[Privatizing Universalism in the Brazilian health system and the Now There Are Specialists Program].

This article reflects on the Now There Are Specialists Program (PATE, acronym in Portuguese) in light of the concept of Privatizing Universalism in the Brazilian health system. The program, presented by the Brazilian Ministry of Health, aims to expand its capacity to provide care and reduce patient waiting lists in the Brazilian Unified National Health System (SUS, acronym in Portuguese). Given the health policies introduced under the PATE, which involve health insurance plans and private hospitals, it is essential to analyze the Program based on the theoretical framework of Privatizing Univer

Bridging geographical disparities across 368 townships with healthcare system and socioeconomic factors in Taiwan.

A universal health insurance program such as the National Health Insurance in Taiwan offers a wide coverage and increased access to healthcare services. Despite its ongoing efforts to enhance healthcare accessibility, differences in health for people living in urban and resource-deprived areas remain substantial. To investigate the longitudinal impact of the healthcare system and other potential structural drivers such as education and economic development on geographical disparities in health, we designed a panel study with longitudinal open secondary data, covering all 368 townships in Taiwa

Health policy, collaboration and investment in the post-COVID era: a review from UHC and health security integration perspective.

BACKGROUND: The COVID-19 pandemic exposed weaknesses in health systems globally, highlighting chronic underinvestment, fragmentation, and a lack of preparedness with catastrophic impact on public health, economies and societies. Lessons learned reconfirmed the necessity to build health systems resilience, integrating efforts to achieve Universal Health Coverage (UHC) and health security in tandem. RESULTS: This study identified 63 global and regional policies, collaborations, and investments (collectively termed “initiatives”) that came out post-COVID-19 and reviewed them in reference to their

Did the Expansion of Insurance Coverage for Oral Health Reduce Self-reported Oral Health Inequalities in Korea? Results of Repeated Cross-Sectional Analysis, 2007-2015.

BACKGROUND: In 2009, the South Korean government expanded universal health insurance to include oral health services. In the present study, we sought to examine whether improved access resulted in a reduction in income-based self-reported oral health inequalities. METHODS: We analyzed repeated cross-sectional data from the Korea National Health and Nutrition Examination Survey (KNHANES) waves IV through VI (2007-2015). We analyzed self-reported oral health status among 68,431 subjects. Changes in oral health inequalities across four income levels (low, middle-low, middle-high, and high) were

Validating the implementation of an indicator reporting policies and laws on free public maternal health-related services in the era of universal health coverage: A multi-country cross-sectional study.

BACKGROUND: The concept of universal health coverage (UHC) encompasses both access to essential health services and freedom from financial harm. The World Health Organization's Maternal Newborn Child and Adolescent Health (MNCAH) Policy Survey collects data on policies that have the potential to reduce maternal morbidity and mortality. The indicator, "Are the following health services provided free of charge at point-of-use in the public sector for women of reproductive age?", captures the free provision of 13 key categories of maternal health-related services, to measure the success of UHC im

Willingness-to-pay for community-based health insurance among formal and informal doctors in Dhaka, Bangladesh: a comparative cross-sectional study.

INTRODUCTION: Health insurance enables financial protection and reduces medical burdens. Currently, less than 1% of Bangladeshi citizens have health insurance coverage. We aimed to assess the willingness-to-pay for community-based health insurance and explore associated factors among formal and informal doctors in Bangladesh. METHODS: This comparative cross-sectional study was conducted on Bangladeshi doctors with formal and informal employment. For each group, 116 doctors were selected via purposive sampling, as no national register of doctors was available, indicating their current employme

Integrating oral health into Universal Health Coverage in Europe: A cross-sectional ecological analysis of services and outcomes.

Integration of oral health within Universal Health Coverage (UHC) remains uneven across Europe, and comparative evidence linking statutory dental coverage and public financing with oral health outcomes is limited. This study assessed relationships between integration of essential oral health services within UHC and prevalence of untreated dental caries and self-reported unmet need for dental care across European countries. A cross-sectional ecological analysis was conducted using publicly available, country-level data for 34 WHO European Region countries (indicator years 2019-2023). Outcomes w

Measuring universal health coverage in reproductive, maternal, newborn and child health: An update of the composite coverage index.

BACKGROUND: Monitoring universal health coverage in reproductive, maternal and child health requires appropriate indicators for assessing coverage and equity. In 2008, the composite coverage index (CCI)-a weighted average of eight indicators reflecting family planning, antenatal and delivery care, immunizations and management of childhood illnesses-was proposed. In 2017, the CCI formula was revised to update the family planning and diarrhea management indicators. We explored the implications of adding new indicators to the CCI. METHODS: We analysed nationally representative surveys to investi

Scaling up effective treatment of hypertension-A pathfinder for universal health coverage.

High blood pressure is the world's leading cause of death, but despite treatment for hypertension being safe, effective, and low cost, most people with hypertension worldwide do not have it controlled. This article summarizes lessons learned in the first 2 years of the Resolve to Save Lives (RTSL) hypertension management program, operated in coordination with the World Health Organization (WHO) and other partners. Better diagnosis, treatment, and continuity of care are all needed to improve control rates, and five necessary components have been recommended by RTSL, WHO and other partners as be

Does the operations of the National Health Insurance Scheme (NHIS) in Ghana align with the goals of Primary Health Care? Perspectives of key stakeholders in northern Ghana.

