[When should tracheotomy be performed in bilateral vocal cord paralysis involving multiple system atrophy?].

OBJECTIVES: Bilateral vocal cord paralysis caused by central nervous system dysfunction results from such diverse causes as cerebrovascular disorder and neurodegenerative disease. Otolaryngologists are often consulted about indications of tracheostomy for such cases, but if their recognition of causative disease is insufficient, it is difficult to judge indications of tracheostomy. We reviewed tracheostomy cases due to bilateral vocal cord paralysis caused by multiple system atrophy (MSA) and considered points to keep in mind in such cases. MATERIALS AND METHODS: We diagnosed 9 cases of vocal

Vocal cord paralysis.

Vocal cord paralysis is a multifacted problem that affects patients of all ages and presents initially to a wide range of healthcare professionals. It can cause laryngeal dysfunction ranging from slight hoarseness to life-threatening airway obstruction. When confronted with a patient with new onset vocal cord paralysis, the physician should determine the etiology of the paralysis. Only after an accurate diagnosis, can restoration of laryngeal function be addressed. Peripheral lesions injuring the vagus nerve or its branches are responsible for 90% of all vocal cord paralysis. Etiologies includ

Acute asphyxia caused by Gerhardt's syndrome associated with asthma.

Vocal cord dysfunction (VCD) has been frequently discussed as confounding the diagnosis of asthma, and it is recommended to examine vocal cord function in cases of difficult asthma. However, there is also strong evidence that asthma may be associated with VCD, sometimes masquerading as exercise-induced asthma. Gerhardt's syndrome frequently induces severe acute respiratory distress requiring ventilatory support, and this condition might be mistaken for a severe acute asthma attack. Here, a case of bilateral paralysis of the vocal cords, which was associated with asthma, is described.

Vocal Cord Disorders.

Vocal cord disorders present with a variety of symptoms including dysphonia, respiratory symptoms, and stridor. When evaluating symptoms, a complete history and through head, neck, and neurologic examinations are necessary. If dysphonia persists for greater than 4 w or there is associated smoking, then larngoscopy is necessary to evaluate the vocal folds. Empiric treatment of dysphonia is not recommended without direct visualization of the vocal folds. Most masses of the vocal folds are benign and resolve with voice hygiene and speech therapy. Surgery is reserved for persistent symptomatic nod

Use of POCUS in the Diagnosis of Paradoxical Vocal Cord Movement.

Point-of-care ultrasound has been instrumental in allowing providers to make prompt diagnoses at the bedside but has been historically underutilized for the diagnosis of upper airway processes, including paradoxical vocal fold motion (PVFM), also known as vocal cord dysfunction. PVFM is characterized by adduction of the vocal cords during inspiration, resulting in stridor and shortness of breath. This case report describes a teenage girl who presented to the pediatric emergency department (ED) with difficulty breathing. Point-of-care ultrasound diagnosed PVFM, which was confirmed with bedside

Classification of laryngeal disorders based on shape and vascular defects of vocal folds.

Vocal fold disorders such as laryngitis, vocal nodules, and vocal polyps may cause hoarseness, breathing and swallowing difficulties due to vocal fold malfunction. Despite the fact that state of the art medical imaging techniques help physicians to obtain more detailed information, difficulty in differentiating minor anomalies of vocal folds encourages physicians to research new strategies and technologies to aid the diagnostic process. Recent studies on vocal fold disorders note the potential role of the vascular structure of vocal folds in differential diagnosis of anomalies. However, standa

Vocal cord paralysis as a presenting sign in the Shy-Drager syndrome.

A 70-year-old patient with bilateral vocal cord paralysis, accompanied by inspiratory stridor and increased snoring, is presented. Respiratory dysfunction, deteriorating over 18 months, necessitated permanent tracheostomy. Only two and a half years after the onset of symptoms the patient developed extrapyramidal signs combined with severe autonomic failure, suggesting a diagnosis of Shy-Drager syndrome. Vocal cord palsy preceding any other neurological or autonomic manifestations of that syndrome has been infrequently described. This diagnosis should be considered in cases of vocal cord palsy

Paradoxical Vocal Fold Movement: A Retrospective Analysis.

BACKGROUND: Paradoxical vocal fold motion (PVFM) is a disorder in which the vocal folds adduct inappropriately during inspiration resulting in episodic dyspnea and sometimes respiratory distress. Diagnosis is obtained through careful history, physical examination, flexible laryngoscopic examination with provocative maneuvers, and laryngeal electromyography. The pathogenesis and clinical findings of this disorder are not known. OBJECTIVES: To determine characteristics of patients with confirmed PVFM and to evaluate efficacy of current treatments. METHODS: A retrospective chart review of the p

Vocal fold hyalinosis in Urbach-Wiethe disease, a rare cause of hoarseness.

BACKGROUND: Lipoid proteinosis is an autosomal recessive disorder characterized by hyalin deposits in the skin and mucosa of the upper aerodigestive tract; currently, no treatment exists. Nearly all patients experience hoarseness and speech difficulties, due to hyalin deposition in the vocal folds and diminished mobility in infiltrated lips, tongue, and palate. METHODS: We describe a patient with extensive hyalin plaques on the vocal folds, which resulted in near-aphonic hoarseness. Hyalin deposits in the vocal folds and skin were treated with laser resection. RESULTS: Both the vocal folds a

Vagal nerve magnetic stimulation for post-traumatic cricopharyngeal achalasia with bilateral vocal cord paralysis: A case report.

