to the four body regions based on driver dummy responses in over 300 frontal NCAP tests were compared against those to drivers involved in real-world crashes of similar severity as represented in the NASS. The results of the study show that the predicted injury risks to the head and chest were slightly below those in NASS, and the predicted risk for the kne

Outcome of cervical near-hanging injuries.

BACKGROUND: Cervical near-hangings are not rare, but have received little attention in the trauma literature. Increasing numbers of patients received from our local jail and detention centers prompted this study. METHODS: Seventeen-year review of a level I Trauma Center Registry identified 67 patients with cervical strangulation for study. Data were analyzed using the Mann-Whitney test to evaluate continuous predictors, and Fisher's exact test for categorical predictors. RESULTS: Ten of 67 patients died (14.9% mortality). Patients having a lower Glasgow Coma Score (GCS) at the scene (3.5 +/-

Morbidity associated with isolated iliac wing fractures.

BACKGROUND: Iliac wing fractures have received little attention in the literature. Commonly occurring after a direct blow, they are rotationally and vertically stable. However, one must be aware of their potential for major morbidity including soft tissue, abdominal organ, and vascular lesions. This study examines the incidence, radiographic findings, soft tissue injuries, and complications associated with isolated iliac wing fractures. METHODS: Retrospective review between 2003 and 2006 of a consecutive series of 450 polytraumatized patients treated in a level-1 University trauma center. Hos

Traumatic degloving lesion of male external genitalia.

Injury to the male external genitalia is relatively uncommon when compared with other parts of the human body. Trauma to external genitalia could result from accidents involving motor vehicles, power farm machinery, gun shot and stab injuries. We present a rare case of traumatic degloving injury of male external genitalia associated with partial penile amputation and loss of both testes sparing the internal abdominal and pelvic organs, resulting from run over by a heavy vehicle tyre.

[Treatment results in 588 patients with multiple trauma].

BACKGROUND: Ten to fifteen million wounded and 700,000 deaths each year, around the world, are the consequences of accidents, according to the World Health Organization. In Chile 2,269 deaths occur each year due to accidents. The successful treatment of these patients requires a schematized initial management, that is taught in the Advanced Trauma Life Support Course. AIM: To review the results of trauma treatment using this model at Hospital del Trabajador in Santiago. PATIENTS AND METHODS: A retrospective review of patients with trauma treated between 1984 and 1994. The severity of trauma

Train versus pedestrian accidents.

Railroad-related trauma has been associated with injuries causing considerable morbidity. This is a retrospective study of 15 consecutive patients who were run over by trains. The initial injury produced a traumatic amputation of 10 extremities in eight patients. Limb salvage in two other patients with severe fractures failed, and subsequent lower extremity amputation was required. Despite efforts to salvage the most distal tissue possible, 8 of 12 extremities amputated in the acute traumatic period required revision to a more proximal level. Alcohol ingestion was involved in 13 of the 15 inci

[Advances in the research of extravascular lung water after severe burn].

Pulmonary edema frequently occurs after severe burn. It not only impairs pulmonary function directly, but also can induce or exacerbate other pulmonary complications such as lung infection, acute lung injury ( ALI), and ARDS. Extravascular lung water (EVLW) is closely related to the pulmonary edema. Dynamical monitor of EVLW has been used to predict and quantify the degree of pulmonary edema clinically. This review focuses on the recent progresses at home and abroad on the formation mechanism, monitoring approach, and prevention and treatment of EVLW after severe burn injury.

Sports Medicine Update: Concussion.

A systematic approach is required for patients with a suspected concussion. Although standardized tools can aid in assessment, the diagnosis of concussion remains a clinical one. At the time of diagnosis, patients should be given both verbal and written review of the common symptoms of concussion, expected course of recovery, as well as strategies to manage symptoms. Most patients benefit from a brief period of rest, followed by a gradual reintroduction of activities, and a graduated return-to-sport protocol. Patients with prolonged recovery from a concussion may benefit from exercise, vestibu