BACKGROUND: In 2005, the World Health Assembly (WHA) of the World Health Organization (WHO) urged member states to aim at achieving affordable universal coverage and access to key promotive, preventive, curative, rehabilitative and palliative health interventions for all their citizens on the basis of equity and solidarity. Since then, some African countries, including Ghana, have taken steps to introduce national health insurance reforms as one of the key strategies towards achieving universal health coverage (UHC). The aim of this study was to get a better understanding of how Ghana's health

Is the neglected tropical disease mass drug administration campaign approach an effective strategy to deliver universal health coverage? A case study of the Liberia neglected tropical disease programme.

BACKGROUND: Access to affordable, quality healthcare is the key element of universal health coverage (UHC). This study examines the effectiveness of the neglected tropical disease (NTD) mass drug administration (MDA) campaign approach as a means to deliver UHC, using the example of the Liberia national programme. METHODS: We first mapped the location of 3195 communities from the 2019 national MDA treatment data reporting record of Liberia. The association between coverage for onchocerciasis and lymphatic filariasis treatment achieved in these communities was then explored using a binomial geo

The health insurance plight of patients with sickle cell disease.

Despite chronic illness that requires routine medical follow-up as well as occasional acute care and hospitalizations, many persons with sickle cell disease (SCD) are not getting the care that they need. The primary reason for lack of proper care is due to the patients' lack of health insurance coverage. This paper will examine the problem of health insurance coverage among persons with chronic illnesses using SCD as a model. It is speculated by the author that persons who suffer from other chronic illnesses may also experience similar problems in obtaining health coverage. Supporting data for

Insurance Coverage for Chronic Diseases and Healthcare of Low-Income People: Evidence From Chinese Administrative Data.

Patient cost-sharing can lead to delays in necessary care, especially among low-income populations. In this study, we examine the impact of health insurance coverage for chronic disease treatments in outpatient care, using recent administrative insurance claims dataset from a low-income population in an underdeveloped city in China. Employing a propensity score matching and difference-in-differences approach, we find that outpatient coverage for certain chronic diseases increased outpatient utilization and expenses among patients with these conditions. Interestingly, these patients also increa

Absence of health insurance is associated with decreased life expectancy in patients with cystic fibrosis.

Life expectancy for individuals with cystic fibrosis (CF) has increased dramatically in the last 30 yr, but it is unclear whether the improved survival has applied equally to individuals with different health insurance status. We developed a retrospective inception cohort of all 189 patients with CF born 1/1/55 to 12/31/70 who had at least one hospitalization at a university referral center. The median survival for patients with CF who were without health insurance was 6.1 yr compared with 20.5 yr for those with Medicaid and 20.5 yr for those with private insurance. Using multivariate Cox regr

Institutional assembly under constraints: pathways to universal health coverage in Brazil, Rwanda, and China.

This paper examines how Brazil, Rwanda, and China have pursued universal health coverage (UHC) beyond classical health system models developed in high-income settings. We introduce the concept of institutional assembly under constraints, offering a complementary framework for understanding UHC pathways in developing countries, where no single trajectory universally applies. Brazil's health system ensures free access at the point of care through tax-based public financing, but limited government spending has produced a dualised system of public and private provision. Rwanda extends coverage to

From regulation to real legal cases: physician-patient disputes as a lens on universal health coverage governance in China, 2015-2025.

Physician-patient disputes (PPD) have been increasingly recognized as structural indicators of health system performance, yet they remain underexplored within universal health coverage (UHC) frameworks, particularly in the Chinese context. This study employed a mixed-methods approach integrating regulatory review and legal cases analysis, revealing that the regulatory framework for PPD has evolved from fragmented arrangements toward a more integrated civil, criminal, and administrative system, encompassing three stages: ex ante prevention, in-process management, and ex post dispute resolution

HCV treatment initiation in the era of universal direct acting antiviral coverage - Improvements in access and persistent barriers.

BACKGROUND: Barriers to HCV treatment initiation persisted after the introduction of direct-acting antivirals (DAAs) in Canada among people who inject drugs (PWID); whether DAA universal coverage lifted these barriers remain unknown. We assessed the evolution of HCV treatment initiation and associated factors among PWID in Montreal, Canada, comparing eras of IFN-based regimens (2011-2013), of DAA restricted access (2014-02/2018), and universal coverage (03/2018-03/2020). METHODS: We included chronically HCV-infected participants followed in a community-based PWID cohort in Montreal, Canada be

Navigating health insurance selection for in vitro fertilization (IVF) benefits: A study protocol.

INTRODUCTION: A large public university added health insurance coverage of 50 % co-insurance for up to two cycles of in vitro fertilization (IVF) to eligible faculty and staff. METHODS: We describe the design and conduct of a randomized controlled trial to evaluate the effectiveness of a health insurance educational intervention on health insurance literacy and IVF benefit utilization. The intervention materials included 1) Key insurance terms; 2) Examples of premiums and deductibles across the insurance plan options; 3) Examples of how premiums and deductibles affect out-of-pocket costs; and

Determining the efficiency path to universal health coverage: cost-effectiveness thresholds for 174 countries based on growth in life expectancy and health expenditures.

BACKGROUND: Assessment of the efficiency of interventions is paramount to achieving equitable health-care systems. One key barrier to the widespread use of economic evaluations in resource allocation decisions is the absence of a widely accepted method to define cost-effectiveness thresholds to judge whether an intervention is cost-effective in a particular jurisdiction. We aimed to develop a method to estimate cost-effectiveness thresholds on the basis of health expenditures per capita and life expectancy at birth and empirically derive these thresholds for 174 countries. METHODS: We develop
