Although varicella is one of the most common infectious diseases in the United States, systemic and ocular complications are rare. We report a patient who developed disciform edema followed by microdendritic keratitis 1 and 2 months, respectively, after resolution of the acute phase of varicella. Cultures were negative, but serologic analysis found positive antibodies against varicella zoster virus and negative antibodies against herpes simplex virus. This is a report of a 30-year-old woman who had a herpetic corneal ulcer following radial keratotomy. Clinical applications of the results of this case reveal the following questions. (1) Can antiviral metabolites be used to prevent corneal scars, especially in the visual axis? (2) Should radial keratotomy be avoided during the vulnerable menstrual cycle period of females to prevent possible triggering of a herpetic ulcer regardless of previous herpes history?

We report 3 patients who experienced a recurrence of nonherpetic keratitis after excimer laser photorefractive surgery. Two patients had a history of culture-positive adenoviral keratoconjunctivitis, and 1 had a clinical diagnosis of Thygeson's superficial punctate keratitis (SPK) prior to excimer laser surgery. Patients should be informed that excimer laser surgery may contribute to a recurrence of keratitis and Thygeson's SPK. Recognition and appropriate treatment can result in resolution and maintenance of a good refractive outcome.

Eye disease due to herpes simplex virus (HSV) is a leading cause of ocular morbidity and the number one infectious cause of unilateral corneal blindness in the developed parts of the globe. Previous studies to determine the efficacy of new antiviral compounds in treating HSV ocular infections have used rabbit models. The eye presents a unique model for the study of the pathogenesis of viral disease, drugs, and local and general host factors. Disease is easily observed and quantitated, and the presence of two organs sometimes permits controlled observations that are otherwise impossible. Perhaps most important, ocular viral infection is disabling and blinding, and the success of its treatment with antiviral drugs has clearly demonstrated the potential for the use of antiviral drugs in other areas.

The relative efficacy of aciclovir (ACV) and trifluorothymidine (TFT) was evaluated in a randomized, open clinical trial of 37 patients with herpetic kerato-uveitis. Twenty-one patients were treated with ACV and 16 with TFT. Topical steroids were withheld as long as the degree of inflammation permitted.
