Avoiding Compartment Syndrome, Vascular Injury, and Neurologic Deficit in Tibial Osteotomy: An Observational Study of 108 Limbs.

INTRODUCTION: Tibial deformities are common, but substantial concern may be associated with corrective osteotomy regarding major complications reported in classic literature. Such studies chiefly focused on high tibial osteotomy, with relatively little investigation of other areas and types of deformity. The primary aim of this study was to identify the rate of compartment syndrome, vascular injury, nerve injury, and other major complications after elective tibial osteotomy. METHODS: One hundred eight tibia osteotomies performed during 2019 to 2021 were evaluated, representing all tibia osteo

A New Laparoscopic Entry Point in Patients With Previous Laparotomy: A Prospective Comparative Study.

INTRODUCTION: Intraperitoneal access and establishing pneumoperitoneum for laparoscopy is a critical step especially in patients who underwent previous laparotomy due to the higher risk of visceral or vascular injuries. In this study, we propose a new entry point for safe laparoscopic access in cases having previous laparotomy. MATERIALS AND METHODS: This is a prospective controlled randomized trial conducted between January 2016 and January 2022 in Ain Shams University Hospitals. It included 232 patients who underwent laparoscopic procedures after previous laparotomy. They were randomly divi

Vascular Injury After Knee Dislocation: A Meta-Analysis Update.

INTRODUCTION: Multisystem injuries affecting vasculature and nerves can be detrimental sequelae of knee dislocations (KDs). The purpose of this study was to provide an update of the current literature since the conducted search used in the publication of Medina et al on May 3, 2013, to evaluate (1) the frequency of vascular and neurologic injury after KD, (2) the frequency with which surgical intervention was conducted for vascular injury in this setting, and (3) the frequency with which each imaging modality was used to detect vascular injury. METHODS: A two-author screening process was cond

Blunt traumatic vertebral artery injury: a clinical review.

Blunt traumatic vertebral injury (TVAI) is frequently associated with head and neck injury and is being detected with increasing frequency due to improved imaging of the trauma patient. In a few cases, it can lead to potentially fatal posterior circulation ischaemia There is debate in the literature regarding whether TVAI should be actively screened for and, if so, how. Management of TVAI may be conservative, medical (antiplatelet agents or anticoagulation), endovascular or open surgery. We review the literature concerning the mechanisms and presentation of TVAI following blunt injury and the

Evaluation of Associated Genes with Traumatic Pain: A Systematic Review.

OBJECTIVES: The knowledge about the molecular pathway of traumatic pain relief is less documented. This systematic review study aimed to identify the genes and molecular pathways associated with various traumatic pains. METHODS: The online databases such as EMBASE, MEDLINE, PubMed, Cochrane Library, International Clinical Trials Registry Platform, Clinical Trials, Google Scholar, Wiley, ISI Web of Knowledge, and Scopus were searched. Two review authors searched and screened all records' titles and abstracts, and the third expert reviewer author resolved their disagreement. The study's design,

Unilateral cervical facet fractures with subluxation: injury patterns and treatment.

STUDY DESIGN: This is a retrospective study of patients with unilateral cervical facet fractures from a Level I academic trauma center. OBJECTIVE: We sought to examine fracture patterns involving only the facets, to examine the incidence of associated neurologic and vascular injuries, and to determine optimum management strategies for these injuries. SUMMARY OF BACKGROUND DATA: Most of the literature regarding unilateral cervical facet injuries has resulted from studies evaluating dislocated locked facets, "fracture-dislocations," or fractures of the lateral mass and pedicle. METHODS: We re

Surgical management for the first 48 h following blunt chest trauma: state of the art (excluding vascular injuries).

This review aims to answer the most common questions in routine surgical practice during the first 48 h of blunt chest trauma (BCT) management. Two authors identified relevant manuscripts published since January 1994 to January 2014. Using preferred reporting items for systematic reviews and meta-analyses statement, they focused on the surgical management of BCT, excluded both child and vascular injuries and selected 80 studies. Tension pneumothorax should be promptly diagnosed and treated by needle decompression closely followed with chest tube insertion (Grade D). All traumatic pneumothorace

Autonomic-vascular dysregulation in CKD-associated hypertension: a narrative review with evidence hierarchy.