RATIONALE: Post-traumatic dysphagia is a severe complication of traumatic brain injury, particularly in cases involving medullary damage. The rare combination of cricopharyngeal achalasia and bilateral vocal cord paralysis presents substantial challenges, with profound swallowing dysfunction and increased aspiration risk. Current treatments primarily target cricopharyngeal dysfunction but lack effective solutions for concurrent vocal cord paralysis, highlighting the need for innovative therapeutic strategies. PATIENT CONCERNS: A 15-year-old male presented with severe dysphagia, confirmed as u

Vocal cord paralysis following lithium button battery ingestion in children.

This study reports on vocal cord paralysis caused by esophageal button battery (BB) ingestion in children. Medical records of children with vocal cord paralysis and esophageal BB ingestion treated at a tertiary referral institute between January 2016 and March 2020 were reviewed. Five patients aged 9-58 months were identified; three were male. Each patient had accidentally swallowed a 20-mm-diameter lithium battery (3 CR2032 type and 2 CR2025 type). One battery was removed within 4 h after ingestion, and three batteries were removed within 12 h. Removal of the battery was achieved with rigid e

[A case of bilateral recurrent laryngeal nerve palsy after thyroid surgery under intraoperative nerve monitoring].

A 66-year-old woman suffering from Basedow disease had total thyroidectomy under intraoperative monitoring (IOM) of recurrent laryngeal nerve (RLN) using Medtronic Xomed Nerve Integrity Monitor-2 (Medotronic, Mineapolice, Minesota, USA). IOM indicated a positive signal for her right RLN while the signal for her left RLN disappeared during the operation. During the surgery, surgeons identified her left RLN which was anatomically intact. During the attempted extubation of the endotracheal tube, the patient experienced severe dyspnea resulting in re-intubation. Fiber-optic examination revealed bi

Lead breakage and vocal cord paralysis following blunt neck trauma in a patient with vagal nerve stimulator.

Patients with medically intractable seizures who are not candidates for epilepsy surgery are left with few options. Vagal nerve stimulation therapy is often a viable alternative for these patients and can have a positive impact on quality of life. Rarely complications may occur. We report a case of mild blunt neck trauma resulting in VNS malfunction and delayed vocal cord paralysis. A systematic review of the literature on VNS malfunction, self-inflicted injuries, vagal nerve injury, and common side effects including voice changes was performed. Only a handful of relevant publications were fou

Anti-IgLON5 Disease Showing an Improvement in Dysautonomia, Including Vocal Cord Palsy, via Combined Immunotherapy.

Anti-IgLON5 disease shows various neurological manifestations, of which dysautonomia is one of the major symptoms and is rarely improved by immunotherapy. We herein report a patient with anti-IgLON5 disease who showed several autonomic failures, including vocal cord palsy for four months. The patient presented with cognitive impairments, bulbar symptoms accompanied by myorhythmia in the pharynx and tongue, cerebellar ataxia with tremor, motor neuron symptoms in the limbs, gastrointestinal dysfunction, orthostatic hypotension, non-rapid eye movement sleep disorder on polysomnography, and severe

[A Critical Estimation of Intraoperative Neuromonitoring (IONM) in Thyroid Surgery].

We investigated 238 patients with 431 nerves at risk (NAR) undergoing thyroid surgery. Positive identification of the recurrent laryngeal nerve was obtained in 99.3 % of NAR with intraoperative neuromonitoring. 19 patients (4.4 % NAR) suffered from unilateral vocal cord dysfunction in the early postoperative phase. A complete restitution of vocal cord function could be demonstrated in 18 of these patients, leaving one patient (0.23 % NAR) with a permanent vocal cord dysfunction. Our data show that reliable predictions concerning the postoperative outcome of nerve function cannot always be made

[Vocal dysfunctions: bases for rehabilitation].

Based on our observations of vocal dysfunction modes, we set up a classification of dysfunctions. Most often, we encounter a "prolonged and usual vocal forcing on an anatomically normal larynx". This forcing often essentially predominates breathing, but it may predominate the larynx, in the posterior glottis or on the vibrating area of the vocal cords. We individualized the very specific group of dysphonias with limited tone where the forcing in located, especially in the resonators. We reviewed the dysphonias with modified larynx, by secondary lesions or primitive pathologies. Analysis of the

Heavier Weight of Resected Thyroid Specimen Is Associated With Higher Postoperative Morbidity in Benign Goiter.

OBJECTIVE: The impact of heavier weight of resected thyroid specimen on postoperative morbidity after total thyroidectomy for multinodular benign goiter remains unclear. METHODS: Data from the prospective StuDoQ|Thyroid registry of the German Society of General and Visceral Surgery were analyzed regarding the weight of the resected thyroid specimen and perioperative morbidity (vocal cord palsy, hemorrhage, surgical site infection, and hypocalcemia). To achieve a homogeneous patient population, only patients with total thyroidectomy for multinodular benign goiter were included. RESULTS: A tot

Thyroplasty type I for unilateral vocal cord paralysis.

Unilateral vocal cord paralysis may occur with trauma, neoplasms, mechanical or central nervous system dysfunction, or following extensive aortic and mediastinal vascular surgery or thyroidectomy. Thyroplasty type I is a form of laryngeal framework surgery (ie, phonosurgery) used to treat unilateral vocal cord paralysis. A silicone implant is placed inside a surgically created window in the thyroid cartilage and pushes the paralyzed cord medially, allowing the moving cord to touch the paralyzed cord and close the opening. The procedure is performed under monitored local anesthesia with sedatio
