[Association between diastolic dysfunction evaluated by left ventricular flow propagation velocity and outcome in patients with hypertrophic cardiomyopathy].

OBJECTIVE: Left ventricular diastolic dysfunction is common in cardiovascular diseases. Hypertrophic cardiomyopathy(HCM) is a typical disease with diastolic dysfunction. We analyzed the association between the left ventricular flow propagation velocity (FPV), quantified by color M-mode Doppler and as an indicator for diastolic dysfunction, and clinical outcome in patients with HCM. METHODS: Standard echocardiography including FPV was performed in 43 cases with HCM and 22 control cases without a clear history of heart disease. All eligible cases were followed up to more than 1 year. RESULTS:

Short-term control of supraventricular tachycardia with verapamil infusion and calcium pretreatment.

Nineteen consecutive patients with atrial fibrillation/flutter or other types of supraventricular tachycardia were given intravenous (IV) calcium salts (1 g) followed by verapamil infusion at a rate of 1 mg/min. Successful treatment was defined as control of ventricular response to less than or equal to 100 beats per minute (bpm) or conversion to sinus mechanism in patients with atrial arrhythmias: 11 patients had atrial fibrillation; three had atrial flutter; four had reentrant supraventricular tachycardias (SVT); and one had paroxysmal SVT. Therapy was successful in all patients. The mean do

[Prenatal diagnosis and treatment of auricular flutter. Apropos of a case and review of the literature].

In newborns, atrial flutter is a rare, but severe condition. Late diagnosis was usual and based on fetal ECG. Now, fetal ultrasonography can differentiate in utero atrial flutter and other supra ventricular tachycardias; so it can be treated before development of cardiac failure. Among numerous and various available drugs, digoxin is universally recommended, but the delayed effect and the maternal serum levels to obtain effective fetal concentration of digoxin are not well known. Furthermore, endogenous "digoxin-like" substances have been found in maternal blood in late pregnancy; this finding

[Calcium channel antagonists and arrhythmias].

Ca channel antagonists (CCA) including verapamil and diltiazem work as antiarrhythmic drugs. Because CCA suppresses conduction through atrio-ventricular (AV) node, it is effective on both AV reciprocating tachycardia (AVRT) and AV nodal reentrant tachycardia (AVNRT). It is also useful to reduce ventricular response during atrial fibrillation/flutter. Idiopathic left ventricular tachycardia (VT) with a right bundle-branch block configuration and superior axis is sensitive to verapamil, and thus called as verapamil-sensitive VT. A caution should be needed to prevent adverse effects based on card

Narrow-QRS and Wide-QRS Tachycardias.

In a woman with rheumatic heart disease, atrial flutter with a rapid ventricular response, and congestive heart failure, treatment with digoxin slows conduction in the atrioventricular node and thus allows atrioventricular conduction to occur by way of a previously unrecognized accessory pathway.

Characterization of atrial flutter after pulmonary vein isolation by cryoballoon ablation.

PURPOSE: Pulmonary vein isolation (PVI) by cryoballoon ablation (CBA) has emerged as a commonly used technique for the treatment of atrial fibrillation. We sought to explore the incidence, risk factors for, and characterization of post-CBA-PVI atrial flutter. METHODS: We analyzed a prospective registry of patients who underwent CBA-PVI at a single institution. We included patients with more than 3 months of follow-up data and excluded those with a history of cavotricuspid isthmus (CTI) ablation. Locations of post-CBA-PVI atrial flutters were determined by analysis of intracardiac electrograms

Sudden unexpected death during ambulatory electrocardiographic monitoring. The Framingham Study.

This report documents the clinical and electrocardiographic events of sudden unexpected death in a 52-year-old man without known heart disease during ambulatory electrocardiographic monitoring. This death occurred two and a half hours after detailed noninvasive testing, including treadmill exercise, that was unrevealing. The sequence of S-T change suggesting epicardial injury, multiform ventricular premature depolarizations, and frequent and early cycle ventricular premature depolarizations followed by ventricular tachycardia-flutter-fibrillation precipitated by an early cycle ventricular prem

[Intravenous sotalol in the acute treatment of supraventricular tachycardias].

The effects of intravenous sotalol (0.5 mg/kg in 6 minutes) were studied in 23 patients with supraventricular tachycardia (ventricular rate greater than 120 bpm) after failure of placebo (isotonic dextrose). Thirteen patients had atrial flutter or fibrillation and 10 a junctional tachycardia of recent onset. Sinus rhythm was restored in 4 of the patients with atrial flutter or fibrillation and the ventricular rate was slowed significantly in 3 patients (less than 100 bpm). In 3 other patients the ventricular rate decreased (31 to 35 p. 100) but remained above 100 bpm. Sinus rhythm was restored

[Complications of radiofrequency ablation: A French experience. Le Groupe de Rythmologie de la Société Francaise de Cardiologie].

The multiplication of the indication of radiofrequency for curative or palliative treatment of cardiac arrhythmias motivated the Rhythmology Working Group to set up a register of the complications related to this technique. Five thousand six hundred and eighty nine cases from 19 French centres were recensed up to March 1996. There were 2765 ablations of a Bundle of Kent, 1579 ablations of slow pathways, 621 ablations of atrial flutter, 25 ablations of atrial tachycardia, 200 ablations of ventricular tachycardia and 499 ablations of the Bundle of His. Eighty five complications (1.5%), two of wh

Combination GLP-1 receptor agonist and SGLT2 inhibitor therapy versus GLP-1 receptor agonist monotherapy on arrhythmia outcomes in non-diabetic obese patients: A real-world time-dependent analysis.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1RA) and sodium-glucose cotransporter-2 inhibitors (SGLT2i) have established cardiometabolic benefits and are increasingly used in combination, including in patients without diabetes. Although both drug classes have been studied extensively for metabolic and cardiovascular outcomes, their effects on cardiac arrhythmias, particularly in non-diabetic obese patients, remain poorly defined. OBJECTIVE: To compare arrhythmia outcomes in non-diabetic obese patients receiving combined GLP-1RA and SGLT2i therapy versus GLP-1RA alone, with part

Association of Fetal Atrial Flutter with Neonatal Atrioventricular Re-entry Tachycardia Involving Accessory Pathway: A Link to be Remembered.

