Varicose veins (VVs) are a common venous disease of the lower extremity characterized by incompetent valves, venous reflux, and dilated and tortuous veins. If untreated, VVs could lead to venous thrombosis, thrombophlebitis and chronic venous leg ulcers. Various genetic, hormonal and environmental factors may lead to structural changes in the vein valves and make them incompetent, leading to venous reflux, increased venous pressure and vein wall dilation. Prolonged increases in venous pressure and vein wall tension are thought to increase the expression/activity of matrix metalloproteinases (M

There are numerous anatomic connections between the allergic conjunctivitis and allergic rhinitis. The most obvious reason is the physical connection via the nasolacrimal apparatus. However, a closer look at innervation, circulatory, lymphatic, and neurogenic systems reveals much more than a physical connection. The eye is richly innervated by parasympathetic nerves that enter the eyes after traveling in conjunction with the parasympathetic input to the nasal cavity. Parasympathetic innervation governing the tear film and nasal secretion can intersect at the pterygopalatine ganglion. Neurogeni

The sequelae of deep vein thrombophlebitis, such as postphlebitic syndrome with or without ulcer, can be treated by direct surgery on the valve. The present surgical treatment of stasis ulcer includes removal of the incompetent communicating veins with excision of the ulcer and skin graft. If this operation is performed on patients who actually have deep venous incompetence, then a high incidence of reulceration and skin graft slough may be expected if there is no concommitant correction of deep venous incompetence. Experimental studies to restore venous valve function such as autogenous vein

The chronic venous disease covers a wide spectrum of venous disorders that are characterized by severely impaired blood return that primarily affects veins in the lower extremities. Morphological and functional abnormalities of the venous system led to chronic venous insufficiency (CVI), and present as leg heaviness/achiness, edema, telangiectasia, and varices. The term 'chronic venous insufficiency' (CVI) refers to a disease of greater severity. Venous dysfunction is associated with venous hypertension and is associated with venous reflux due to poorly functioning or incompetent venous valves

The anatomic appearance and competency of internal jugular valves were investigated in both cadavers and surgical patients. In 20 cadavers, the internal jugular vein (IJV) was dissected and the appearance of the valves observed. In two adult patients, the movements of the valves were observed by endoscopic visualization using an intravascular fiberscope, ultrasound techniques, and invasive venography. Transvalvular pressure gradients were measured in 10 adult surgical patients to examine the competence of the jugular venous valve. Nineteen internal jugular valves obtained from the 20 cadavers

From 1982 through 1990, 13 patients with double inlet ventricle and common atrio-ventricular valve underwent definitive cardiac surgery. Nine patients simultaneously had anomalous systemic venous connection and 4 had anomalous pulmonary venous connection. One patient with double inlet left ventricle underwent a successful partition of ventricle and atrio-ventricular valve. All other 12 patients underwent a Fontan operation, which utilized different techniques to deal with various forms of anomalous systemic and pulmonary venous connection. Four patients with common atrioventricular valve regur

We tested the method of estimating capillary pressure from venous pressure transients obtained after sudden venous clamping in a hydrodynamic model. The basic principles were confirmed in the model, but it was found that when occlusion was caused over a relatively wide distance or in a predistended vessel, capillary pressure was overrated. This problem was due to volume backflow from the occlusion site, since it could be eliminated by placing a one-way valve upstream from the occlusion site. Upstream from the valve, the venous pressure transient accurately followed capillary pressure. Downstre

The microvasculature of the esophagus was studied by scanning electron microscopy of vascular corrosion casts in human infants and rabbits. In both species, segmental circumferential arteries arise from main longitudinal arteries, the latter giving off numerous perforating arteries. The tunica muscularis is supplied by branches of circumferential and perforating arteries, the submucosa and its glands by branches of perforatings. Terminal arborizations of perforating arteries feed a subepithelial capillary network. These capillaries are drained by a venous plexus in the lamina propria which is

IMPORTANCE: In patients with mechanical valves in the aortic and mitral positions, percutaneous access to the left ventricle (LV) via a transfemoral approach for catheter ablation of ventricular tachycardia (VT) has been considered infeasible.
OBJECTIVE: To describe the outcomes of a novel percutaneous trans-right atrial (RA) access to the LV via a femoral venous approach for catheter ablation of VT in patients with mechanical aortic and mitral valves.

