[Laryngological aspects of treatment of male-female transsexuals].

The authors discuss laryngologist's participation in the adapting treatment of transsexuals of M/F type, including reduction of laryngeal prominence of thyroid cartilage, voice rehabilitation and nose operation. Literary reference is presented in the paper, as well as a case of a 27-year-old patient a transsexual of M/F type, in whom the prominence of thyroid cartilage was removed. Ethic and legal aspects of that management are also demonstrated.

Trends in voice rehabilitation in the United Kingdom.

Three main basic forms of vocal rehabilitation are available for post-laryngectomy patients in this country: oesophageal, mechanical vibrator and valved speech. In Sheffield, we advocate the use of valved speech, and insertion of the prosthesis preferably at the time of laryngectomy. We studied current vocal rehabilitation trends in the United Kingdom by a random survey of 100 consultant otolaryngologists and present our findings here. While all forms of vocal rehabilitation are provided in the majority of centres, 20% of units involved in head and neck surgery do not have optimum levels of sp

Expiratory muscle strength training: speech production outcomes in patients with multiple sclerosis.

PURPOSE: This study investigated the effect of expiratory muscle strength training (EMST) on voice production, dysarthria, and voice-related quality-of-life issues in persons with multiple sclerosis (PwMS). It was hypothesized that PwMS would have improved voice production and reduced voice-related quality-of-life issues following EMST. PARTICIPANTS AND METHODS: Seventeen participants with MS and 14 healthy (H) controls completed 8 weeks of EMST, followed by 4 weeks of no training. Analyzed outcomes as a function of EMST were maximal expiratory pressure (MEP), sustained vowel prolongation (SV

Effectiveness of voice therapy in functional dysphonia: where are we now?

PURPOSE OF REVIEW: To review the recent literature since the 2009 Cochrane review regarding the effectiveness of voice therapy for patients with functional dysphonia. RECENT FINDINGS: A range of articles report on the effects of voice therapy treatment for functional dysphonia, with a wide range of interventions described. Only one study is a randomized controlled trial. A number of excellent review articles have extended the knowledge base. In primary research, methodological issues persist: studies are small, and not adequately controlled. Studies show improved standards of outcome measurem

Does the voice improve with vocal hygiene education? A study of some instrumental voice measures in a group of kindergarten teachers.

This study investigated the efficacy of a program of vocal hygiene education designed for an occupational group at risk for dysphonia, namely, kindergarten teachers. Twelve female teachers explored concepts and knowledge of vocal abuse and vocal hygiene in a workshop session and attempted to practice vocal hygiene for 2 months. They showed significant voice improvement as assessed instrumentally in terms of three acoustic and electroglottographic parameters. There was no significant change of voice in a control group of 13 teachers. The results suggest that teachers were able to improve their

Efficacy of Voice Therapy in Improving Vocal Function in Adults Irradiated for Laryngeal Cancers: A Pilot Study.

BACKGROUND: Radiation therapy (XRT) for laryngeal cancers causes acute and chronic vocal dysfunction. Although these deleterious effects of XRT are well-established, there is a dearth of research with respect to effective voice rehabilitation following XRT for laryngeal cancers. OBJECTIVE: To obtain preliminary data on the efficacy of voice rehabilitation, using vocal function exercises (VFEs) in improving vocal function in adults irradiated for laryngeal cancer. The comparison treatment group (VH) received vocal hygiene counseling. STUDY DESIGN: Randomized clinical trial. METHODS: Particip

Voice source differences between registers in female musical theater singers.

Musical theater singing typically requires women to use two vocal registers. Our investigation considered voice source and subglottal pressure P(s) characteristics of the speech pressure signal recorded for a sequence of /pae/ syllables sung at constant pitch and decreasing vocal loudness in each register by seven female musical theater singers. Ten equally spaced P(s) values were selected, and the relationships between P(s) and several parameters were examined; closed-quotient (Q(closed)), peak-to-peak pulse amplitude (U(p-t-p)), amplitude of the negative peak of the differentiated flow glott

Influence of Messa di Voce speed on vocal stability of untrained, healthy subjects.

INTRODUCTION: Despite its importance in voice training, comprehensive research into sustained vowel phonation with constant pitch and increasing and decreasing loudness, the so-called Messa di Voce, is lacking. The study examines the laryngeal behavior during Messa di Voce, regarding the impact of the speed of execution on voice stability parameters. MATERIALS AND METHODS: Nine untrained, healthy subjects (5 female, 4 male) were asked to perform Messa di Voce exercises on the vowel [i:], involving a gradual increase and decrease of volume. During the first task, each phase should take 3 s, wh

[The measurement of voice quality and voice capacity to penetrate background noise and multidimensional voice program].

