Adenoid Cystic Carcinoma of Bartholin's Gland: Case Report.

Primary adenoid cystic carcinoma (ACC) of Bartholin's gland is a rare gynecologic malignancy. We report a case of locally advanced ACC of Bartholin's gland. A 62-year-old presented with left Bartholin's gland carcinoma and underwent left radical vulvectomy, left-sided inguinal-femoral lymph node dissection, posterior pelvic exenteration, and pedicle abdominal muscle flap. On her 3 months follow-up exam she was disease free.Pelvic exenteration for thetreatment of this rare disease in the vulva is a potential curative option.

The prognostic significance of micrometastases in node-negative squamous cell carcinoma of the vulva.

Nodal involvement is one of the most significant prognostic factors in squamous cell carcinoma (SCC) of the vulva. We conducted a retrospective analysis of 31 women with histologically node-negative SCC from a population-based cohort of Grampian women. Median follow-up was 42 months after radical vulvectomy with groin node dissection. In total, 13 women (42%) were found to have micrometastases on immunohistochemistry. The risk of recurrence was almost 20-fold higher in those with micrometastases compared to those without (hazard ratio=19.6 (95% CI 2.3-171).

Epithelial-myoepithelial carcinoma with high-grade transformation of bartholin gland - A case report and literature review.

OBJECTIVE: This case report and literature review aims to highlight the diagnostic challenges and treatment of epithelial-myoepithelial carcinoma (EMC) with high-grade transformation (HGT) of the Bartholin gland, a rare and aggressive tumor. CASE REPORT: A 51-year-old perimenopausal Indonesian woman with a 20-year history of untreated left Bartholin cyst infection presented with a painful vulvar mass and vulvar bleeding. Imaging revealed a 5 cm lobulated mass in the left vulva, and a biopsy confirmed poorly differentiated carcinoma. Immunohistochemistry demonstrated features consistent with e

Presentation of a patient with in situ amelanotic melanoma of the vulva.

Amelanotic malignant melanoma of the vulva is extremely rare. The authors describe here a case of amelanotic malignant melanoma of the vulva, occurring in a 71-year-old woman without any clinical symptoms. The woman had a small nodular lesion in the left labia majora. Local excision was performed. Histological examination revealed an in situ malignant melanoma without any evidence of invasive disease. All suspicious lesions in the vulva region, even if there are no clinical symptoms, should be biopsied, and if an in-situ melanoma is identified, partial or total vulvectomy should be considered.

Extramammary Paget's disease of the vulva.

Vulvar Paget's disease is an extremely rare neoplasm that accounts for less than 1% of the vulvar malignancies. We present a case of a 75-year-old woman, who had an eczematoid lesion involving the labia majora and minora bilaterally, with infiltration to the clitoris. Enlarged non-fixed lymph nodes were palpable in the inguinal region bilaterally. A biopsy of the vulva showed Paget's disease. She underwent radical vulvectomy with bilateral inguinal lymph node dissection. The patient remained disease free at 6-month follow-up.

[Carcinoma of the vulva in women: clinical and morphopathological problems].

Surgery is the mainstay of the treatment of vulvar cancer. Radical vulvectomy together with some form of lymphadenectomy is the usual treatment today, but the desire to minimize morbidity have led to a selective decrease in the extent of the surgery in recent years. We recommended retroperitoneal pelvic lymphadenectomy be used only if the inguinal nodes were positive. The approach has become widely accepted. Improvements in perioperative care and anesthetic techniques have allowed the surgical approach to be used in most patients, even in older women seen so commonly with this cancer.

[Is involvement of the clitoris in epidermoid carcinoma of the vulva an adverse prognostic sign?].

Retrospective evaluation of 360 patients with vulvar cancer during the period from 1975 to 1994. From this group in 215 patients radical vulvectomy with bilateral inguinal lymph node extirpation was performed. After excluding 35 patients in the IVth stage of the disease, 11 with vulvar melanoma and 9 who did not attend in the group of the remaining 160 cases 3- 5- and 10-year survival rates were evaluated. Forty of the cancers clitoris of these 160 cases do not confirm that this localisation is a bad prognostic factor.

Malignant transformation of condyloma acuminatum: report of a case.

Condyloma acuminatum is a benign warty lesion, whose malignant change is rarely seen. A 69-year-old woman with a large vulvar condyloma acuminatum, which she had had for about 14 years, underwent a vulvectomy, and a malignant component to the lesion was found unexpectedly. The continuity from a benign to a malignant lesion was evident in the histopathologic examination. Infection with human papillomavirus type 6 was documented by DNA hybridization. The use of a one-step reverse dot blot hybridization with nonisotope DNA probe in this case was convenient and is recommended.

The Keystone Flap for Vulvar Reconstruction: A Case Series and BODY-Q Evaluation.

BACKGROUND: Vulvar carcinoma is a rare cancer that mainly affects women older than 60 years of age. The most common form is squamous cell carcinoma. The main treatment is vulvectomy, followed by reconstruction. Among reconstructive techniques, the keystone perforator island flap (KPIF) is a relatively novel solution, offering robust vascularity, minimal complications, and rapid recovery. This study analyzed the use of KPIF on 14 patients after vulvectomy, evaluating the aesthetic and functional results through the BODY-Q questionnaire, which has never been applied to evaluate this reconstructi

Insights from plastic and reconstructive surgery wound specialists in managing complex vaginal and vulvar toxicities following radiation therapy.