Hypertension and chronic kidney disease frequently coexist and mutually accelerate cardiovascular and renal injury. This narrative review prioritizes direct human autonomic phenotyping (Level 1: microneurography, HRV/BRS), human vascular correlates (Level 2: PWV, FMD), and complementary preclinical evidence (Level 3) to elucidate autonomic-vascular mechanisms. Autonomic imbalance, characterized by sympathetic overactivity and reduced parasympathetic restraint, represents a key interface between neural control and vascular pathology in this setting. This narrative review synthesizes experimenta

Extremity vascular trauma in civilian population: a seven-year review from North India.

This is a retrospective study of the experience with extremity vascular trauma at a tertiary level referral centre in North India where the majority of the population lives in villages and the incidence of high-speed automobile accidents and civilian violence is low. The aim was to study the aetiology, pattern of injuries and the mortality and morbidity rates due to vascular trauma in our population. Data relating to 148 patients presenting with vascular trauma requiring surgical intervention other than amputation between January 1996 and December 2002 were collected retrospectively. There wer

Orthoplastic classification systems: the good, the bad, and the ungainly.

Over the last few decades, there have been many important advances in the treatment of severe lower limb injuries. This article looks at a few of the more widely used classification systems and Injury Severity Scores to examine their utility in a practical setting. Gustilo and Anderson formulated their landmark classification system in 1976 (J Bone Joint Surg Am. 1976;58:453-458). For the Gustilo classification system to serve any useful purpose, it is necessary to include supplemental information, whenever discussing these injuries, that includes the mechanism and energy of the injury and the

"External Iliac Artery Injury After Total Hip Arthroplasty in an Infected Intrapelvic Hemiarthroplasty": A Case Report.

CASE: We report a patient with intrapelvic prosthetic protrusion associated with infection after hemiarthroplasty for a fracture neck of the femur. We discuss the challenges involved in the management of this complex case with recommendations to manage a vascular injury. CONCLUSION: In patients with an intrapelvic prosthesis, thorough preoperative vascular evaluation and vigilant postoperative check are required to prevent a vascular injury. In a suspected vessel injury after primary or revision total hip arthroplasty, it is imperative to get proximal control of the vessel before re-explorati

Despite Major Therapeutic Advances, Vena Caval Trauma Remains Associated with Significant Morbidity and Mortality.

BACKGROUND: Penetrating inferior vena caval injuries remain a challenging operative entity. This study reviews our local experience with the injury over a nine-year period and attempts to contextualize it within the published literature that emanates from South Africa on the topic. METHODS: A single-centre retrospective review of prospectively collected data was performed of all patients who underwent a laparotomy for a penetrating IVC injury. Descriptive statistics were calculated for demographics, clinical and biochemical parameters, intraoperative data, ICU admission and outcomes. RESULTS

Visualization of vascular injuries in extremity trauma.

A tandem of particle-based computational methods is adapted to simulate injury and hemorrhage in the human body. In order to ensure anatomical fidelity, a three-dimensional model of a targeted portion of the human body is reconstructed from a dense sequence of CT scans of an anonymized patient. Skin, bone and muscular tissue are distinguished in the imaging data and assigned with their respective material properties. An injury geometry is then generated by simulating the mechanics of a ballistic projectile passing through the anatomical model with the material point method. From the injured va

[Endovascular treatment of posttraumatic arteriovenous fistulas of the subclavian artery].

The authors describe the use of different methods of endovascular treatment of patients with presenting with posttraumatic arteriovenous fistulas in the first segment of the subclavian artery exemplified by two clinical cases. The first case report concerns a patient having developed was an arteriovenous fistula after catheterization of the jugular vein. The arteriovenous aneurysm was occluded by means of embolizing spirals. In the second case report is patient having developed an arteriovenous fistula after a stab-and-slash wound in the area of the sternoclavicular junction. Long-term existen

Global Disparity Within the Management of Traumatic Abdominal Vascular Injuries: Analysis of a Global Dataset.

BACKGROUND: Vascular injuries are common to trauma and carry a high risk to limb and life. Integrating specialist vascular care into existing trauma systems is essential; however, global data on this topic remains limited. This study describes global patterns of abdominal vascular trauma, through secondary analysis of the Global Outcomes After Laparotomy for Trauma (GOAL-Trauma) study dataset, examining system- and patient-level differences across resource settings. METHODS: Patients of any age with blunt or penetrating trauma who underwent an abdominal vascular procedure during trauma laparo
