A laser's high degree of coherence leads to interferences, which-in the absence of precautions-can cause severe image distortions such as fringes and speckles and which thereby strongly hamper a meaningful interpretation of hyperspectral images in laser-based widefield microspectroscopy. While images and spectra of homogenous samples may already suffer from interferences, any structured object such as a tissue thin section will add to these distortions due to wavelength- and, in particular, sample-dependent phase shifts (structure sizes, absorption coefficients, refractive indices). Emergency departments (EDs) receive patients from every background, socioeconomic group and health status. Hence, EDs can play a critical role in offering human immunodeficiency virus (HIV) testing and help in the national strategy of early HIV detection. The present study was conducted on 400 patients attending various EDs after taking Institutional Review Board approval. They were screened for HIV antibodies by three rapid/simple assay tests having different principles/antigens as per the NACO guidelines. Twenty-three (5.75%) of the 400 patients were HIV reactive. Fifteen (65.22%) of the 23

Many hepatitis C virus (HCV)-infected individuals do not have any obvious risk factors and one of the putative sources of infection may be inadvertent exposure to infected blood or body fluids in the clinical setting. The aim of this study was to assess the role of medical procedures in transmission of hepatitis C in north-eastern Poland. In total, 194 patients with chronic hepatitis C were eligible for the study. The control group consisted of 275 age- and sex-matched individuals. Patients with a history of intravenous drug use were excluded. On multivariate analysis, transfusions [odds ratio

Accidental occupational infection of anaesthetists from patient body fluids is a very real and potentially fatal risk that will be significantly reduced with the routine use of universal precautions and the adoption of specific safe work practices. Employers are required by law to provide a safe working environment and safe systems for work which anaesthetists should implement according to recommendations in this paper. A protocol should be available to be acted upon in the event of occupational parenteral, mucous membrane and non-intact skin exposure to infected patient body fluids. Recommend

In the event of an influenza pandemic, the workload of healthcare workers (HCWs) would raise dramatically. Moreover, due to the nature of this occupation, one's own risk of infection is also increased. Given this background, the question arises as to whether HCWs would actually report to work during an influenza pandemic. To answer this question an anonymous and voluntary questionnaire was distributed to HCWs of a German university hospital during the seasonal influenza vaccination. In total, 36.2% of respondents declared that, in the event of a potential influenza pandemic, they would not go

BACKGROUND: Healthcare-associated infections (HAIs) remain a major threat to patient safety and healthcare workforce protection, particularly after the COVID-19 pandemic. Standard precautions (SPs) are the most fundamental infection prevention measures used to reduce pathogen transmission and occupational exposure in clinical settings. Although awareness of SPs has increased, adherence among nurses continues to vary across practice domains. Factors such as familiarity with SP-related knowledge, years of nursing experience, and perceived barriers in daily practice may influence compliance; howe

It goes without saying that the members of any professional group are more likely to modify their behavior if they are provided with logical, rational reasons to enact the suggested change. In the mid 1980s, health care providers, including dental personnel, were advised to adopt universal precautions and to alter their infection control habits with minimal justification, apart from the general unease and paranoia surrounding AIDS. Therefore, it is understandable that some practitioners would react with scepticism to the idea that their traditional infection control techniques were less than a

This study was performed to compare compliance with standard precautions for the use of multidose vials (MDVs) and fingerstick devices in emergency departments (EDs) and intensive care units (ICUs). Between December 2007 and February 2008, 389 nurses from the EDs or ICUs of six university-affiliated hospitals in Korea were asked to complete the questionnaire. A total of 338 (86.9%) nurses completed the survey, corresponding to 159 of 184 ED and 179 of 205 ICU nurses. A comparison of MDV use in EDs and ICUs indicated a significant difference only in disinfection of the rubber septum of heparin

STUDY OBJECTIVE: Exposure to HIV-1 is of profound concern to health care workers. HTLV-I and HTLV-II, retroviruses with similar modes of transmission as HIV-1, also cause disease in human beings. Emergency department resuscitations are high-risk situations for such exposure. The purpose was to determine the seroprevalence of HIV-1 and HTLV I-II in patients undergoing ED resuscitations, the magnitude of health care worker exposure, and risk factors associated with infection.
DESIGN: Prospective identity-unlinked seroepidemiologic study.
SETTING: ED of a 950-bed private inner-city teaching hos

The implementation of isolation precautions for patients with suspected Coronavirus Disease 2019 (COVID-19) and pending test results is resource intensive. Due to the limited availability of single-bed rooms at our institution, we isolated patients with suspected COVID-19 together with patients without suspected COVID-19 on-site in multiple-bed rooms until SARS-CoV-2-test results were available. We evaluated the likelihood of SARS-CoV-2 transmission to individuals sharing the room with patients isolated on-site. This observational study was performed at the University Hospital Basel, Switzerla

AIMS AND OBJECTIVES: This study aims to investigate the problems experienced by nurses and doctors as a result of exposure to surgical smoke and the precautions that need to be taken.
BACKGROUND: Electrosurgery is carried out in almost all operating rooms, and all of those who work in these rooms are exposed to surgical smoke, especially doctors and nurses. A review of the literature reveals that there are very few studies that have been carried out on surgical smoke, and there are no studies researched on the problems experienced by those working in operating rooms.
DESIGN: This descriptive

OBJECTIVES: To describe the investigation and molecular characterization of a vancomycin-resistant Enterococcus faecium (VREF) strain responsible for a nosocomial outbreak in the haematology unit of a tertiary-care university hospital.
PATIENTS AND METHODS: Two patients admitted to the haematology unit developed infection/colonization with VREF over a 3 month period when compared with none in the 2 previous years. On the basis of the identification of a clonal link between these two strains, weekly rectal screening was implemented for all patients in the haematology unit and contact precautio

Currently there is no consensus statement about the safety of electroconvulsive therapy in patients who have implanted electrodes for deep brain stimulation. We present a summary of the existing literature on this topic, consisting of 21 cases, and then report a case performed at the University of Maryland Medical Center. Notably, with appropriate safety precautions and careful patient selection, there were no adverse events reported in the literature that were related to the presence of the deep brain stimulation device in any of the cases. Based on our review of the literature and the case w

We present a 16-year-old boy, who had presented to us with catatonic features of mutism, withdrawal, passive negativism, grimacing, gesturing, echopraxia, and excitement of 5 days duration while taking antiretroviral therapy (ART) for a period of 2 years. He had history of birth asphyxia and acquired HIV infection from his father when the same syringe and needle was used on both of them in a medical setting where the father and son had consulted for treatment of pyrexia of unknown origin. He was the eldest of a three children family in which the biologic father had acquired HIV through extrama

OBJECTIVE: To review the evidence on identifying and managing misuse of and dependence on opioids among primary care patients with chronic pain.
QUALITY OF EVIDENCE: MEDLINE was searched using such terms as "opioid misuse" and "addiction." The few studies on the prevalence of opioid dependence in primary care populations were based on retrospective chart reviews (level II evidence). Most recommendations regarding identification and management of opioid misuse in primary care are based on expert opinion (level III evidence).
MAIN MESSAGE: Physicians should ask all patients receiving opioid th

In France, the prevalence of hepatitis C virus (HCV) infection in chronic hemodialysis patients is high, about 15 to 42%. Although the incidence and the prevalence of HCV infection decreased during the last decade, de novo cases of HCV infection still occur in hemodialysis units. To date the nosocomial transmission is the principal cause of contamination. The strict application of universal precautions and the separation of chronic hemodialysis patients according to their virological status may decrease the incidence of HCV infection in hemodialysis units. The transaminase level and HCV viremi
