Musculoskeletal responses to spaceflight: mechanisms, countermeasures, and key gaps.

Spaceflight and partial-gravity environments impose profound challenges to the human musculoskeletal system, driving rapid muscle atrophy, progressive bone loss, and tendon maladaptation. At the molecular level, unloading suppresses anabolic signaling, enhances proteolysis, and induces mitochondrial stress, whereas bone and tendon exhibit reduced extracellular matrix turnover and impaired mechanotransduction. Recent space-omics and cross-species studies, including rodent and Caenorhabditis elegans models, reveal that these catabolic responses are evolutionarily conserved and involve systemic p

Simulated microgravity alters sperm navigation, fertilization and embryo development in mammals.

As human space exploration extends toward long-duration missions and off-Earth settlements, understanding the impact of microgravity on reproduction is vital for sustaining life beyond Earth. This study investigates how simulated microgravity influences sperm navigation, fertilization, and subsequent early embryo development across three mammalian species; human, mouse, and pig, using a dual-axis 3D clinostat and clinically relevant in vitro culture systems. Sperm function was assessed via microchannels, revealing that microgravity impaired directional navigation and fertilization capacity in

Russian Countermeasure Systems for Adverse Effects of Microgravity on Long-Duration ISS Flights.

The system of countermeasures for the adverse effects of microgravity developed in the USSR supported the successful implementation of long-duration spaceflight (LDS) programs on the Salyut and Mir orbital stations and was subsequently adapted for flights on the International Space Station (ISS). From 2000 through 2010, crews completed 26 ISS flight increments ranging in duration from 140 to 216 d, with the participation of 27 Russian cosmonauts. These flights have made it possible to more precisely determine a crew-member's level of conditioning, better assess the advantages and disadvantages

Simulation of space adaptation syndrome on earth.

The three Spacelab D-1 Scientist Astronauts were exposed to a 1 1/2 h 3 g centrifuge run in the supine position, resulting in a linear 3 g acceleration in the x-direction. They used their space experience to evaluate their readapting to normal gravity and compared their observations with 'Space Adaptation Syndrome'. After the centrifuge runs, the vestibular visual system appeared to be modified in a very specific and reproducible manner. Readapting to the normal 1 g environment took at least 6 h. During this period there was a striking similarity to the astronaut's experience during adaptation

The missing discipline in space health operations: Nursing as mission assurance.

Exploration-class missions and expanding civilian spaceflight increase the need for reliable health protection under constraint-limited rescue bandwidth, dependence on devices and protocols, and high consequence for small failures. Spaceflight stressors act as a bundled ecology (microgravity, radiation, circadian disruption, and isolation/confinement) producing integrated, aging-like vulnerability across immune, sleep-cognitive, and deconditioning domains. Yet discussions of space health system design often treat nursing as downstream implementation rather than a safety-critical discipline res

Pharmacology in space. Part 1. Influence of adaptive changes on pharmacokinetics.

The topic of pharmacology in space, i.e. the administration of drugs during space flight and the subsequent pharmacokinetic handling of the pharmaceuticals, is a new field about which little is known. In a two-part series, Claire Lathers and colleagues highlight some of the current questions in this field. In this first article the physiological and biochemical changes associated with weightlessness in space are discussed. These changes induce adaptive alterations which may influence the pharmacokinetic properties of drugs. The cardiovascular system is of particular relevance here. Also discus

Ambient Temperature Preservation Supports Recovery of Beating Capacity in Human Cardiac Organoids After Exposure to Simulated Space Stressors.

Human space exploration exposes astronauts to environmental stressors including microgravity (µg) and ionizing radiation, which may adversely affect cardiovascular health. Human induced pluripotent stem cell (hiPSC)-derived cardiac organoids, also referred to as cardioids, provide a physiologically relevant model to investigate these effects, but preservation of complex 3D tissues remains challenging. Current methods rely mainly on cryopreservation, creating variability and logistical constraints, particularly for spaceflight research. Alternative preservation strategies suitable for both grou

Artificial Intelligence Interpretation of Point-of-Care Lung Ultrasound in Microgravity.

INTRODUCTION: Long-duration space missions demand reliable, portable, and autonomous medical diagnostic tools. Lung ultrasound (LUS) is ideal for space exploration due to its safety and versatility, but interpreting LUS images requires training. Artificial intelligence (AI) can assist with image interpretation by detecting lung sliding, an ultrasound sign produced by the movement of lung and chest wall during breathing. The presence of lung sliding helps to rule out pneumothorax, or a collapsed lung, which is a condition that may arise from barotrauma or rapid changes in pressurization. METHO

Two-photon polymerization of miniaturized 3D scaffolds optimized for studies on glioblastoma multiforme in spaceflight-like microgravity conditions.

The obtainment of innovative models recalling complex tumour architectures and activitiesin vitrois a challenging drive in the understanding of pathology molecular bases, yet it is a crucial path to the identification of targets for advanced oncotherapy. Cell environment recapitulation by 3D scaffolding and gravitational unloading of cell cultures represent powerful means in tumour biomimicry processes, but their simultaneous adoption has consistently been explored only in the latest decade. Here, an unprecedented bioengineering approach capitalizing on spaceflight biology practice is proposed

Microgravity Exacerbates Periodontitis In Vivo.

