The breech presentation and the vertex presentation following an external version represent risk factors for neonatal hip instability.

UNLABELLED: The aim of this study was to evaluate the frequency and type of hip-joint instability and the frequency of hip dislocation requiring treatment in neonates who had been lying in the breech presentation and were delivered vaginally after an external version or by caesarean section, and to compare them with neonates who were naturally in the vertex presentation. Breech presentations without ongoing labour were subjected to an attempted external version and, in cases where this proved unsuccessful or where labour had started, to deliver by caesarean section. None of the breech presenta

[Is the Mauriceau maneuver deleterious? Study of 103 cases].

The Mauriceau manoeuvre has a poor reputation in France where some obstetricians believe it leads to an increase in the number of neonatal traumal injuries. To evaluate this hypothesis we examined the results of a personal series of 103 cases of breech extraction where foetal head extraction was performed using the Mauriceau manoeuvre. Our study showed that the level of traumal complications was not worse than that of the general neonatal population. We therefore conclude that this active and organized method of breech delivery is safe and provides a young obstetrician with valuable experience

Perinatal outcome and the type and number of maneuvers in shoulder dystocia.

OBJECTIVES: To ascertain the type and order of maneuvers that should be used for the treatment of shoulder dystocia, and to determine its correlation with perinatal outcome. METHODS: We reviewed all consecutive cases of shoulder dystocia from January 1986 to August 1994 in our institution to obtain the type, order and number of maneuvers used for delivery. Patients were stratified by the number of maneuvers required for delivery. Outcome parameters included cord pH, Apgar score, neonatal trauma (Erb's palsy and fracture), and maternal trauma. RESULTS: The incidence of shoulder dystocia was 0

Breech presentation in the third trimester: factors influencing spontaneous cephalic version and delivery outcome differences between spontaneous and non-spontaneous cephalic presentations.

INTRODUCTION: The prevalence of breech presentation ranges from 6.9% to 10.0% between weeks 32 and 36 of gestation, gradually declining to 3.0-5.0% by the time an external cephalic version (ECV) can be offered. The primary objective of our study was to identify the factors favoring a spontaneous version between the routine third-trimester ultrasound and the proposal of external cephalic version. We also compared the maternal and neonatal outcomes at delivery between fetuses who spontaneously turned to the cephalic presentation and those who were in the cephalic presentation after external ceph

Intrapartum Ultrasound Guidance to Make Safer Any Obstetric Intervention: Fetal Head Rotation, Assisted Vaginal Birth, Breech Delivery of the Second Twin.

Intrapartum ultrasound (US) is more reliable than clinical assessment in determining parameters of crucial importance to optimize the management of labor including the position and station of the presenting part. Evidence from the literature supports the role of intrapartum US in predicting the outcome of labor in women diagnosed with slow progress during the first and second stage of labor, and randomized data have demonstrated that transabdominal US is far more accurate than digital examination in assessing fetal position before performing an instrumental delivery. Intrapartum US has also be

Vaginal delivery of vertex-nonvertex twins: a fading skill?

BACKGROUND: Delivery of the second twin is one of the most challenging events for practicing obstetricians. Due to the increased incidence of twin gestations and lack of well-designed clinical trials, evidence-based recommendations are lagging behind clinical demand. OBJECTIVE: The present review examines the practice of second nonvertex twin deliveries. METHODS: A review of the literature was conducted using the term vertex-nonvertex; twin gestation; mode of delivery; and cesarean delivery. RESULTS: There is a lack of evidence supporting the appropriate approach to second nonvertex twin de

[External version of the foetus from breech presentation in patients in condition following abdominal caesarean section (author's transl)].

Since 1974 we have performed more than 500 external versions of the fetus from breech to vertex presentation under tocolysis near to term. Among these were 20 patients who had previously had a caesarean section; in 15 cases the version was successful and in 5 cases unsuccessful. There were only 4 cases of short-lasting bradycardia or slight longer-lasting tachycardia in the cardiotocograms. Twice complications occurred (once vaginal bleeding and once umbilical cord presentation) which did not lead to serious consequences. After successful versions made on 15 patients, 10 patients could be deli

Should we abandon Kielland's forceps?

To assess the risks associated with the use of Kielland's forceps 2708 consecutive deliveries were studied prospectively and the neonatal outcome related to the mode of delivery. Of the 1191 primigravidas, 279 (23.4%) underwent instrumental delivery, of whom 65 (5.5%) were delivered with Kielland's forceps. There was no difference in early neonatal outcome (as judged by Apgar scores, intubations, and admission to the special care baby unit) between these babies and those delivered normally or by non-rotational forceps, but a higher proportion of the 127 (10.7%) delivered by emergency caesarean

Spontaneous version after preterm prelabor rupture of membranes.

