The current and continuing important role of ventilation-perfusion scintigraphy in evaluating patients with suspected pulmonary embolism.

After the publication of the Prospective Investigation of Pulmonary Embolism Diagnosis (PIOPED) study in 1990, there was considerable controversy concerning the ventilation-perfusion (V/Q) study in regard to its low sensitivity and high number of nondiagnostic examinations when used in patients with suspected pulmonary embolism (PE). Many lessons have been learned from the PIOPED database that have greatly improved our interpretive skills in the 2 decades since the study was performed. One of the key problems negatively impacting interpretation was the predominantly inpatient population that w

Abnormal distribution of pulmonary blood flow in aortic valve disease. Relation between pulmonary function and chest radiograph.

Wasted ventilatory volume (V(D)) and its ratio to tidal volume (V(D)/V(T)) were measured at rest and during exertion in 17 patients with aortic valve disease. We considered V(D)/V(T) to indicate abnormal ventilation: perfusion relations if it did not decrease on exertion, or if the exercising value was greater than 40 per cent. Plain chest radiographs were independently examined for evidence of diversion of pulmonary blood to the upper lobes. There was significant agreement (p<0.05) between radiographic and pulmonary function estimations of abnormality. This suggests that the raised pulmonary

Evaluation of pulmonary perfusion and ventilation in suspected chronic thromboembolic pulmonary hypertension via phase-resolved functional lung magnetic resonance imaging: correlations with hemodynamics and cardiopulmonary function.

BACKGROUND: Phase-resolved functional lung (PREFUL) magnetic resonance imaging (MRI) is a reliable, noninvasive method for assessing pulmonary ventilation and perfusion. However, the differences in subgroups of patients with chronic pulmonary embolism (CPE) have not been thoroughly investigated, and there may be significant clinical value in examining noninvasive methods for evaluating hemodynamics and right heart function in patients with CPE. This study aimed to evaluate pulmonary perfusion and ventilation in patients with suspected chronic thromboembolic pulmonary hypertension (CTEPH) via P

[Intensive care in lung and heart-lung transplantation].

Intensive care after lung, and heart-lung transplantation may have simple post operative course specially after preventive procedures of reperfusion injury, nosocomial infections during mechanical ventilation and immunosuppression risks. Nevertheless a severe mediastinal shift may occurred after single lung transplantation in emphysema. Rapid changes in ventilation/perfusion ratio during lung infection or rejection specially in pulmonary hypertension are responsible of dramatic respiratory failure. Knowledge of multiorgan dysfunction and multidisciplinary experience encourage to future develop

Diffusion-weighted 19F-MRI of lung periphery: Influence of pressure and air-SF6 composition on apparent diffusion coefficients.

Lung functional magnetic resonance imaging (MRI) has become a reality using different inert hyperpolarized gases, such as 3He and 129Xe, which have provided an extraordinary boost in lung imaging and has also attracted interest to other chemically inert gaseous contrast agents. In this context, we have recently demonstrated the first diffusion-weighted images using thermally polarized inhaled sulfur hexafluoride (SF6) in small animals. The aim of this study was to evaluate whether or not the diffusion coefficient of this fluorinated gas is sensitive to pulmonary structure, gas concentration an

[Intrapulmonary blood shunting during surgical interventions in children].

It has been observed that controlled lung ventilation during surgery in children is accompanied by an increase in intrapulmonary blood shunting, which averages 7.3% in urological surgery, 12% in abdominal surgery and 16.8% in thoracic surgery. The shunting is caused by ventilation-perfusion disturbances. The use of high-frequency jet ventilation, i.e. application of 16.7 Hertz oscillatory modulations together with conventional controlled lung ventilation enables one to decrease intrapulmonary shunting, reduce the signs of circulatory hyperdynamia and the load on the heart ventricles and leads

Chronic thromboembolic pulmonary hypertension: take care to a "favourable" apparently evolution. A case report.

Chronic thromboembolic pulmonary hypertension (CTEPH) caused by intraluminal thrombus organization and fibrous stenosis or complete obliteration of pulmonary arteries, is a not rare but life-threatening complication of acute pulmonary embolism. The prognosis of medically treated patients with CTEPH is poor and worsens as pulmonary hypertension exacerbates. We describe the case of a 43-years old with a history of progressive shortness of breath, hemoptysis, chest discomfort and syncope. Echocardiographic and imaging studies showed changes consistent with chronic thromboembolic pulmonary hyperte

Mechanisms of exercise intolerance in cardiac failure: abnormalities of skeletal muscle and pulmonary function.

The syndrome of chronic heart failure is characterized by exercise intolerance. Exercise is limited by shortness of breath and fatigue, and either symptom occurs in the same patient depending on the type of exercise performed. Exercise capacity correlates poorly with indices of central hemodynamic function, but the increased ventilatory response in chronic heart failure correlates well with exercise capacity. Possible pulmonary causes have been explored, including increased dead space ventilation, abnormal airway function, and abnormal diffusion capacity. However, the finding of hypocapnia and

Lung volume and distensibility in insulin-dependent diabetes mellitus.

The effect of insulin-dependent diabetes mellitus on pulmonary function was studied in 22 diabetic nonsmokers (11 men) with a mean age of 40 yr (SE, 4 yr) and no history of cardiorespiratory disease. Exponential analysis of static pressure-volume data obtained during deflation of the lungs gave the exponent K, an index of pulmonary distensibility. In the diabetic subjects, mean values for total lung capacity, functional residual capacity, VC, and FEV1 were similar to predicted values. Diffusing capacity, K, and in K did not differ significantly from corresponding values in healthy subjects. Th

Computer-based evaluation of cardiopulmonary function for the optimization of ventilatory therapy in the adult respiratory distress syndrome.

