Partial liquid ventilation versus conventional mechanical ventilation with high PEEP and moderate tidal volume in acute respiratory failure in piglets.

This prospective randomized pilot study aimed to test the hypotheses that partial liquid ventilation combined with a high positive end-expiratory pressure (PEEP) and a moderate tidal volume results in improved gas exchange and lung mechanics without negative hemodynamic influences compared with conventional mechanical ventilation in acute lung injury in piglets. Acute lung injury was induced in 12 piglets weighing 9.0 +/- 2.4 kg by repeated i.v. injections of oleic acid and repeated lung lavages. Thereafter, the animals were randomly assigned either to partial liquid ventilation (n = 6) or con

Upper airway function in Ondine's curse.

Congenital central alveolar hypoventilation (Ondine's curse) is a rare disorder with absence of automatic control of ventilation but preservation of voluntary breathing. Phrenic pacing, used to treat this condition, is usually not successful without a tracheotomy. We performed fiberoptic videoendoscopy on an affected infant airway. During wakefulness and ventilation by a negative pressure ventilator, the airway was normal, but with sleep onset, passive inspiration produced phasic epiglottic collapse. This case demonstrates the importance of central control in maintaining upper airway patency.

[Negative pressure pulmonary edema: 3 case reports].

Negative pressure pulmonary edema is a complication, described since 1977, caused by upper airway obstruction in both children and adults. Although its aetiopathogeny is multifactorial, especially outstanding is excessive negative intrathoracic pressure caused by the forced spontaneous inspiration of a patient against a closed glottis, that causes high arteriole and capillary fluid pressures that favor transudation into the alveolar space The resulting pulmonary edema can appear a few minutes after the obstruction of the airway or in a deferred way after several hours. The clinical manifestati

Congenital central hypoventilation syndrome: not just another rare disorder.

Congenital central hypoventilation syndrome (CCHS) is a rare syndrome, present from birth, and is defined as the failure of automatic control of breathing. Patients have absent or negligible ventilatory sensitivity to hypercapnia and hypoxaemia during sleep and wakefulness. Therefore, especially while asleep, children with CCHS experience progressive hypercapnia and hypoxaemia. They lack arousal responses and sensations of dyspnoea to the endogenous challenges of isolated hypercapnia and hypoxaemia and to the combined stimulus of hypercapnia and hypoxaemia. Patients with CCHS do not exhibit si

Preload responsiveness-guided fluid removal in mechanically ventilated patients with fluid overload: A comprehensive clinical-physiological study.

This study investigated fluid removal strategies for critically ill patients with fluid overload on mechanical ventilation. Traditionally, a negative fluid balance (FB) is aimed for. However, this approach can have drawbacks. Here, we compared a new approach, namely removing fluids until patients become fluid responsive (FR) to the traditional empiric negative balance approach. Twelve patients were placed in each group (n = 24). FR assessment was performed using passive leg raising (PLR). Both groups maintained stable blood pressure and heart function during fluid management. Notably, the FR g

Home mechanical ventilatory support in patients with restrictive ventilatory disorders: a 48-year experience.

UNLABELLED: We performed a retrospective analysis to the effects of negative pressure ventilation (NPV), tracheal intermittent positive pressure ventilation (TIPPV), and nasal intermittent positive pressure ventilation (NIPPV, volume or pressure-controlled ventilatory mode), in 114 patients with restrictive ventilatory disorders instituted in our hospital from 1956 until 2005. The patients were assigned on "ad hoc" basis to NPV, TIPPV, or NIPPV. All patients were subdivided in an idiopathic kyphoscoliosis group (IK, n=64), a post-poliomyelitis syndrome group (PP, n=30), or a miscellaneous grou

Systolic pressure variation in hemodynamic monitoring after severe blast injury.

