Traumatic Urethral Pseudoaneurysm in a 12-Year-Old Boy Leading to Progressive Urethral Stricture and Complete Obstruction: A Case Report and Literature Review.

Urethral pseudoaneurysm is an extremely rare cause of urethrorrhagia in children. Most previously reported pediatric cases required transcatheter arterial embolization and generally resolved without long-term sequelae. We present a 12-year-old boy who sustained blunt perineal trauma and subsequently developed a bulbar urethral pseudoaneurysm requiring two sessions of arterial embolization. Unlike prior reports, the patient developed progressive circumferential urethral stricture leading to complete obstruction five months after initial injury. To our knowledge, this is the first reported pedia

How I Do It: The prostatic urethral lift for obstructive median lobes.

Millions of men in North America suffering from lower urinary tract symptoms (LUTS) secondary to benign prostatic hyperplasia (BPH) are managed on medical therapy. Most patients, however, report poor adherence, and yet relatively few pursue more definitive surgical solutions. The Prostatic Urethral Lift (PUL) was designed to address many of the patient-identified barriers to surgery, namely iatrogenic sexual dysfunction, incontinence, lengthy recovery and postoperative catheterization. Randomized studies and large real world multicenter and database studies have demonstrated safety and effecti

Obstructed kidney and sepsis secondary to urethral catheter misplacement into the distal ureter.

An 86-year-old woman underwent routine catheter replacement in the community. The new catheter failed to drain urine. Attempts to remove the catheter failed, both by the community nurse as well as by the urology team in the hospital. A CT scan confirmed that the catheter balloon was inflated in the distal right ureter. The patient was started on antibiotics and listed for cystoscopy under general anaesthetic. The catheter was visualised entering the right ureter and the balloon punctured using a wire under image intensifier guidance. Once removed, a new catheter was inserted. Very dilated uret

[Treating cervico-urethral obstructions with laser diode: initial clinical experience].

BACKGROUND: The first laser employment for the treatment of the cervico-urethral obstructions (CUO) dates back to the early '90s. This study begins with the analysis of laser basics in Medicine and has the purpose to weigh pros and cons (and limits as well) of laser use in CUO due to Benign Prostatic Hyperplasia (BPH). That is an ageing man's typical disease and there is a huge need of minimally invasive treatments, because people affected by this kind of illness suffer very often from comorbidities, and that could make traditional surgery quite risky. AIM OF THE STUDY: The target of this stu

Anterior urethral valves in children: an uncommon multipathogenic cause of obstructive uropathy.

INTRODUCTION: Anterior urethral valves (AUVs) are uncommon congenital anomalies causing urethral obstruction in boys. PATIENTS AND METHODS: Medical records of 13 children were reviewed retrospectively. Each patient (pt) was evaluated with voiding cysto-urethrogram (VCUG) and renal ultrasonography. Pts older than 5 years performed an uroflowmetry. Serum creatine was determined in all children. RESULTS: All pts had difficulty in voiding and eight had recurrent urinary tract infection. Renal function was normal in all pts. VCUG showed filling of Cowper's duct in a pt. In a case, a vesicouretera

Obstructing urethral calculus in a woman revealed to be the cause of chronic pelvic pain.

Urethral calculi are extremely rarely reported in Caucasian females and are usually associated with an anatomical abnormality such as a diverticulum or a stricture. Ureteric calculi can move to become lodged in the urethra, although this is rare in women because of their short urethral length. We present a case of a 55-year-old woman presenting with urinary retention secondary to an obstructing upper tract calculus that had moved into the urethra. Four years previously, the patient had been diagnosed with chronic pelvic pain following a primary posterior vaginal wall repair. Following treatmen

The use of a low-profile cystostomy tube to relieve urethral obstruction in a dog.

A 10-year-old, spayed female Dalmatian was diagnosed with granulomatous urethritis causing urethral obstruction. Due to the extensive involvement of the urethra, a urethrostomy was not possible. A commercially available, silicone, low-profile gastrostomy tube was placed as a prepubic cystostomy tube to achieve urinary diversion. This tube is easy to use, has a one-way valve, and lies flush with the skin margin, thereby decreasing the likelihood of inadvertent removal. This tube should be considered to achieve long-term urinary diversion when urethral involvement is extensive.

The varied sonographic appearance of the urinary tract in the fetus and newborn with urethral obstruction.

The urinary tract in the fetus responds to chronic obstruction differently than in adults. This difference leads to sonographic findings which are unique and occasionally misleading when urinary tract obstruction occurs in utero. These findings include development of macroscopic cysts and relative lack of dilatation of the intrarenal collecting system. Seven cases are presented to illustrate the spectrum of findings in fetal urinary tract obstruction at the urethral level, and criteria for distinguishing atypical renal obstructive disease with cysts from other forms of renal cystic disease in

Presentation Variables Associated With the Development of Severe Post-obstructive Diuresis in Male Cats Following Relief of Urethral Obstruction.

BACKGROUND: Diuresis following relief of urethral obstruction is a potentially life-threatening complication of feline urethral obstruction. Evidence regarding the incidence of post-obstructive diuresis (POD) in cats is scarce. Establishing historical, physical examination, and initial clinicopathologic variables associated with risk for developing POD may better enable clinicians to direct treatment for this common feline emergency and to educate clients regarding financial expectations. OBJECTIVES: To report the incidence of POD in a large group of cats with urethral obstruction and determi

Pediatric urethral obstruction caused by Cowper's duct syringocele diagnosed and treated endoscopically: a case report.

