TrapEase vena cava filter: experience in 751 patients.

PURPOSE: To evaluate the clinical safety and efficacy of the TrapEase vena cava filter in a 4-year single-center experience. METHODS: The clinical and imaging data of 751 patients (384 men; mean age 64 years, range 16-99) who had a TrapEase inferior vena cava (IVC) filter placed between January 1, 2001, and December 31, 2004, were reviewed retrospectively. More than a third of patients (297, 39.5%) presented with pulmonary embolism (PE), 188 (25.0%) had deep vein thrombosis (DVT), 40 (5.3%) had both PE and DVT, and the rest (226, 30.1%) had other symptoms. Indications for filter placement wer

Temporary vena cava filter placement for pulmonary embolism.

Current clinical trials are under way to determine the safety and efficacy of temporary inferior vena cava filters for use in patients who need but have contraindications to anticoagulation medications for a short time (<10 days). To date, no data on these types of filters have been published. The authors describe a 20-year-old male trauma patient in whom a pulmonary embolism developed early in his hospital course and who was appropriately placed on anticoagulation therapy. Surgical intervention, however, was necessary to repair complex facial fractures sustained in a motorcycle collision. A f

Pulmonary embolus after vena cava filter placement.

This report characterizes risk factors in patients who suffer pulmonary embolus (PE) after insertion of vena caval filter (VCF) and formulates an organized diagnostic and therapeutic plan of management. Three hundred eighteen patients were included in a review of patients undergoing insertion of VCF from 1989 to 1995. Ten patients (six men and four women, ages 25-72 years) from this group (3.1%) experienced PE after VCF insertion. Risk factors for deep venous thrombosis were documented in these ten patients. Venacavography was performed after diagnosis of PE. Thrombus length measured from the

Extended indications for placement of an inferior vena cava filter.

To study the morbidity and mortality rates after placement of an inferior vena cava filter and to define the appropriate indications for interruption of the inferior vena cava, the records of all patients who underwent insertion of a Greenfield filter during the decade January 1978 to December 1987 were reviewed. Patients were designated as having either a traditional or extended indication for placement of an inferior vena cava filter. Two hundred sixty inferior vena cava filters were placed in 264 attempts, with no deaths related to insertion of the filter. An extended indication was the pri

New criteria for placement of a prophylactic vena cava filter.

In order to identify patients who, having had an initial pulmonary embolism, are likely to develop recurrent emboli despite adequate anticoagulation therapy, ten patients (group 1) with documented recurrent pulmonary embolism during anticoagulation therapy were compared with 31 patients (group 2) who showed no evidence of recurrent emboli during the treatment period. Risk factors for thromboembolic disease were similar between the two groups. Noninvasive venous studies of the lower extremities, including Doppler venous ultrasound and phleborheography (PRG), were performed upon all patients aft

Radiologic follow-up of vena cava filter devices.

A total of 66 vena cava filters was implanted to prevent pulmonary emboli: 33 Mobin-Uddin umbrellas and 33 Kim-Ray-Greenfield devices. Radiologic examinations were available in 59 patients, 37 having one or more follow-up studies. Most of the Mobin-Uddin umbrellas became thrombosed. Although most Kim-Ray-Greenfield devices remained patent, some migrated, and several penetrated the cava wall; this latter finding can be diagnosed on plain films. Neither of these devices is fully safe nor effective in preventing pulmonary emboli; further improvements are needed.

Urological complications caused by inferior vena cava filters: a systematic review.

INTRODUCTION: Inferior vena cava (IVC) filters act in preventing pulmonary embolisms (PE). Various complications have been reported with their use. However, a credible urological complication rate, filter characteristics, and clinical presentation has yet to be summarized. Thus, we reported these complications in the form of a systematic review. EVIDENCE ACQUISITION: A search strategy was designed using PubMed, MEDLINE, and EMBASE on February 10th, 2022. The design of this search strategy did not include any language restrictions. CT of inferior vena cava filters: normal presentations and potential complications.

With massive pulmonary embolism (PE) being the first or second leading cause of unexpected death in adults, protection against PE is critical in appropriately selected patients. The use of inferior vena cava (IVC) filters has increased over the years, paralleling the increased detection of deep venous thrombosis (DVT) and PE by improved and more available imaging techniques. The use of IVC filters has become very common as an alternative and/or as a supplement to anticoagulation, and these filters are often seen on routine abdominal CT, including in the emergency setting; therefore, knowledge

Are retrievable vena cava filters placed in trauma patients really retrievable?

