BACKGROUND: Lobectomy using video-assisted thoracoscopic surgery (VATS) still is a controversial operation despite its many observed benefits. The controversy may be due to difficulties performing the procedure. This study addresses a standardized anterior approach facilitating the operation.
METHODS: This report describes 156 VATS lobectomies performed with a uniform anterior three-port technique. The surgeon and the assistant stand on the abdominal side of the patient, and the setup is independent of the lobe to be resected.

Video-assisted thoracic surgery (VATS) has developed very rapidly in these two decades, and has replaced conventional open thoracotomy as a standard procedure for some simple thoracic operations as well as an option or a complementary procedure for some other more complex operations. In recurrent or persistent hyperparathyroidism, accurate location of the abnormal gland is essential before further surgery, but the variety of available imaging techniques suggests that no one procedure is universally reliable. We report two cases in which clear preoperative visualization of adenoma with double-phase 99mTc-MIBI scintigraphy and exact high-resolution CT location permitted successful minimally invasive surgery.

Disruption of a mainstream bronchus is a rare but dreaded complication of pneumonectomy. When the bronchial tump measures at least 15 mm, the conventional therapeutic strategy is to drain the pleural cavity followed by closure of the fistula via trans-mediastinal sternotomy. After an experimental study on cadavers to test the technical feasibility of main bronchus closure via a cervical approach using a video-mediastinoscope, we used video-assisted mediastinal surgery successfully in a patient with a left main bronchus fistula.

The Authors report their experience in the thoracoscopic surgical treatment of neurovascular abnormalities of the upper extremities. OBJECTIVE: We evaluated the feasibility and efficacy of video-assisted thoracic surgery (VATS) for fibropurulent thoracic empyema.
PATIENTS AND METHODS: Twenty-six consecutive patients with thoracic empyema resistant to medical therapy were treated by VATS from 1997 to 2006.
