[Antimycotic therapy in otomycosis with tympanic membrane perforation].

Especially after prolonged antibiotic ototopic therapy otomycosis is not rare. An inoculation of fungi into the tympanic cavity however may have serious sequelae. Therefore an eradication of fungi from the external auditory canal is imperative before surgery. In addition to thorough cleaning of the outer ear canal antimycotic preparations are recommended in treating otomycosis. However, all of the commercially available ear drops contain ototoxic agents. In the case of defects of the tympanic membrane a damage of the inner ear may result. Alternatively, we suggest an aqueous solution of Micona

[Acoustic and mechanical properties of tympanic membrane transplants].

BACKGROUND: The human tympanic membrane has reasonably good sound transmission properties and withstands high static pressure loads. Destruction of the tympanic membrane resulting from middle ear diseases or trauma may be repaired by different types of grafts. Middle ear surgery mostly uses autologous temporal fascia, cartilage, or cartilage perichondrium transplants which differ in their acoustical characteristics and mechanical strength. METHODS: We have investigated the acoustical and mechanical properties of these materials and compared them with human tympanic membranes by constructing a

The anterior hitch method of tympanic membrane repair.

Anterior perforations of the tympanic membrane have been previously shown to have a higher failure rate than others following tympanoplasty procedures. A technique has been previously described utilizing an anterior hitch graft stabilization with avoidance of anterior blunting of the tympanic membrane. Follow-up cases in which this technique has been utilized show greatly improved results in terms of perforation closure.

Tympanic membrane injury in welders: is prevention neglected?

Injury to the ear in welders is a recognized but poorly documented entity. Five cases are described to demonstrate a spectrum of severity of injury, ranging from minor burns to tympanic membrane perforation requiring myringoplasty. Welders are most at risk when working overhead, in cramped conditions or adjacent to other welders. Conventional safety helmets offer inadequate protection to the ears and it is recommended that insert ear plugs should be offered to all workers at risk, in addition to safer working practices. An increased awareness of these risks among occupational physicians, welde

Effects of otic drops on chinchilla tympanic membrane.

Experimental studies have shown that if antibiotic otic drops reach the middle ear cavity they produce severe inflammation. However, the effects of these preparations on the tympanic membrane have not been thoroughly investigated. This study was designed to assess morphological changes in the chinchilla tympanic membrane two to 21 days after a single 0.2-mL application of an antibiotic otic preparation (Cortisporin Otic Suspension) to the middle ear cavity. At two days, the epidermal and the mucosal layers were destroyed. By four days, reepithelialization had occurred and all layers of the tym

[Clinical study on perforation of the tympanic membrane and discussion based on experimentally induced tympanic rupture].

UNLABELLED: Traumatic perforation of the tympanic membrane (TM) was clinically studied by analyzing photographs of 231 ears treated in the Department of Otolaryngology of Tokyo Women's Medical College from May 1983 to December 1991. The dynamic features of TM and the mechanism of perforation are discussed. RESULTS AND DISCUSSION: 1) Direct and indirect injuries of the TM through the external auditory canals were observed in 101 and 130 ears, respectively. One hundred and twenty-seven injured ears were seen in males and 104 in females. Subjects ages ranged from 1 to 64 years. One hundred and t

Application of Multispectral Imaging in the Human Tympanic Membrane.

Multispectral imaging has recently shown good performance in determining information about physiology, morphology, and composition of tissue. In the endoscopy field, many researches have shown the ability to apply multispectral or narrow-band images in surveying vascular structure based on the interaction of light wavelength with tissue composition. However, there has been no mention to assess the contrast between other components in the middle ear such as the tympanic membrane, malleus, and the surrounding area. Using CT, OCT, or ODT can clearly describe the tympanic membrane structure; never

The effect of surgeon experience on tympanic membrane closure.

OBJECTIVE: Review of the English language literature finds little documentation of the relation of otology or otolaryngology outcomes to a surgeon's age, years in practice, or numbers of cases previously performed. Because of one surgeon's adoption of a new tympanoplasty technique for uncomplicated tympanic membrane perforations, our institution was situated to report an example of a surgical learning curve with its outcome. Experience versus outcome is worth establishing objectively because these relationships reflect on training and certification. DESIGN: This retrospective review of the me

Middle ear effusions and structure of the tympanic membrane.

OBJECTIVE: To study the effect of various middle ear effusions on the structure of the lamina propria of the tympanic membrane. METHODS: Sterile and infective middle ear effusions were induced by obstruction of the eustachian tube in specific pathogen-free (SPF) rats and in rats with upper airway infections (URI), respectively. The condition of the tympanic membrane was monitored at regular intervals. After varying survival times, the animals were killed and the tympanic membranes processed for light and electron microscopy. RESULTS: Sterile effusions always resulted in tympanosclerotic lesi

Early repair of blast ruptures of the tympanic membrane.

A method and results of early repair of blast rupture of the tympanic membrane in 24 selected cases, where flaps and/or free edges of the torn eardrum could be detected by examination with the operating microscope, are presented. By reversion of the flaps and approximation of the edges, the perforation was closed or significantly diminished. This ensured the healing of the tympanic membrane in all cases within days instead of months. No complications were noted during a subsequent two to three-years' follow-up. The improved hearing was stable.