To investigate prenatal and postnatal outcomes of atrial flutter and its association with the development of a second tachycardia, following restoration of sinus rhythm, in the fetus or newborn. This study is a retrospective review of all fetuses that presented with atrial flutter from January 2001 to December 2019 at the University Hospital of Wales, Cardiff, UK. The specific type of arrhythmia, its time of appearance and clinical characteristics, echocardiographic findings, medical management, and postnatal outcomes were evaluated. Sixteen fetuses were diagnosed with atrial flutter (AFL). Th

Cardiac arrhythmias in patients with surgical repair of Ebstein's anomaly.

Preoperative, perioperative and postoperative arrhythmias in 52 consecutive patients who underwent operation for Ebstein's anomaly were reviewed. There were 25 male and 27 female patients (mean age 18 years, range 11 months to 64 years). Thirty-four patients had one or more documented arrhythmias preoperatively (18 had paroxysmal supraventricular tachycardia, 10 had paroxysmal atrial fibrillation or flutter, 13 had ventricular arrhythmia and 3 had high grade atrioventricular block). Seven patients without documented arrhythmias had a history typical of tachyarrhythmias. During the perioperativ

Rare Presentation of a Metastatic Pancreatic Neuroendocrine Neoplasm Presenting with Atrial Flutter.

UNLABELLED: Pancreatic neuroendocrine neoplasms (PanNENs) rarely secrete serotonin, which is the main cause of carcinoid syndrome. One of its unusual manifestations is carcinoid heart disease or Hedinger's syndrome which is seldom accompanied by cardiac arrhythmias. We report the case of an 88-year-old woman who presented with recently experienced episodes of palpitations and a newly developed atrial flutter with a ventricular rate of 130 beats per minute. Echocardiography revealed thickened and tethered tricuspid and pulmonary valve leaflets causing severe valvular regurgitation and right ven

Catheter ablation of the atrioventricular node using radiofrequency energy.

BACKGROUND: Catheter ablation of the atrioventricular (AV) junction using stored direct current (DC) energy from a standard DC Cardioverter defibrillator was first reported in 1982. Since then many patients have been treated using this procedure for refractory supraventricular arrhythmias, usually atrial fibrillation and flutter. Undesirable thermal effects such as barotrauma and arcing are largely responsible for complications associated with the use of DC energy. This report details our experience of catheter ablation of the AV junction using radiofrequency (RF) energy in a series of 30 cons

Sotalol. An updated review of its pharmacological properties and therapeutic use in cardiac arrhythmias.

Sotalol is a nonselective beta-adrenoceptor antagonist which prolongs cardiac repolarisation independently of its antiadrenergic action (class III antiarrhythmic properties). The antiarrhythmic action of sotalol appears to arise predominantly from its class III properties, and the drug exhibits a broader antiarrhythmic profile than the conventional beta-blockers. Sotalol is effective in controlling paroxysmal supraventricular tachycardias and the ventricular response to atrial fibrillation/flutter in Wolff-Parkinson-White syndrome, in maintaining sinus rhythm after cardioversion of atrial fibr

Neonatal atrial flutter: significant early morbidity and excellent long-term prognosis.

Twenty-five neonates (16 boys and 9 girls) who had atrial flutter were identified. Diagnosis was made on or before the first day of life in 18 (72%). Heart failure were present in 9 patients, and hydrops fetalis was present in another 5. Atrial and ventricular rates did not differ between symptomatic and asymptomatic patients. Atrioventricular conduction was variable in 16 patients, and documented 1:1 conduction occurred in 5. Digoxin was the initial drug therapy given to 21 patients, with 7 reverting to sinus rhythm. Electrical cardioversion (pacing or synchronized shock) was attempted in 13

[Current concepts in diagnosis and treatment of tachyarrhythmias].

A diagnostic and therapeutic approach of supraventricular and ventricular tachyarrhythmia is always challenging. Several criteria serve to discriminate correctly between these two types of tachycardia. Cardiac arrhythmias are terminated reliably by intravenous (IV) application of antiarrhythmic drugs: adenosine for supraventricular arrhythmia, amiodarone or ajmaline for ventricular tachycardia. Furthermore, AV-nodal tachycardia, atrioventricular reciprocating tachycardia, and typical atrial flutter is treated curatively by radiofrequency ablation during an electrophysiological study. Ventricular fibrillation in two infants treated with amiodarone hydrochloride.

Antiarrhythmic agents may induce ventricular arrhythmias. Whereas amiodarone is a reported cause of ventricular fibrillation in children, there are no reported cases in infants. We observed two patients with atrial flutter and congestive heart failure who were treated with amiodarone. Both had anatomic heart defects and had failed conventional therapy. Ventricular fibrillation occurred 3 and 12 days after the initial dose. Q-T interval, electrolytes, digoxin level, and amiodarone level (one patient) were normal. Amiodarone may provoke life-threatening ventricular tachyarrhythmias during the co