A computer based evaluation system has been developed for the assessment of respiratory pressure flow dynamics, pulmonary gas exchange and ventilation perfusion interrelationships. This system is based on the acquisition of primary data on-line from intubated and ventilated patients consisting of airway pressures, ventilatory flows and mass spectrometric quantification of inspired and expired gas concentrations. The system has been applied to the study of patients with the adult respiratory distress syndrome (ARDS) following trauma and/or sepsis. The programs developed for evaluation of these

The transcutaneous PO2 measurement as an additional evaluation index to the physiological and pathological conditions under delivery.

Transcutaneous PO2 was continuously recorded from 17 newborns sub partu by normal deliveries and four cases with umbilical cord complications. These data were correlated with the corresponding fetal heart rate alterations and the maximum of contraction directly and 30 s thereafter. During the course of delivery three stages are differentiated: first stage of labor, full cervix dilatation, second stage of labor. The statistical evaluation succeeded in the X2-test (P less than 0.01). Deriving from this result the tcPO2 measurement is an additional aiding procedure beside the cardiotocographic co

Ventilation and perfusion alterations after smoke inhalation injury.

Previous studies of human victims of smoke inhalation injury have demonstrated retention of intravenously infused 133xenon2, 6 suggesting either: (1) true intrapulmonary shunting (Qs) secondary to alveolar collapse, flooding, or obliteration, or (2) perfusion of low ventilation/perfusion compartments (low VA/Q) secondary to bronchospasm, bronchial constriction, or partial bronchial occlusion by cellular debris. To differentiate between and quantitate the relative contribution of intrapulmonary shunt versus low VA/Q compartments, multiple inert gas analysis, as described by Wagner et al.,12 was

Application of bromhexine metabolite VIII (NA 872) and bromhexine-glucose in treatment of adult respiratory distress syndrome; experimental and clinical results .

The authors study the changes in cardiopulmonary function after application of bromhexine metabolite VIII (NA 872) in an animal model of adult respiratory distress syndrome (ARDS) and bromhexine glucose solution (BGS) in ARDS patients. After damage of the lung alveolar surfactant by intratracheal atomization of 0.2 M HCl in LEWE Minipigs significant changes in lung compliance, dead space ventilation, venous admixture, alveolo-arterial oxygen tension difference, pulmonary vascular resistance, and oxygen uptake are present under application of NA 872 in comparison to untreated animals. Depression of cardiac output is a mechanism of shunt reduction in the therapy of acute respiratory failure.

The relationship between changes in cardiac output and intrapulmonary shunt associated with mechanical ventilation was evaluated in 20 patients with the adult respiratory distress syndrome (ARDS). The distribution of ventilation-perfusion (VA/Q) ratios and the level of intrapulmonary shunt was determined by the multiple inert gas technique. Pulmonary blood flow was distributed predominantly to either effective gas-exchanging units or shunt units. Positive end-expiratory pressure (PEEP) or high tidal volume ventilation led to decreases in shunt and cardiac output without altering the overall pa

Acute experimental pulmonary responses to cardroom cotton dust.

Laboratory rabbits were exposed for 60 min to aerosols of dry cardroom cotton dust 4 days/wk (Tuesday through Friday) for 15 consecutive weeks. As a parameter of pulmonary pathophysiology, arterial blood gases were monitored with time after challenge. Post-exposure blood gas analyses at 1 hr showed progressive decreases in arterial oxygen tension, with concommitant increases in arterial carbon dioxide tension and the alveolar-arterial oxygen gradient. These responses appeared to result from acute reversible airway obstruction which caused unequal gas distribution and ventilation-perfusion ineq

Pulmonary shunt as a prognostic indicator in head injury.

Severe head injury may cause momentary respiratory arrest. Resultant hypoxia would increase cerebral edema and adversely affect the quality of survival. This study examines the effect of hypoxemia on outcome. Pulmonary shunt was calculated as a convenient measurement of respiratory insufficiency in 86 severely head-injured patients who underwent surgery. All samples were taken shortly after induction into anesthesia when controlled ventilation with high inspired-oxygen concentration had been established. In 39 patients who improved, mean pulmonary shunt was 8.9%. Twelve patients who survived w

The alteration of CO2 respiratory sensitivity in chickens by thoracic visceral denervation.

We measured respiratory movements in nine groups of six cockerels, 20-24 weeks of age. We opened the thorax and all air sacs, and unidirectionally ventilated each lung separately. The right lung received constant P(CO2), while the P(CO2) was altered to the left lung. There were only small differences in response to P(CO2) alterations whether both pulmonary circulations were intact, the left pulmonary circulation was blocked, or the left lung was denervated and the right pulmonary circulation blocked, suggesting (1) that the extrapulonary and pulmonary P(CO2) -sensitive afferents (in one lung)

Chemoreceptor influence on pulmonary blood flow during unilateral hypoxia in dogs.

Dogs anesthetized with 30 mg/kg pentobarbital were artificially respired after differential cannulation of the main stem bronchi. Following median sternotomy, blood flow was monitored by electromagnetic flow probes on the left pulmonary artery (QL) and on the pulmonary trunk or aorta, QT. Following 10 min of bilateral 100% O2, QL was 42.5 +/- 7% of QT. When 6% O2, was substituted as the gas mixture inspired by the left lung while the right lung remained on 100% O2, PaO2 was above 70 mm Hg and QL fell to 24.5 +/- 5% of QT. Room air was then used to ventilate the right lung while the left lung r