Fluid management in patients following blast injury is a major challenge. Fluid overload can exacerbate pulmonary dysfunction, whereas suboptimal resuscitation may exacerbate tissue damage. In three patients, we compared three methods of assessing volume status: central venous (CVP) and pulmonary artery occlusion (PAOP) pressures, left ventricular end-diastolic area (LVEDA) as measured by transesophageal echocardiography, and systolic pressure variation (SPV) of arterial blood pressure. All three patients were mechanically ventilated with high airway pressures (positive end-expiratory pressure

An Anesthesiologist's Perspective on the History of Basic Airway Management: The "Progressive" Era, 1904 to 1960.

This third installment of the history of basic airway management discusses the transitional-"progressive"-years of anesthesia from 1904 to 1960. During these 56 yr, airway management was provided primarily by basic techniques with or without the use of a face mask. Airway maneuvers were inherited from the artisanal era: head extension and mandibular advancement. The most common maneuver was head extension, also used in bronchoscopy and laryngoscopy. Basic airway management success was essential for traditional inhalation anesthesia (ether, chloroform) and for the use of the new anesthetic agen

The effect of different modes of artificial ventilation and of some prophylactic means on the incidence of postoperative deep vein thrombosis.

The occurrence of postoperative deep vein thrombosis (DVT) was studied in 209 elective surgery patients aged 40 years or over. Most of the operations were cholecystectomies or other major abdominal operations. In the detection of thrombosis the 125I-fibrinogen method was used. Of 209 patients, 51 (24.4%) developed postoperative DVT and of these 10 developed thrombosis in both legs. In 36% of the DVT cases the process started during the operation or on the first postoperative day. Clinical signs of DVT did not agree with the 125I-fibrinogen test very well, whereas the correlation of the 125I-fi

1-4-2: Evaluation of applied mechanical power to individual lungs in a simulator-based setting of one ventilator for two patients.

BACKGROUND: The concept of ventilating multiple patients concurrently using a single ventilator has been proposed as a solution when the demand for ventilators surpasses the available supply. While the practicality of this approach has been established, a thorough evaluation of the risks involved has yet to be comprehensively addressed. METHODS: Two circuits, a simple one (circuit-1) and another with an adjustable resistance valve (circuit-2), were evaluated within an experimental framework utilizing two computer-controlled lung simulators (TestChest and ASL 5000). These simulators were venti

Negative pressure pulmonary oedema following tracheal tube obstruction in a paediatric patient: a preventable anaesthesia related morbidity.

The purpose of this study is to describe negative pressure pulmonary oedema due to undetected tracheal tube obstruction in a paediatric patient. A healthy 6 week-old scheduled for release of tongue-tie under general anaesthesia was noticed to be diagnosed at the preparation of the surgical site. The patient was quickly assessed, and ventilation with 100% oxygen was commenced. The heart sounds were still present. Two minutes later, pink frothy secretion was noticed in the lumen of the tracheal tube. Assisted manual ventilation was continued for about 3 hours in the intensive care unit (ICU). Cl

Contraction and relaxation of upper airway muscles during expiratory application of negative pressure at the mouth.

Upper airway muscles (UAM) are activated during inspiration, but little is known about their activity during expiration. We studied six healthy volunteers in supine position. Negative pressure (-10 to -30 cm H2O) was applied cyclically at the mouth during expiration. Subjects actively breathed during inspiration and either contracted or relaxed their UAM during expiration. Supraglottic pressure (Psg) was measured with a catheter with the tip at the supraglottic level. During relaxation, but not during contraction, there was expiratory flow limitation (FL) accompanied by high frequency pressure

Extracorporeal Membrane Oxygenation Combined With Esophageal Pressure-Guided Mechanical Ventilation as Salvage Therapies in Postpneumonectomy Acute Respiratory Distress Syndrome.

BACKGROUND: Massive hemoptysis and postpneumonectomy acute respiratory distress syndrome (ARDS) are rare but life-threatening conditions with limited therapeutic options. This case highlights the feasibility of combining venovenous extracorporeal membrane oxygenation (VV-ECMO) with esophageal pressure-guided ventilation to support both surgical and respiratory management in a critically ill patient. CASE SUMMARY: A 67-year-old woman developed massive hemoptysis after bronchoscopic biopsy of a left hilar lung mass. Despite bronchial artery embolization and selective right lung ventilation, sev

[Noninvasive ventilation of a 4-year-old boy with severe central late onset hypoventilation syndrome].

