As clinicians, we sometimes fail to look after ourselves properly and do not regularly eat healthy foods or drink enough. Sleep is another factor that we often neglect. A lack of it can compromise our personal health and performance at work, and the "sleep debt" that results when this is chronic can take far longer to recover from than one might think. Now that junior doctors work more shift rotas and senior colleagues have onerous on-call responsibilities, we all need to be aware of the effects of sleep deprivation, which can lower the mood and motivation, weaken leadership, and result in mor

BACKGROUND: There is inconsistent evidence as to whether work schedule (including rotating shifts and night work) can affect reproductive outcomes.
METHODS: We investigated the association between work schedule and risk of spontaneous abortion in U.S. nurses. The Nurses' Health Study II is a prospective cohort study established in 1989. In 2001, information about occupational activities and exposures during pregnancy was collected from female nurses for the most recent pregnancy since 1993. Of 11,178 eligible respondents, 9547 (85%) indicated willingness to participate in the occupational stu

Melatonin has a diverse range of physiological effects in humans. Reported effects include modulation of the sleep-wake, thermoregulatory, cognitive, cardiovascular and immune systems. While integrating these broad-ranging effects is difficult when current paradigms are used, the diverse effects of melatonin on human physiology may be better understood by shifting our theoretical perspective. Traditionally, research has treated melatonin as a classical hormone for which a defined effect in physiological systems and a mechanism of action can be elucidated. In this article, we suggest that it ma

To elucidate sleep disturbances in the etiology of peptic ulcers, the prevalence of peptic ulcer disease was compared among shift workers and daytime workers. The subjects (N = 11,657) were employees of various institutions, such as factories, banks, or schools, and had undergone a mass x-ray examination of the upper gastrointestinal tract. With present shift workers (N = 2269), the prevalence of gastric ulcers was 2.38% (N = 54) and that of duodenal ulcers, 1.37% (N = 31). With the past shift workers (N = 2111), the prevalence of gastric ulcers was 1.52% (N = 32) and that of duodenal ulcers,

BACKGROUND: In Turkey, the path to specialisation is a highly demanding and costly process. After completing six years of intensive medical education, general practitioners must pass the highly competitive Medical Specialisation Examination (TUS) to enter residency training. To pass the examination for entry into specialist training, medical students in Turkey are compelled to attend highly expensive preparatory courses during the final two years of their six-year medical education. Additionally, graduates working as general practitioners often struggle to prepare for the specialist examinatio

BACKGROUND: Evidence for the carcinogenicity of shift work in humans is limited because of significant heterogeneity of the results, thus more in-depth research in needed. The Nightingale Study is a nationwide prospective cohort study on occupational exposures and risks of chronic diseases among female nurses and focuses on the potential association between shift work and risk of breast cancer. The study design, methods, and baseline characteristics of the cohort are described.

Our study shows that urological health of men working in shifts in the oil fields of the Tyumen region is worse than mean national and regional characteristics. Newly diagnosed chronic bacterial prostatitis was prevalent among urogenital diseases. This situation can be explained by professional working and rest conditions for workers in the oil fields engaged in turn, by examination of the urologist participating in the health check-up team. Males with diagnosed prostatitis received treatment with the drug vitaprost plus produced in Russia with a good result.

Excessive daytime sleepiness is one of the most common sleep-related patient symptoms, and it affects an estimated 20 percent of the population. Persons with excessive daytime sleepiness are at risk of motor vehicle and work-related incidents, and have poorer health than comparable adults. The most common causes of excessive daytime sleepiness are sleep deprivation, obstructive sleep apnea, and sedating medications. Other potential causes of excessive daytime sleepiness include certain medical and psychiatric conditions and sleep disorders, such as narcolepsy. Obstructive sleep apnea is a part

We investigated the relationship between shift work and excessive daytime sleepiness (EDS) among participants in the Japan Nurses' Health Study (JNHS). Responses of 9,728 female nurses to the 6th follow-up questionnaire were cross-sectionally analyzed. EDS was defined as an Epworth Sleepiness Scale score ≥11. EDS-associated factors were evaluated using Poisson regression analysis after adjustment for multiple confounders. Of the participants (mean age, 52.2 ± 8.0 yr), 28.7% were engaged in shift work, and the overall prevalence of EDS was 24.6%. EDS-associated factors were investigated separat

