[Pupillary dilatation with Eckardt iris retraction sutures within the scope of vitreoretinal surgery. A follow-up study].

In cases of complicated vitreoretinal surgery, maximal mydriasis is necessary. Preexisting or intraoperatively occurring miosis in aphakic eyes can be remedied by the iris suture by Eckardt. In our hospital, the iris suture technique was carried out in 31 cases. The postoperative situation depends on the extent of retraction of the iris sutures and the general course of the disease. At maximal retraction, the iris is forced against the ciliary body, causing persisting synechias; therefore, to us it seems important to separate all synechias after opening the iris sutures. The formation of anter

Factors influencing ophthalmology trainees' decision to pursue vitreoretinal surgery fellowship in Saudi Arabia: a cross-sectional study.

BACKGROUND: The demand for vitreoretinal surgery (VRS) specialists is increasing globally due to the rising prevalence of retinal diseases such as diabetic retinopathy and age-related macular degeneration. In Saudi Arabia, the burden of retinal diseases is particularly high, with diabetic retinopathy affecting about one-third of diabetic patients. Despite this, factors influencing ophthalmology trainees' decisions to pursue VRS fellowship remain underexplored. This study aimed to identify the key factors shaping career decisions among ophthalmology trainees in Saudi Arabia. METHODS: A cross-s

The effects of active warming on perioperative inadvertent hypothermia in patients undergoing vitreoretinal surgery under local anesthesia.

PURPOSE: Perioperative inadvertent hypothermia (PIH) is the decrease in core temperature below 36°C. We aimed to assess whether PIH develops in patients operated under local anesthesia (ULA) for vitreoretinal surgery in the operating room and investigate active warming efficacy. METHODS: Seventy-two patients were divided into two groups: Group 1 contained unwarmed patients (n = 36), and Group 2, warmed patients (n = 36). The core temperatures, heart rate (HR), and mean arterial pressure (MAP) of the patients were measured at the beginning of surgery, after 20 min, 40 min, 1 h, at the end of t

Transconjunctival sutureless vitrectomy versus 20-gauge vitrectomy for vitreoretinal surgery: a meta-analysis of randomized controlled trials.

BACKGROUND: Transconjunctival sutureless vitrectomy is appealing to vitreoretinal surgeons around the world. However, its use versus 20-gauge vitreoretinal surgery is still controversial. The aim of this meta-analysis was to compare the safety and functional outcomes of transconjunctival sutureless vitrectomy versus 20-gauge vitrectomy for vitreoretinal surgery. METHODS: Data sources, including PubMed, Medline, EMBASE, and Cochrane Controlled Trials Register, were searched to identify potentially relevant randomized controlled trials. A meta-analysis was performed using RevMan 5.0 software. T

The Influence of Low-Dose Dexmedetomidine on Recovery Times Following Sedation for Vitreoretinal Surgery: A Retrospective Cohort Study.

BACKGROUND: Dexmedetomidine for procedural sedation during ophthalmic surgery is popular, in part due to high levels of patient satisfaction and analgesia, and low intraocular pressure and respiratory depression, compared to alternative medications. However, longer recovery times were reported when dexmedetomidine was used for cataract or vitreoretinal surgery. Because of these reports, a concern was raised by our hospital's retinal surgeons and administrators that dexmedetomidine might be delaying discharge home from the postanesthesia care unit. Therefore, we performed a retrospective analys

Adverse Events Following Vitreoretinal Surgeries Under Adequacy of Anesthesia with Combined Paracetamol/Metamizole-Additional Report.

Background/Objectives: Some patients undergoing vitreoretinal surgery (VRS) require general anesthesia (GA), despite the possibility of developing intolerable postoperative pain perception (IPPP). Intraoperative rescue opioid analgesia (IROA) administration during GA poses a risk of perioperative nausea and vomiting (PONV), which may result in suprachoroidal hemorrhage with permanent visual impairment. Adequacy of Anesthesia (AoA) optimizes intraoperative IROA titration. Intravenous preemptive analgesia (IPA) with cyclooxygenase-3 (COX-3) inhibitors is added to GA to reduce the IROA dose. In t

Visual outcomes of vitreoretinal surgery in eyes with severe open-globe injury presenting with no-light-perception vision.

BACKGROUND: Severe ocular trauma causing no light perception (NLP) typically carries a dismal prognosis, and implies no further therapeutic intervention. We have identified a cohort of patients with verified NLP following open-globe injury who have recovered vision of light perception (LP) or better. We evaluated the outcomes of vitreoretinal surgery performed on eyes that were NLP post open-globe injury. METHODS: Retrospective review of outcomes of secondary vitreoretinal surgery performed at Massachusetts Eye and Ear Infirmary from 1 January 2001 to 31 December 2006 on all cases of open-glo