PURPOSE: Concerns have arisen regarding the use of retrievable inferior vena cava filters (rIVCFs) in trauma patients due to increasing reports of low retrieval rates. We hypothesized that complete follow-up with a dedicated trauma nurse practitioner would be associated with a higher rate of retrievability. This study was undertaken to determine the rate of retrievability of rIVCFs placed in a Canadian Lead Trauma Centre, and to compare the rate of retrievability in our trauma population to our non-trauma patients. METHODS: We performed a retrospective cohort study of all patients with rIVCF

Open surgical removal of retained and dislodged inferior vena cava filters.

Retained inferior vena cava (IVC) filters can lead to significant complications. Six patients underwent open surgical removal of complicated IVC filters that could not be removed endovascularly. Struts of the filter perforated the IVC wall in all patients and the adjacent viscera in five. Caval clamping and longitudinal cavotomy with direct closure were used in two patients to remove permanent filters. IVC clamping was not needed in three patients, in whom the filter was first collapsed and then removed through a stab venotomy in the IVC (n = 2) or lumbar vein (n = 1). A broken strut that infe

Inferior vena cava filters for primary prophylaxis: when are they indicated?

Hidradenitis suppurativa is a chronic inflammatory disorder characterized by occlusion of the follicular pilosebaceous units of the skin. The treatment options are sometimes very limited and unpleasant odor and abundant drainage complicate the disease. Ankylosing spondylitis is a form of seronegative spondyloarthritis with predominantly axial but also peripheral joint involvement. Both of the conditions lower the patient's quality of life and affect everyday activities. We describe a 39-year-old male patient with both diseases treated with different medications with only a modest result. After

Increasing use of vena cava filters for prevention of pulmonary embolism.

PURPOSE: To test whether the use of vena cava filters continues to increase in the era of retrievable filters, suggesting that indications for insertion are broadening. METHODS: Data from 1979 through 2006 are from the National Hospital Discharge Survey. RESULTS: From 1979 through 1984, 17,000 vena cava filters were inserted; 8000 in patients with pulmonary embolism, 4000 in patients with deep venous thrombosis only, and 5000 in patients at risk of pulmonary embolism who had neither. From 1985 through 2006, 803,000 vena cava filters were inserted: 285,000 in patients with pulmonary embolism,

Inferior vena cava filters in children: our experience and suggested guidelines.

Although use of inferior vena cava (IVC) filters for prophylaxis against pulmonary embolism (PE) is well reported in adults, long-term studies in children are lacking. We performed retrospective review of imaging and clinical database of IVC filters for the last 12 years. Thirty-five patients (mean age: 15.5 y) underwent filter placement and/or retrieval. Indications for placement were contraindication to anticoagulation with known deep venous thrombosis (DVT) (18) or high risk of venous thromboembolism (5), recurrent DVT despite anticoagulation (1), and prophylaxis before endovascular thrombo

Improving the tracking and removal of retrievable inferior vena cava filters.

Therapeutic and prophylactic inferior vena cava (IVC) filters should be placed based on currently accepted indications to prevent a fatal pulmonary embolism (PE). The protective effect of filters is offset by the potential for lower extremity deep venous thrombosis (DVT), caval thrombosis, and possible otherwise unnecessary life-long anticoagulation (AC). The duration of treatment for most DVTs or PEs is 3 to 6 months of AC/filter. Filters should be retrieved when duration of treatment for a DVT/PE has been met, the risk of a PE is no longer high, and/or there is no longer a contraindication t

[Vena cava filters and treatment of venous thromboembolism in cancer patients].

Despite numerous publications, there is still only one randomised clinical trial with vena cava filter in the treatment of venous thromboembolism (VTE). This study has shown a potential and early benefit on the risk of pulmonary embolism (PE) (the first three months) but a late negative effect on the risk of deep vein thrombosis (DVT) recurrences (beyond the sixth month) especially on the risk of filter thrombosis. Consequently, the international recommendations are against a systematic use of vena cava filter to treat VTE (grade 1A) and they suggest to use them in case of a recurrence despite
