Evaluation of a new hyperbaric oxygen ventilator during pressure-controlled ventilation.

INTRODUCTION: The stability of a new hyperbaric ventilator (Shangrila590, Beijing Aeonmed Company, Beijing, China) at different clinically relevant pressures in a hyperbaric chamber during pressure-controlled ventilation (PCV) was investigated. METHODS: The ventilator was connected to a test lung in the multiplace hyperbaric chamber. The inspiratory pressure (PI) of the ventilator was set to 1.0, 1.5, 2.0, 2.5 and 3.0 kPa (approximately 10, 15, 20, 25 and 30 cmH₂O). The compliance and resistance of the test lung were set to 200 mL·kPa⁻¹ and 2 kPa·L⁻¹·s⁻¹, respectively. Experiments were conduc

Determinants of Mortality for Ventilated Hospital-Acquired Pneumonia and Ventilator-Associated Pneumonia.

UNLABELLED: Hospital-acquired pneumonia (HAP) is the most common hospital-acquired infection, accounting for 22% of all nosocomial infections. The available studies to date have not attempted to assess whether confounding factors may account for the observed difference in mortality for the two forms of nosocomial pneumonia associated with mechanical ventilation, namely ventilated HAP (vHAP) and ventilator-associated pneumonia (VAP). OBJECTIVES: To determine if vHAP is an independent predictor of mortality among patients with nosocomial pneumonia. DESIGN SETTING AND PARTICIPANTS: Single-cente

History of Mechanical Ventilation. From Vesalius to Ventilator-induced Lung Injury.

Mechanical ventilation is a life-saving therapy that catalyzed the development of modern intensive care units. The origins of modern mechanical ventilation can be traced back about five centuries to the seminal work of Andreas Vesalius. This article is a short history of mechanical ventilation, tracing its origins over the centuries to the present day. One of the great advances in ventilatory support over the past few decades has been the development of lung-protective ventilatory strategies, based on our understanding of the iatrogenic consequences of mechanical ventilation such as ventilator

Analysis of home support and ventilator malfunction in 1,211 ventilator-dependent patients.

Risk management is an important aspect of home ventilation (HV). We examined the nature of calls to a home support helpline to identify patient/equipment problems and strategies to minimise risk for patients, healthcare teams and manufacturers. From 1,211 adult and paediatric patients with neuromuscular disease, chronic obstructive pulmonary disease or chest wall disease receiving HV, all calls to a dedicated respiratory support telephone hotline between January 1, 2006 and June 30, 2006 were analysed. 1,199 patients received noninvasive ventilation, 12 tracheostomy ventilation; 149 had two ve

Bi-level home ventilators for non invasive positive pressure ventilation.

With the widespread use of non-invasive positive pressure mechanical ventilation, great efforts have been made to produce machines, the bi-level home ventilators, which are less sophisticated, cheaper and able to better compensate air leaks with respect to the domiciliary volume-target machines and the traditional ventilators used in intensive care unit. As consequence of quick technologic evolution, bi-level home ventilators may be nowadays successfully applied for both the non-invasive ventilatory domiciliary treatment of chronic respiratory failure and the management of acute respiratory fa

Ventilator-induced lung injury and recommendations for mechanical ventilation of patients with ARDS.

Mechanical ventilation is life sustaining and is the standard therapy for acute respiratory failure. The 16th century anatomist Vesalius is often credited for the earliest account of positive-pressure ventilation. In his work De humani corporis fabrica (On the Fabric of the Human Body), he described how an animal could be resuscitated by blowing into a reed inserted into a hole in its trachea. Although positive pressure ventilation using bellows was first used for drowning victims in the 1700s, there were soon concerns that such therapy could in fact be harmful to the lungs. In 1827, Leroy d'E

Respiratory syncytial virus pneumonia ventilated with high-frequency oscillatory ventilation.