OBJECTIVE: Radiation therapy is essential for treating vaginal and vulvar cancers but can cause chronic pain, sexual dysfunction, infections, and delayed wound healing. These complications profoundly impact survivors' quality of life and may impede cancer-directed treatment in recurrent disease. We describe a multidisciplinary approach involving Plastic and Reconstructive Surgery, Gynecologic Oncology, and Radiation Oncology for managing complex radiation-induced wounds. METHODS: We retrospectively reviewed two patients with severe vaginal/vulvar radiation wounds treated at a tertiary wound c

[A Case of Robot-Assisted Total Pelvic Exenteration for Locally Advanced Rectal Cancer with Vaginal Invasion].

A 66-year-old woman presented with discharge of necrotic tissue and bleeding from the vagina during uterine cancer screening. She was diagnosed with lower rectal cancer cT4b(vagina)N3M0, cStage Ⅲc. As the tumor protruded into the lumen from the posterior vaginal wall, preservation of the anterior vaginal wall was challenging. Robot-assisted total pelvic exenteration with vulvectomy was performed. The patient was discharged without any apparent postoperative complications. Upon histopathological examination, the lesion was identified as being pT4bN0M0, pStage Ⅱc. Six months after the surgery, n

Surgical management of stage I and II vulvar cancer: the role of the sentinel node biopsy. Review of literature.

Recognition of the psychosexual consequences of radical vulvectomy and better understanding of the lymphatic drainage and histopathologic features of vulvar cancer have led to a more conservative surgical approach, especially in patients with early-stage disease. Every patient with early vulvar cancer should be managed individually and the risk of conservative therapy balanced against the dangers and advantages of more radical therapy. The results of the sentinel node (SN) procedure in early cancer of the vulva are encouraging, and it might be possible in the near future to avoid the morbidity

[Primary carcinoma of Bartholin's glands with HPV 18 detection].

A primary carcinoma of the Bartholin's gland is a rarely type of vulva-carcinoma. The patients median age are 55 years and the etiology of the carcinoma are still unknown. Viral influences are discussed. Most frequent types of carcinomas of Bartholin's gland are those of squamous cells and adenocarcinomas. This article reports on a 55 year old patient with a primary carcinoma of Bartholin's gland. The human papillomavirus 18 was detected by a polymerase chain reaction. The correct diagnosis was delayed because of an initial misinterpretation of the carcinoma as a Bartholin cyst, marsupializati

Co-existence of extramammary Paget's disease and Bowen's disease of vulva.

Extramammary Paget's disease and Bowen's disease are histologically similar and immunohistochemistry is often required to make the diagnosis. We present a case of vulval Paget's disease with Bowen's disease in an elderly female. Strong positivity for cytokeratin 7, anti CAM 5.2, carcinoembryonic antigen (CEA) and periodic acid-Schiff (PAS) stain in clitoral, left labial and interface regions of the vulvectomy specimen confirmed the diagnosis of Paget's disease (PD) while positive staining for p63 in the right labial and interface regions helped in establishing the diagnosis of concurrent Bowen

Persistence of injectable collagen in human urethra: case report.

A polypoid lesion was found near the bladder neck during cystoscopy in a woman with urinary incontinence who had undergone periurethral collagen (Contigen) injections 3 years before. She had previously received radiation therapy in addition to a radical vulvectomy for vaginal cancer. On transurethral resection of the lesion, particles of unresorbed collagen material extruded from the capsulated suburothelial space. Histologic evaluation verified the material as the foreign collagen. The persistence of glutaraldehyde cross-linked collagen in our patient was much longer than previously reported

Severe vaginal infection with Entamoeba histolytica in a woman who recently returned from Mexico: case report and review.

Genital amebiasis is a rare complication of infection with Entamoeba histolytica, even in areas where the pathogen is endemic. We describe a patient who apparently contracted intestinal amebiasis on a trip to Mexico and who presented with ulcerative vulvovaginitis 2 months later. Her condition rapidly progressed to severe necrotizing vulvovaginitis that required a radical vulvectomy. Histopathologic examination of the surgical specimen revealed the presence of E. histolytica trophozoites. The patient recovered after surgery and antiamebic therapy. We review the epidemiology and pathogenesis of

[Radiotherapy of vulvar cancer. Treatment results of electron therapy in 446 patients 1956 to 1978].

446 patients with malignant tumour of the vulva were treated from 1956 to 1978. In 432 patients with invasive squamous cell carcinoma, a five-year survival rate of 43.1% was obtained after exclusive exposure to electron radiation. Severe complications were seen in 10.9% of the treated patients. On comparing the results with those obtained after electrocoagulation or vulvectomy, it was seen that a therapy based on electron radiation only yielded more favourable results, especially in cases with advanced stage carcinomas. In younger women with early tumour stages, surgery should be given prefere