Cowper's duct syringocele is an uncommon and often overlooked cause of urethral obstruction in children. We report a 7-year-old boy who had experienced severe voiding difficulty, frequency, and urgency for three months and could void only while sitting. Ultrasonography, computed tomography, magnetic resonance imaging, and voiding cystourethrography were nondiagnostic. Cystourethroscopy revealed a prominent posterior urethral crest and verumontanum, two ectopic pinpoint ostia in the bulbous urethra, and a blind membranous pouch. Incision of the bulbous ostia released abundant proteinaceous mate

Feline urethral obstruction alters the urinary microbiota and comparison to oral, preputial, and rectal microbiotas.

OBJECTIVE: To document differences in the microbiota of healthy cats versus cats with urethral obstruction (UO); compare the urinary microbiota with the oral, preputial, and rectal microbiota; and demonstrate that 16S rRNA gene sequencing will reveal rich and diverse urinary microbiota. METHODS: 15 client-owned cats with UO and 15 age-matched healthy cats were included from July 2020 through April 2021. Exclusion criteria were evidence of urinary tract infection, urolithiasis, antimicrobial administration, urinary catheterization in the past 30 days, or a comorbidity. This study was a prospec

The effect of urinary diversion on kidney function in posterior urethral valves and ureterovesical obstruction.

INTRODUCTION: Posterior urethral valves (PUV) and ureterovesical junction obstruction (UVJO) are common etiologies of lower urinary tract obstruction. The primary objective of therapy is to alleviate the obstruction in the urinary system. Temporary urinary diversion may be necessary in certain situations, such as Urinary tract infection (UTI), sepsis, and unchanging hydronephrosis. This study, aims to assess kidney parameters after urinary diversion (either pyelostomy or ureterostomy) in patients with PUV and UVJO. METHOD: We conducted a retrospective analysis of all patients diagnosed with P

Long-term outcomes of anti-incontinence surgery and subsequent transvaginal sling incision for urethral obstruction.

INTRODUCTION AND HYPOTHESIS: The suburethral sling procedure has been widely used as the first-line treatment for stress urinary incontinence (SUI) in women. Although the success rate is high, difficult urination and urine retention can occur in a small portion of patients. A transvaginal sling incision can solve this problem but recurrent SUI may occur. This study investigated the long-term outcomes of women who underwent the pubovaginal sling (PVS) procedure and subsequent transvaginal sling incision for urethral obstruction. METHODS: We retrospectively reviewed the voiding conditions of wo

Chronic partial urethral obstruction in female rats: description of an experimental model and initial results.

PURPOSE: To develop an experimental model of infravesical urinary obstruction in female rats. METHODS: After median caudal laparotomy, the urethra of 14 female rats was delicately separated from the vagina and loosely wrapped with cellophane tape measuring 0.4 x 1.0 cm. The animals were evaluated 4 (n=7) and 8 (n=7) weeks later. Five additional control animals were only subjected to separation of the urethra and vagina and monitored for 12 weeks. RESULTS: After four weeks, three rats presented vesical dilation associated with discrete ureteral ectasis in 2 animals, with the third presenting

Severe urethral obstruction diagnosed at 14 weeks' gestation: variability of outcome with and without drainage.

We present 3 case reports to illustrate the variability of outcome of severe fetal posterior urethral obstruction. Two of the described cases support the view that early in-utero decompression of an obstructed fetal urinary system into the amniotic cavity, in the selected patient, will allow adequate lung development and will prevent the development of severe renal dysplasia. It will not prevent the abdominal wall deformity of the prune belly syndrome. The evidence suggests that to allow maximum time for lung development and to prevent increasing renal dysplasia, drainage should be performed b

Characterization and treatment of acid-base and renal defects due to urethral obstruction in cats.

In 23 cats, urinary obstruction of 24 to 48 hours' duration caused marked azotemia, hyperphosphatemia, hyperkalemia, and metabolic acidosis. The metabolic acidosis was a consistent finding and was severe in all cats (venous pH, 7.11 +/- 0.09). Serum sodium and chloride were normal. Glycosuria was found in 17 (74%) of the cats. There was no clear difference in blood pH, serum chemical values, or electrolyte concentrations between cats obstructed 24 hours and those obstructed 48 hours or longer. At a mean of 8.4 hours after relief of obstruction, acid-base status was corrected to normal, using f

Cystic kidneys, renal dysplasia and microdissection data in 5 children with congenital valvular urethral obstruction.

A survey of the findings in 5 children with urethral obstruction due to valves is presented. The microdissection data found in the 7 kidneys of this series were compared. The microdissection findings in all 7 kidneys are mostly the same with a continuum of processes ranging from relatively mild to severe derangements. We could not find two varieties of renal change designated type II or IV by potter. The characterization of the lesions by nephron dissection always approached those of the type II (multicystic, dysplastic kidney). Renal dysplasia was associated with extrarenal malformations in 2

Metastatic castrate-resistant prostate cancer to the penis resulting in malignant priapism and urethral obstruction.

A 76-year-old male with metastatic castrate-resistant prostate cancer was found to have metastatic disease to the urethra and penis on PSMA PET scan. This resulted in constant pain due to obstruction of blood flow through the corpora cavernosum resulting in priapism and urethral obstruction. The patient underwent a radical penectomy, partial urethrectomy and perineal urethrostomy creation. Final pathology confirmed metastatic prostatic adenocarcinoma. Although rare, prostate cancer can metastasize to the corpora spongiosum or cavernosa. Treatment options are limited, and surgical excision with