This cross-sectional study examined the association between shift work and thyroid function using data from the National Health and Nutrition Examination Survey 2007 to 2010. Weighted univariate and multivariate regression models were applied, with additional subgroup and interaction analyses performed. A total of 3578 participants were included (mean age 41.17 ± 13.31 years). After adjusting for potential confounders, shift workers exhibited significantly higher levels of thyroid-stimulating hormone (TSH) (β = 0.30; 95% CI: 0.14-0.45), thyroglobulin antibody (TgAb) (β = 11.62; 95% CI: 3.49-19

OBJECTIVE: Stability in the timing of key daily routine behaviors such as working/doing housework, sleeping, eating, and engaging in social interactions (i.e., behavioral-social rhythms) contributes to health. This study examined whether behavioral-social rhythms were associated with cardiovascular disease (CVD) risk factors in retired night shift workers and retired day workers and explored whether past night shift work exposure moderated this association.
METHODS: A total of 154 retired older adults participated in this study. Multiple logistic regression models were used to examine associa

This study examined different fluid replacement quantities during intermittent work while wearing firefighting protective clothing and self-contained breathing apparatus in the heat (35 degrees C, 50% relative humidity). Twelve firefighters walked at 4.5 km per h with 0% elevation on an intermittent work (50 min) and rest (30 min) schedule until they reached a rectal temperature of 39.5 degrees C during work periods and 40.0 degrees C during rest, heart rates of 95% of maximum and/or exhaustion. During the heat-stress trials subjects received one of four fluid replacement quantities, high (H),

BACKGROUND: Public housing employees work in challenging conditions with many stressors. A Colorado housing authority requested an evaluation of its voluntary reduced work schedule pilot program and its association with employee wellbeing. Previous research has shown promising short-term improvements in employee productivity and morale from reduced schedule programs, but researchers have questioned whether impacts on wellbeing could be sustained. The study aimed to evaluate if program participants and non-participants reported differences in wellbeing measures over 18 months post-implementatio

As sleep restriction has negative effects on performance, ensuring sufficient sleep for shift workers is essential. Quick returns (< 11 h off between shifts) shorten sleep and are associated with increased fatigue and risk of accidents, but there is limited research on other aspects of cognitive performance and work performance. The aim of the present quasi-experimental field study was to investigate the effects of quick returns on objective and subjective measures of sleep, fatigue and cognitive performance. In total 36 newly graduated nurses were followed during two pre-scheduled work period

The lack of sleep is a significant problem in the modern world. The structure of the economy means that 24 hour working is required from some of us, sometimes because we are expected to be able to respond to share-price fluctuations on the other side of the planet, sometimes because we are expected to serve kebabs to people leaving nightclubs, and sometimes because lives depend on it. The immediate effect is that we feel groggy; but there may be much more sinister long-term effects of persistent sleep deprivation and disruption, the evidence for which is significant, and worth taking seriously

The restriction of the resident physician work week to 80 hours has had dramatic affects on resident education and life-style. While effects on mood, psychological distress, and burn-out have been studied, the resultant changes in tangible quality of life have received little attention. Birth rate was considered a measurable, relevant outcome. The resident marital and parental status by duty month was collected from a single orthopaedic surgical residency program for the four academic years preceding and following the implementation of the 80-hour work week. The number of births to residents d

Since 1. 1. 96, new regulations of working hours have been introduced in our hospital with regard to emergency service and compensation through leisure time. The average payment was reduced by 23% for surgeons on call. By this means, three additional surgeons could be employed. For every resident in training there is now less time for his residency than before. Sufficient surgical residency requires enough time on the ward and in the operating room. Thus, other conditions for residency programs are necessary to avoid unfulfilled tasks being pushed into the pre-existing area of non-documented w

Shiftworkers working on the night shift transpose their sleeping hours to the time of day which is inconvenient for sleep because of the endogenous circadian rhythm sleep wakefulness, the circadian rhythms of other body functions and the environmental schedule. As a result of this, a certain number of workers suffer from insomnia. This study aimed at examining the effects of two hypnotic agents with different elimination times on sleep disturbances which follow night shift work. Three groups of shiftworkers took zopiclone, nitrazepam and placebo capsules during a week of work on the night shif

Night work disrupts the circadian system, alters melatonin signaling, and is associated with sleep and mental health disturbances. The pineal gland is central to circadian regulation. However, structural neuroimaging evidence of pineal adaptation to chronic circadian disruption in humans remains limited. This cross-sectional neuroimaging study included 110 healthcare workers, 32 night workers and 78 day workers. High-resolution T1- and T2-weighted magnetic resonance imaging was used to quantify total pineal gland volume, parenchymal volume, and cyst prevalence. Circadian-related sleep disturba
