A cautionary tale: Non-steroidal anti-inflammatory drug use and localized provoked vulvodynia.

Localized provoked vulvodynia (LPV) is characterized by chronic vulvar pain upon light touch to the vulvar vestibule, a specialized ring of tissue immediately surrounding the vaginal opening. LPV affects ∼14 million people in the US. Current treatments for LPV include nonsteroidal anti-inflammatory drugs (NSAIDs), but clinical evidence has demonstrated that they are ineffective for vulvar pain. Here, the effects of NSAID treatment on inflammation and resolution in an in vitro model of LPV were explored using enzyme-linked immunosorbent assays (ELISAs), calcium imaging, and metabololipidomics.

Localized provoked vulvodynia as an immune-mediated inflammatory disease: rationale for a new line of research.

Localized provoked vulvodynia (LPV), also called vulvar vestibulitis or provoked vestibulodynia, is a major cause of dyspareunia that severely impacts sexual health. At the tissue level, lymphocytic inflammation and hyperinnervation are characteristic pathological features, explaining the main symptoms and signs. A recent experimental animal study suggests that the histopathological findings of LPV may be due to mucosal CD4 Th17 immune responses to microbial antigens. We hypothesize that LPV is an immune-mediated inflammatory disease and challenge the concept of LPV as a chronic pain syndrome

Bladder agenesis and incomplete kidney duplication: Ileal reservoir with continent diversion as definitive treatment.

Bladder agenesis is an extremely rare entity. A 12-year-old female patient presented with urinary incontinence, recurrent urinary tract infections, visible vaginal introitus and urethra, and two holes at the vulvar vestibule. An investigation revealed bladder agenesis. Surgery confirmed the absence of bladder, and ileal reservoir in omega (Ω) was performed with continent diversion. At the 30-month follow-up, there was no complication in clean intermittent catheterization.

Under the hood: vulvar anatomy and pathology with a focus on MRI.

Vulvar anatomy and pathology can be a challenging subject to master, especially given the paucity of resources available on the subject. This review provides an overview of normal anatomy and imaging appearance of the vulva, including the mons pubis, labia majora, labia minora, clitoris, clitoral hood, external urethral meatus, vestibule and vaginal introitus, the Bartholin (greater vestibular) glands and the Skene (lesser vestibular or paraurethral) glands. We will also review the imaging appearance of various benign and malignant pathologies that affect these structures, including congenital

Recent advances in understanding provoked vestibulodynia.

Vulvodynia refers to pain in the vulva of at least 3 months' duration in the absence of a recognized underlying cause. Provoked, localized vestibulodynia is the term used to describe superficial pain confined to the vulvar vestibule, provoked by touch. This review will focus on provoked vestibulodynia with regard to its suggested causative factors and will discuss the role of inflammation, vulvovaginal infections, mucosal nerve fiber proliferation, hormonal associations, central pain mechanisms, pelvic floor muscle dysfunction, and genetic factors. Clinical observations, epidemiological studie

Urogenital sinus origin of mucinous and ciliated cysts of the vulva.

The origin of mucinous and ciliated cells in the vulva and the pathogenesis of cysts lined by these epithelia were investigated. Small mucinous glands numbering from one to more than a hundred were encountered in 9 of 19 vulvas (53%) that were consecutively examined at autopsy. Eleven other cysts were encountered clinically. Six were lined by mucinous epithelium, three by pseudostratified cells with cilia plus small foci of mucinous epithelium, and two by ciliated epithelium only. Four of the cysts with ciliated epithelium were otherwise typical Bartholin's gland cysts. It is concluded that gl

Evaluation of a device to facilitate female urethral catheterization.

Urethral catheterization is a skilled procedure that nurses in hospital settings perform routinely. The opening of the female urethra is located within the vulvar vestibule, making insertion of urinary catheters into females a greater technical challenge than in males. Researchers evaluated whether a new device might decrease the time required for catheter insertion, increase the likelihood of inserting the catheter on the first attempt (improved accuracy), and reduce patient discomfort. Comments about the device from both patients and nurses also are reported.

Pelvic floor muscle function in women with provoked vestibulodynia and asymptomatic controls.

INTRODUCTION AND HYPOTHESIS: The purpose of the present study was to assess vaginal resting pressure (VRP), pelvic floor muscle (PFM) strength and endurance, and surface EMG activity in women with and without provoked vestibulodynia (PVD). METHODS: This was an assessor-masked comparison study including 70 women. Exclusion criteria were any previous pregnancy and presence of candida. Sensitivity of the vulvar vestibule was rated at three sites with Q-tip pressure measurement and a numerical rating scale for pain. VRP and PFM strength and endurance were measured with a high precision pressure t

Chronic stress in women with localised provoked vulvodynia.