This is the first study to examine induced periodontitis under simulated microgravity in vivo. Microgravity significantly increased alveolar bone loss, elevated GCF alkaline phosphatase levels, and caused severe periodontal destruction with notable immune cell infiltration. Understanding the underlying molecular pathways may guide future treatments for periodontitis in space and on Earth.

Molecular Mechanism of Microgravity-Induced Intestinal Flora Dysbiosis on the Abnormalities of Liver and Brain Metabolism.

Space flight has many adverse effects on the physiological functions of astronauts. Certain similarities have been observed in some physiological processes of rodents and astronauts in space, although there are also differences. These similarities make rodents helpful models for initial investigations into space-induced physiological changes. This study uses a 3D-Clinostat to simulate microgravity and explores the role of microgravity in space flight-induced liver and brain abnormalities by comparing changes in the gut microbiota, serum metabolites, and the function and physiological biochemis

Sperm Motility of Mice under Simulated Microgravity and Hypergravity.

For deep space exploration, reproductive health must be maintained to preserve the species. However, the mechanisms underlying the effect of changes in gravity on male germ cells remain poorly understood. The aim of this study was to determine the effect of simulated micro- and hypergravity on mouse sperm motility and the mechanisms of this change. For 1, 3 and 6 h, mouse sperm samples isolated from the caudal epididymis were subjected to simulated microgravity using a random position machine and 2g hypergravity using a centrifuge. The experimental samples were compared with static and dynamic

Long-duration bed rest as an analog to microgravity.

Long-duration bed rest is widely employed to simulate the effects of microgravity on various physiological systems, especially for studies of bone, muscle, and the cardiovascular system. This microgravity analog is also extensively used to develop and test countermeasures to microgravity-altered adaptations to Earth gravity. Initial investigations of bone loss used horizontal bed rest with the view that this model represented the closest approximation to inactivity and minimization of hydrostatic effects, but all Earth-based analogs must contend with the constant force of gravity by adjustment

Systematic review of countermeasures to minimise physiological changes and risk of injury to the lumbopelvic area following long-term microgravity.

BACKGROUND: No studies have been published on an astronaut population to assess the effectiveness of countermeasures for limiting physiological changes in the lumbopelvic region caused by microgravity exposure during spaceflight. However, several studies in this area have been done using spaceflight simulation via bed-rest. The purpose of this systematic review was to evaluate the effectiveness of countermeasures designed to limit physiological changes to the lumbopelvic region caused by spaceflight simulation by means of bed-rest. METHODS: Electronic databases were searched from the start of

Adapting to artificial gravity (AG) at high rotational speeds.

Short-radius centrifugation offers a promising countermeasure to the adverse effects of prolonged weightlessness. Head movements made in a rotating environment elicit Coriolis effects, which seriously compromise sensory and motor processes. We have previously found that, contrary to common belief, participants can adapt to the Coriolis effects associated with single-quadrant yaw head turns during 23-rpm short-radius centrifugation, while maintaining their adaptation to stationary environments. Here, we focus on motion sickness and illusory motion, the most problematic subjective side effects.

Biopsychosocial Health Considerations for Astronauts in Long-Duration Spaceflight: A Narrative Review.

Long-duration spaceflights beyond low-Earth orbit, including missions to the Moon and Mars, pose significant health risks. Although biomedical approaches commonly appear in the literature, considering psychological and social factors alongside physiologic health offers a more holistic approach to astronaut care. Integrating the biopsychosocial (BPS) framework into medical planning addresses complex spaceflight challenges and aids in developing mitigation strategies. This review examined health risks associated with long-duration spaceflight within a BPS framework. Sources included governmental

Space motion sickness.

Space motion sickness, presumably triggered by sudden entry into a weightless environment, occurred with unexpected frequency and severity among astronauts who flew the Skylab missions. Recovery from symptoms was complete within 3-5 days, and as revealed by the Skylab M131 Human Vestibular Function Experiment, all crewmembers were immune to experimentally induced motion sickness after mission day 8. This syndrome has been recognized as a possible threat to the early mission well-being and operational efficiency of at least some individuals who will fly space missions in the future. The causes

Investigating microgravity's role in morphological changes, growth behavior, DNA damage, and cytotoxic responses of AMJ13 breast cancer cells.

Breast cancer is one of the most common types of cancer in women, and its treatment depends on the type and severity of the disease. Traditional treatments lead to side effects that require searching for alternative treatment methods. Microgravity is one such method that is currently being studied. In summary, exposure of cancer cells to microgravity dramatically changes morphological, biological, and genetic functions and increases the apoptosis rate. In this manuscript, a comprehensive study is conducted on the effects of microgravity on the breast cancer cell line (AMJ13) in terms of extern

Three-week dry immersion results in a more severe impairment of cardiac baroreflex than head-down bed rest of the same duration.

Ground-based models of microgravity, head-down bed rest (HDBR) and dry immersion (DI), may differ in their effects on cardiovascular control, but this has not been directly tested yet during similarly long exposure. This study aimed at comparison of head-up tilt (HUT) induced changes in cardiac baroreflex functioning during 3-week long HDBR and DI. HUT test (60°) was performed in two groups of healthy young men before and then on 7th, 14th and 19th days of HDBR or DI. Fifteen-min recordings of systolic arterial pressure (SAP) and RR-interval (RRI) were used to calculate their spectral powers,