BACKGROUND: Preterm prelabor rupture of membranes (PPROM), defined as rupture of membranes before the onset of labor at < 37 weeks' gestation, affects approximately 3% of all pregnancies. For PPROM prior to 34 weeks' gestation, in the absence of contraindications to expectant management, antibiotics are administered to increase the latency period and reduce the incidence of maternal and neonatal infection. The latency period additionally provides an opportunity for fetal presentation to change, which impacts the mode of delivery. It is important to better understand the incidence of spontaneou

Combined Complementary and Alternative Therapies for the Management of a Breech Fetus: A Feasibility Study.

OBJECTIVE: Cesarean rates are rising in the United States, especially for breech presentations, which complicates 3 to 4% of term pregnancies and contributes to maternal morbidity. Complementary and alternative therapies (CT) like moxibustion, chiropractic, and hypnosis have been suggested as noninvasive options to encourage fetal version. This study assessed the feasibility and acceptability of combined CT for breech management. STUDY DESIGN: Women aged > 18 with a singleton breech fetus at 34 to 37 weeks engaged in three study visits within 2 weeks. The intervention included therapies such

Significance of Selected Environmental and Biological Factors on the Risk of FASD in Women Who Drink Alcohol during Pregnancy.

Prenatal alcohol exposure (PAE), which refers to alcohol consumption by pregnant women, is associated with the risk of numerous severe complications during fetal development. The State Agency for Alcohol Problem Solving reports that the incidence of fetal alcohol spectrum disorder (FASD) in Poland's general population is over 1.7%, and the incidence of fetal alcohol syndrome (FAS) is estimated at more than 0.5%. This study aimed to evaluate the significance of alcohol exposure and focused on the pattern of alcohol intoxication exhibited by the mother during pregnancy and other environmental fa

Changes in fetal presentation in twin pregnancies.

OBJECTIVE: The aim of the study was to describe the occurrence of spontaneous version in twins in the third trimester, as well as the correlation between antepartum and birth presentation. METHODS: Twin pregnancies were divided into four gestational age intervals: 20-23; 24-27; 28-31; and 32-36 weeks. Fetal presentation was categorized as cephalic (C) or non-cephalic (NC). Hospital records were reviewed to determine demographic factors, including mode of conception. Chi-square was used to compare distributions of presentations, and Cramer's V measure of association was used to correlate prese

Intra- and early postpartum ultrasonography: a review. Part I.

The objective of this article is to review current literature pertaining to intra- and early postpartum sonography. All the manuscripts published in the English language regarding this topic were selected from a MEDLINE search from 1966 through August 1997. Additional sources were identified through cross-referencing. Currently, intra- and early postpartum sonography may be performed for either maternal or fetal indications. Maternal indications include cervical assessment in preterm labor/rupture of membranes, assessment of the lower uterine segment, size and position of uterine fibroids, gui

Characteristics and Outcomes Associated with the Attempt of External Cephalic Version in U.S. Birth Certificate Data.

OBJECTIVES: Malpresentation is the indication for approximately 17% of primary cesarean deliveries (CD). External cephalic version (ECV) is a safe and effective procedure to reduce CD. Our primary objective is to identify associations between patient demographics and rate of ECV attempt. Secondary objectives include determining the rate of ECV attempt in the United States, and assessing neonatal and maternal outcomes associated with ECV attempt. STUDY DESIGN: Data were obtained from the National Center for Health Statistics birth certificate data from 2014 to 2021. We analyzed live, singleton

Manual Versus Instrumental Rotation of the Fetal Head in Malposition at Birth (ROTATE Study): Protocol for a Randomized Controlled Trial.

BACKGROUND: Approximately 5% of women will be affected by fetal malposition at full cervical dilatation, with the occiput in transverse or posterior positions. These women are more likely to require assistance to give birth to their babies with either rotational vaginal birth or cesarean birth at full dilatation. Three different rotational methods can be used: rotational (Kielland) forceps, rotational vacuum, and manual rotation. Current evidence supporting the use of the 3 rotational methods is only from observational data. To date, no randomized controlled trial (RCT) of rotational methods h

[Breech Presentation: CNGOF Guidelines for Clinical Practice - External Cephalic Version and other Interventions to turn Breech Babies to Cephalic Presentation].

OBJECTIVES: To provide guidelines regarding efficiency and safety of external cephalic version (ECV) attempt and alternatives methods to turn breech babies to cephalic presentation. METHODS: MedLine and Cochrane Library databases search in French and English and review of the main foreign guidelines between 1980 and 2019. RESULTS: ECV is associated with a decreasing rate of breech presentation at birth (LE2), and potentially with a lower rate of cesarean section (LE3) without an increase of severe maternal (LE3) and perinatal morbidity (LE3). It is therefore recommended to inform women with