Four infants below 6 months of age with proven respiratory syncytial virus infection in need of assisted mechanical ventilation were successfully treated by high-frequency oscillatory ventilation. One of the four infants fulfilled the criteria for extracorporeal membrane oxygenation before the start of oscillation, and one on the second day on high-frequency oscillatory ventilation. However, extracorporeal membrane oxygenation was not needed in any of the infants. All survived, and three appeared to be without any pulmonary sequelae.

Effects of Lung-Protective Ventilation on Cerebral Oxygenation During One-Lung Ventilation in Neonates.

OBJECTIVE: This study aimed to investigate the effects of different lung ventilation strategies on cerebral oxygen saturation (rSO2) during one-lung ventilation (OLV) in neonates. METHODS: A total of 50 neonates scheduled for elective OLV between October 2022 and April 2024 were enrolled and randomly assigned to either the control group (Group C) or the study group (Group S). In Group S, the respiratory rate was adjusted to maintain end-tidal carbon dioxide (PETCO2) levels between 45-55 mmHg at 30 minutes of OLV (T3) and subsequently restored to normal levels (35-45 mmHg) at 45 minutes of OLV

Semiautonomous ventilation in a porcine hemorrhage and lung injury model provides lung protective ventilation.

BACKGROUND: Mechanical ventilation requires frequent reassessment from providers to ensure delivery of lung protective ventilation. However, in resource-limited settings, the time and attention lung protective ventilation requires are not always feasible. This study aimed to compare a physiologic closed-loop control (PCLC) ventilator capable of self-adjusting based on patient parameters against standard of care (SOC) ventilatory management in a porcine model. METHODS: The study compared SOC (n = 15) with PCLC (n = 15) for three porcine injury models: hemorrhage, lung injury, and hemorrhage wi

High-frequency oscillatory ventilation versus conventional mechanical ventilation for very-low-birth-weight infants.

BACKGROUND: The efficacy and safety of early high-frequency oscillatory ventilation as compared with conventional synchronized intermittent mandatory ventilation for the treatment of infants with very low birth weight have not been established. METHODS: We conducted a randomized, multicenter clinical trial to determine whether infants treated with early high-frequency oscillatory ventilation were more likely than infants treated with synchronized intermittent mandatory ventilation to be alive without requiring supplemental oxygen at 36 weeks of postmenstrual age. Eligible infants weighed 601

["Fuldaer ventilation surgery"--a surgical concept in severe ventilation disorders of the middle ear].

BACKGROUND: This report presents the long-term results of a special surgical technique in cases of persistent severe dysfunction of the Eustachian tube. PATIENTS AND METHODS: We performed this operation on 16 patients (18 ears) between 1982 and 1997. Almost all patients had undergone previous surgery. This included myringotomies and tube insertions, adenoidectomies, tonsillectomies, myringoplasties and mastoidectomies, sinus surgeries, and an operation on a cleft palate. The concept developed by one coauthor (Draf) combines different methods to improve ventilation of the middle ear and protec

The use of an endotracheal ventilation catheter for jet ventilation during a difficult intubation.

This case report describes the use of an endotracheal ventilation catheter (ETVC) to provide prolonged intraoperative jet ventilation, reintubation and the maintenance of tracheal access following extubation. It emphasizes that excellent oxygenation and ventilation can be achieved but such management can be complicated by a pneumothorax even when the risks are minimized. A 43-yr-old man presented for possible pulmonary sleeve resection. Placement of a double lumen endotracheal tube (DLT) by direct laryngoscopy was unsuccessful due to the inability to visualize the glottis. A 7.5 mm endotrachea

Peritoneal ventilation: an animal model of extrapulmonary ventilation in experimental adult respiratory distress syndrome.

Adult respiratory distress syndrome (ARDS) is a critical medical problem in which severe arterial hypoxemia is often poorly responsive to conventional modes of mechanical ventilation. We studied the efficiency of mechanical ventilation of the peritoneal cavity in rabbits with experimental ARDS caused by lung lavage. The study shows that peritoneal ventilation is significantly effective in oxygenation of hypoxemic animals with ARDS and is also effective for carbon dioxide elimination. Peritoneal ventilation may be considered as an investigational method for extrapulmonary oxygenation in severe