BACKGROUND: Localised provoked vulvodynia (former vulvar vestibulitis syndrome) is the most common cause of superficial dyspareunia among young women. In a clinical setting, it is obvious that many women with localised provoked vulvodynia show signs of chronic stress. The objective of this study was to compare chronic stress in a group of women with localised provoked vulvodynia, referred to the vulvar unit at the Division of Obstetrics and Gynecology, Danderyd Hospital, Stockholm, Sweden with control women of the same age and educational level, using two independent methods. METHODS: Forty-t

A causal role for right temporo-parietal junction in signaling moral conflict.

Chronic vulvar pain or discomfort for which no obvious aetiology can be found, i.e. vulvodynia, can affect up to 16% of women, and it may be found in girls and women across all age groups and ethnicities. Most patients describe it as burning, stinging, irritation, or rawness. The symptoms may spread to the whole vulva (generalised vulvodynia) or only to part of it, such as the clitoris (clitorodynia) or the vestibule of the vagina (vestibulodynia). This condition is often underreported and underrecognised by health care providers. Vulvodynia is a significant burden to society, the health care

Effectiveness of Combination of Tibolone and Lactobacilli Plus Lactoferrin in Postmenopausal Women with Vulvar Vestibular Pain: A Preliminary Report.

BACKGROUND: Postmenopausal dyspareunia and vulvar pain are common complaints, affecting about 60% of women within a few years after hormone levels begin to decline (such as estrogen and androgen). Atrophic changes mainly located in the vulvar vestibule and vulnerability to vulvovaginal infections in postmenopause could be predisposing factors to the development of vulvar burning/pain and introital dyspareunia (vestibulodynia secondary to atrophy). Tibolone is the most effective and safe alternative for treating menopausal symptoms. The role of Lactobacilli and lactoferrin shows its effectivene

Chronic pelvic pain and botulinum toxin.

Botulinum toxin is being explored as a treatment for chronic pelvic pain, a major cause of suffering and disability in both women and men worldwide. For chronic pelvic pain in women, botulinum toxin may be injected into pelvic floor muscles such as levator ani and obturator internus. For pain associated with genitopelvic penetration disorders (vaginismus, vestibulitis, and vulvar pain, bulbospongioussus and ischiocavernosus may be treated. There have been numerous uncontrolled studies of botulinum toxin for chronic pelvic pain in women showing benefit, however, the few randomized controlled cl

Vulvar vestibular papillomatosis.

Vulvar vestibular papillomatosis is considered an anatomical variant of the vulva. Recognition of this condition enables one to distinguish it from warts and therefore avoid unnecessary therapy. A 29-year-old lady presented to this clinic with a history of 'small growths' in her vulva since two years. Examination identified skin colored translucent papules; some of them appeared digitate and were seen on the vestibule and inner aspect of both labia minora. They were soft to feel and non-tender. Few lesions looked like elongated pearly penile papules. A provisional diagnosis of vestibular papil

Hymenal stenosis and fibrosis in two adult women.

BACKGROUND: Hymenal abnormalities are most commonly a result of incomplete apoptosis of the urogenital sinus during embryology. Infrequently, however, noncongenital abnormalities of the hymen can occur that can cause significant sequelae such as severe introital dyspareunia. CASES: We report on two adult women who developed severe introital dyspareunia secondary to hymenal stenosis and fibrosis in the absence of other vulvovaginal pathology. Neither woman had point tenderness of the vulvar vestibule, but their symptoms of searing pain on vaginal penetration was reproduced by stretching the hy

IgE-mediated allergy against human seminal plasma.

Human seminal plasma hypersensitivity has to be differentiated from allergic reactions to latex, spermicidal agents, local anesthetics or components of lubricants. The present review article discusses IgE-mediated allergic reactions (type I) to specific components of the seminal plasma. Such incidents are rare, even though there seems to be a considerable number of unreported cases. Since the first publication in 1958, human seminal plasma allergy has been increasingly recognized, and approximately 80 cases have been described. Most affected women are younger than 40 years, presenting with an

Inflammation-induced mast cell-derived nerve growth factor: a key player in chronic vulvar pain?

Provoked vulvodynia (PV) is characterized by localized chronic vulvar pain. It is associated with a history of recurrent inflammation, mast cell (MC) accumulation and neuronal sprouting in the vulva. However, the mechanism of how vulvar-inflammation promotes neuronal sprouting and gene-expression adaptation in the spinal cord, leading to hypersensitivity and painful sensations, is unknown. Here, we found that vulvar tissue from women with PV (n = 8) is characterized by MC accumulation and neuronal sprouting compared to women without PV (n = 4). In addition, we observed these changes in an anim