BACKGROUND: In the literature we found only five reports about noninvasive ventilation in cases with central hypoventilation syndrome. PATIENT AND METHOD: We report about a 4-year-old boy with severe late onset hypoventilation syndrome. During an interval of 3 months with nasal mask ventilation during sleep he showed an excellent cognitive and statomotoric development. After this time, he needed a noninvasive ventilation with a negative pressure system. RESULTS AND DISCUSSION: In our opinion, noninvasive nasal mask ventilation is a modern method in the treatment of patients with central hypo

Induction of subject-ventilator asynchrony by variation of respiratory parameters in a lung injury model in pigs.

BACKGROUND: Subject-ventilator asynchrony (SVA) was shown to be associated with negative clinical outcomes. To elucidate pathophysiology pathways and effects of SVA on lung tissue histology a reproducible animal model of artificially induced asynchrony was developed and evaluated. METHODS: Alterations in ventilator parameters were used to induce the three main types of asynchrony: ineffective efforts (IE), auto-triggering (AT), and double-triggering (DT). Airway flow and pressure, as well as oesophageal pressure waveforms, were recorded, asynchrony cycles were manually classified and the asyn

Lung recruitment and endotracheal suction in ventilated preterm infants measured with electrical impedance tomography.

AIMS: Although suctioning is a standard airway maintenance procedure, there are significant associated risks, such as loss of lung volume due to high negative suction pressures. This study aims to assess the extent and duration of change in end-expiratory level (EEL) resulting from endotracheal tube (ETT) suction and to examine the relationship between EEL and regional lung ventilation in ventilated preterm infants with respiratory distress syndrome. METHODS: A prospective observational clinical study of the effect of ETT suction on 20 non-muscle-relaxed preterm infants with respiratory distr

An exceptional cause of acute respiratory failure in an infant: negative pressure pulmonary edema.

Acute respiratory failure is an important reason for pediatric intensive care admissions. Lung parenchymal disease, airway obstruction, or neuromuscular dysfunction are the most common causes. Negative pressure pulmonary edema, characterized by pulmonary edema associated with upper airway obstruction, can rarely cause sudden onset respiratory failure. Herein, we describe an infant who suffered sudden onset respiratory failure and pulmonary hemorrhage while being held facedown for a bath, was admitted to the pediatric intensive care unit, intubated and mechanically ventilated for three days, an

Continuous negative pressure in the treatment of infants with pulmonary hypertension and respiratory failure.

We report the successful use of continuous negative pressure (CNP) with standard intermittent mandatory ventilation (IMV) in five patients suffering from respiratory failure and persistent pulmonary hypertension of the newborn (PPHN). These infants all fulfilled criteria for use of extracorporeal membrane oxygenation (ECMO) with PaO2 less than 40 torr, alveolar-arterial oxygen difference (AaDO2) greater than 620 mm Hg, and oxygenation index (OI) greater than 50. Despite a considerable amount of conventional ventilation with mean airway pressures (PAW) between 14 and 26 cm water, none of these

Spontaneous positive end-expiratory pressure (sPEEP).

Spontaneous positive end-expiratory pressure (sPEEP) is a simple, low-cost method of providing low to moderate levels of PEEP as an alternative to continuous positive airway pressure (CPAP), whether used alone or with continuous positive pressure ventilation (CPPV) in intermittent mandatory ventilation (IMV). The therapeutic effects of sPEEP therapy are demonstrated in 7 adult respiratory distress syndrome patients. All patients, initially hypoxemic on a high F1O2, had an acceptable PaO2 (with F1O2 less than or equal to 0.4) within 12 hours of initiation of sPEEP therapy. sPEEP, unlike CPAP an