The paper presents a method of measuring the vocal penetrating capacity under conditions of masking voice. The noise consists of a frequency mixture of vowels in combination with voiced and unvoiced consonants. The comprehensibility of the test matrix is measured with respect to the capacity of the voice to penetrate background noise. The results of the measurement, which are based on more than 1,000 patients, show a significant correlation between the vocal inset phase of the voice status and the patient's subjective assessment of his ability to increase loudness of the duration of vocal use

The effectiveness of voice therapy for patients with non-organic dysphonia.

Forty-five patients diagnosed as having non-organic dysphonia were assigned in rotation to one of three groups. Patients in one group received no treatment and acted as a control group. Patients in the other two groups received a programme of either 'indirect' therapy or 'direct with indirect' therapy, respectively. A self-report questionnaire of vocal performance, observed ratings of voice quality, and computer-derived acoustic measurements (signal-to-noise ratio, pitch perturbation and amplitude perturbation) were carried out on all patients before and after treatment to evaluate the changes

Effectiveness of a mobile phone application in managing voice health for teachers: A randomized controlled trial.

OBJECTIVE: Teachers are at constant risk of straining their voices due to professional demands, making them more susceptible to voice disorders and therefore increased absenteeism. A widely accessible prevention method would be beneficial, leading to the development of the ReSt mobile application. The aim of this study was to evaluate its effectiveness. METHODS: A randomized controlled trial was conducted with 399 teachers, allocating 205 participants to the training group and 194 to the control group. Over a 46-day period, participants in the training group engaged with the ReSt app. Outcome

Voice gender and the segregation of competing talkers: Perceptual learning in cochlear implant simulations.

Two experiments explored the role of differences in voice gender in the recognition of speech masked by a competing talker in cochlear implant simulations. Experiment 1 confirmed that listeners with normal hearing receive little benefit from differences in voice gender between a target and masker sentence in four- and eight-channel simulations, consistent with previous findings that cochlear implants deliver an impoverished representation of the cues for voice gender. However, gender differences led to small but significant improvements in word recognition with 16 and 32 channels. Experiment 2

The Relationship Between Pitch Discrimination and Acoustic Voice Measures in a Cohort of Female Speakers.

BACKGROUND: Evidence across a range of musically trained, hearing disordered and voice disordered populations present conflicting results regarding the relationship between pitch discrimination (PD) and voice quality. PD characteristics of female speakers with and without a musical training background and no self-reported voice disorder, and the relationship between PD and voice quality in this particular population, have not been investigated. AIMS: To evaluate PD characteristics in a cohort of female participants without a self-reported voice disorder and the relationship between PD and aco

Immediate effects of a semi-occluded water-resistance ventilation mask on vocal outcomes in women with dysphonia.

INTRODUCTION: Semi-occluded vocal tract exercises (SOVTEs) are frequently used exercises in voice therapy. An important shortcoming to most SOVTEs is the inability to include continuous speech in these exercises. A variation of water-resistance therapy (WRT), during which a patient phonates through a resonance tube ending in water, was developed to include continuous speech: the semi-occluded water resistance ventilation mask (SOVM-WR). The current study investigated the immediate effects of this innovative technique on vocal outcomes of women with dysphonia. METHODS: A pretest-posttest rando

Voice Handicap Index Changes After Microflap Surgery for Benign Vocal Fold Lesions Are Not Associated With Recommended Absolute Voice Rest Duration.

PURPOSE: Voice rest is frequently prescribed after phonosurgery, but optimal type and duration for voice outcomes have not been demonstrated. Studies to date have been characterized by heterogeneity in surgical procedures and laryngeal diagnoses. We sought to analyze the effect of recommended absolute voice rest duration on outcomes of microflap surgery for benign vocal fold lesions. A secondary purpose was to identify patient factors associated with postoperative voice outcomes. METHOD: Forty-three patients were included in this retrospective review of patients aged 18 years and above who un

Semi-occluded vocal tract exercises in healthy young adults: Articulatory, acoustic, and aerodynamic measurements during phonation at threshold.

Semi-occluded vocal tract exercises (SOVTEs) are increasingly popular as therapeutic exercises for patients with voice disorders. This popularity is reflected in the growing research literature, investigating the scientific principles underlying SOVTEs and their practical efficacy. This study examines several acoustic, articulatory, and aerodynamic variables before, during, and after short-duration (15 s) SOVTEs with a narrow tube in air. Participants were 20 healthy young adults, and all variables were measured at threshold phonation levels. Acoustic variables were measured with a microphone