Sequelae of the postphlebitic syndrome can now be treated by direct valve surgery. However, present surgical treatment of stasis ulcer, including removal of the incompetent communicating veins, ulcer excision, and skin grafting, remains essential to patient care. When done alone, perforator interruption and ulcer care are effective but allow a high rate of ulcer recurrence. Experimental studies to restore venous valve function have included autogenous vein valve transplantation, and synthetic valve procedures also have been tried. Clearly, the patency rate of an autogenous vein graft is better

Oedema is a complex condition, and understanding its underlying causes is essential for safe and effective management. The most common aetiology is poor venous return, and venous disease develops when the veins are unable to withstand the hydrostatic pressure within them, leading to a cascade of tissue changes including lipodermatosclerosis, haemosiderin deposition and venous leg ulceration. The most disabling consequence is often secondary lymphoedema. Under normal circumstances, most interstitial fluid that is not reabsorbed at the venous end of the capillaries enters the lymphatic system an

The venous drainage of the peripheral nerves was studied in the upper and lower limbs of two human fresh cadaver subjects after total body perfusion with a radio-opaque lead oxide mixture. Four patterns of extraneural drainage were identified in which the venae nervosa drained: directly to the venae comitantes of the neurovascular bundle; indirectly via nearby veins, derived usually from muscles; to the periarterial venous plexus; or, in the case of the cutaneous nerves, to the perivenous plexus. The various patterns of the drainage along the length of the radial, median, ulnar, sciatic, anter

PURPOSE: Nonoperative therapeutic approaches to chronic venous ulceration, although effective, often require prolonged dressing care and immobilization with leg elevation. Results of skin grafting, perforator ligation, and valve interpositions and reconstructions improve results of ulcer healing but have not uniformly prevented ulcer recurrence. Our hypothesis is that reconstruction of chronic venous ulcers by excision of the diseased tissue bed and replacement with a free flap containing multiple competent microvenous valves and a normal tissue microcirculation will result in long-term cure o

Chronic venous insufficiency is a pathologic condition of the skin and subcutaneous tissues in the lower extremity caused by stasis of the blood flow. Incompetency or failure of the venous valves results in reflux and ambulatory venous hypertension, which is more severe with deep than with superficial venous incompetency. Superficial chronic venous insufficiency (varicose veins) is effectively managed with ligation and stripping of incompetent perforator and superficial veins to restore normal venous physiology. Deep chronic venous insufficiency (postphlebitic leg) presents a widespread pathol

BACKGROUND: The ophthalmic and facial veins are frequently stated to be devoid of valves, facilitating the spread of infection from the mid-face to the cavernous sinus.
METHODS: Twelve superior and eight inferior ophthalmic veins together with 13 angular and facial veins were harvested from adult cadavers. Each vein was opened longitudinally and examined by stereomicroscopy; the number, location and geometry of valve cusps were recorded.
RESULTS: Ten valves were identified in nine (75%) superior ophthalmic vein specimens: four valves were in the superior ophthalmic vein and the remainder wer

The basis of in-office evaluation of leg vascular disease involves Doppler ultrasound to estimate flow in arteries and veins. Arterial evaluation localizes sites of obstruction by segmental pressures obtained by Doppler signal assessment using pressure cuffs. Ankle compared with brachial arterial pressure is the most rapid screening test. Plethysmography permits more automated and sensitive analysis of pressure waveforms. Venous evaluation identifies sources of increased pressure transmitted by incompetent valves to surface veins causing varicosities and telangiectases. Doppler ultrasound perm

The primitive right sinuatrial valve persists in humans as the crista terminalis, the valve of the inferior vena cava and the valve of the coronary sinus, while according to the known data the primitive left sinuatrial valve is supposed to have no derivatives. Ten human right atria were opened with intercaval incisions and the precaval segment of each crista terminalis was studied macroscopically. Three specimens did not present any peculiarities at this level, but the other 7 had sagittal muscle bundles and supernumerary valves in individual arrangements. Supernumerary valves were present in

An infant with truncus arteriosus, complete atrioventricular canal, and total anomalous pulmonary venous connection successfully underwent one-stage complete repair. Residual mitral valve regurgitation required reoperation after 12 days. The patient is doing well at 6 months' follow-up. Echocardiography demonstrates no residual defects, competent atrioventricular valves, and normal pulmonary pressure. This case illustrates the potential for successful one-stage repair even of associated complex heart defects involving venous, intracardiac, and arterial pathways.

Any obstacle slowing down or blocking the venous return, causes hypertension, then dilatation which destroys the valves of the perforating or muscular veins. The superficial veins become flooded. A symptomatic treatment with sclerosis or stripping is not sufficient. The valve is of crucial importance and varices represent a symptom indicating a snag in the venous return. The phlebologist's role is to look for and identify the snag as well as finding a treatment.

BACKGROUND: Early anatomists suspected that the radiculomedullary veins draining the spinal cord had valves preventing their retrograde filling with anatomical casting material. Modern investigations have discarded the presence of true valves and introduced instead the notion of a pseudo-valvular configuration for which the term antireflux mechanism was coined in the 1970s. The angiographic anatomy of the antireflux mechanism has not been well documented so far.
METHODS: This article discusses anatomical and clinical features of the antireflux mechanism with a series of 12 angiographic observ
