A 52-year-old female complained of lumbago and weakness in the lower extremities 6 days after craniotomy for clipping an aneurysm. Neurological examination revealed symptoms consistent with lumbosacral cauda equina compression. The symptoms affecting the lower extremities spontaneously disappeared within 3 days. Magnetic resonance (MR) imaging 10 days after the operation demonstrated a lumbar spinal subdural hematoma (SSH). She had no risk factor for bleeding at this site, the symptoms appeared after she began to walk, and MR imaging suggested the SSH was subacute. Therefore, the SSH was proba

A 48-year-old woman presented with sudden left hemiplegia with headache, which deteriorated two days later. CT scan showed repeated intratumoral and subdural hemorrhages. Magnetic resonance imaging showed a parasagittal tumor infiltrating into the superior sagittal sinus, with intratumoral hemorrhage and acute subdural hematoma in the interhemispheric fissure. The intratumoral hematoma had several different intensities, which indicated repeated hemorrhages. The subdural hematoma and the tumor were removed via frontoparietal craniotomy. The histological diagnosis was fibrous-type meningioma wit

This is a report of a case of a subdural haematoma with resulting paraplegia after attempted spinal anaesthesia. Epidural and subdural haematomas are rare complications after central neural blockade. The complication described here was the result of an unsuccessful attempt to puncture the spinal channel. The patient was a 72-year-old woman with a fracture of the left femoral neck, which it was intended to stabilize operatively. Findings that made lumbar spinal puncture difficult were severe overweight, and lordosis and scoliosis of the lumbar spine resulting from degenerative changes. Spinal a

Traumatic acute subdural hematoma remains one of the most lethal of all head injuries. Since 1981, it has been strongly held that the critical factor in overall outcome from acute subdural hematoma is timing of operative intervention for clot removal; those operated on within 4 hours of injury may have mortality rates as low as 30% with functional survival rates as high as 65%. Data were reviewed for 1150 severely head-injured patients (Glasgow Coma Scale (GCS) scores 3 to 7) treated at a Level 1 trauma center between 1982 and 1987; 101 of these patients had acute subdural hematoma. Standard t

The article is devoted to the difficulties of diagnosing a combination of delirium tremens with acute subdural (epidural) traumatic hematoma. The possibility of such a combination is known from the literature and is confirmed by the author's own observation of 3 cases. Hallucinational excitement of delirium tremens not infrequently masks the symptomatology of subdural hematoma in its latent period. This symptomatology is best expressed in the pre- and post-delirium states. The above pathological state should be differentiated from febrile delirium tremens, alcoholic encephalopathy and a combin

Traumatic acute arterial subdural hematomas are rare and are associated with a high mortality rate. The authors report a case in which the initial computed tomographic (CT) scan examination failed to reveal a hematoma. Within the subsequent 4 hours, while recovering from anesthesia given so that hemostasis of bleeding soft tissue injuries could be obtained, the patient suddenly developed bilaterally dilated pupils, both nonreactive to light. A repeat CT scan then revealed large subdural and intracerebral hematomas. The case is discussed and the pertinent literature is reviewed.

A case presenting with headaches, organic dysfunction with visual hallucinations, and incontinence of urine, all of two days' duration is described here. EEG obtained on the fourth hospital day showed right-sided PLEDS and on the fifth hospital day a generalized seizure occurred. The other clinical and laboratory data led to a diagnosis of bilateral subdural hematomas. The patient recovered following surgical treatment. No etiologic factor other than the subdural hematomas was found to explain the PLEDS. Possible mechanisms of pathophysiology are discussed.

BACKGROUND AND OBJECTIVES: Chronic subdural hematoma (CSDH) is one of the most common pathologies in our daily practice. The standard treatment is the evacuation making a burr-hole and placement of a subdural drainage, which has shown to decrease its recurrence. However, this procedure can entail risks such as parenchymal damage, infection, or the onset of seizures, prompting the consideration of subgaleal drainage as an alternative. Our objective is to compare the use of subdural and subgaleal drainage in a cohort of patients undergoing intervention for CSDH, as well as to analyze the differe

A 76-year-old female was admitted with a headache. She had no febrile course before admission. Computed tomography (CT) demonstrated bilateral frontal hypodense areas without enhanced rim. She was made a diagnosis of a chronic subdural hematoma. Because her general condition was poor and she had no neurological deficit, she was planned to be treated conservatively. On a few days after the admission, she was suffered from agranulocytosis. However, leukopenia disappeared within a few days by the effective treatment and any neurological deficit was not observed in these period. On thirty days aft

OBJECTIVE: The primary aim of this research is to investigate the predictive value of subdural effusion thickness in determining the progression of post-traumatic subdural effusion to chronic subdural hematoma. Studying this progression is crucial as it helps in early diagnosis and effective management of chronic subdural hematoma, which is a serious and life-threatening condition. This research is valuable and relevant for improving patient outcomes and reducing the associated risks and complications.
METHODS: We conducted a retrospective examination of the clinical data obtained from 124 pa

BACKGROUND: We present the rare case of a spontaneous intracranial subdural empyema caused by Helicobacter cinaedi in a preexisting chronic subdural hematoma (CSDH).
CASE DESCRIPTION: A 72-year-old man with a history of the right CSDH that remained radiologically unchanged for the past 2 years with conservative management was transferred to our hospital because of fever and convulsive seizure. Systemic sources of infection were not identified. Fever and extremely high levels of serum C-reactive protein (CRP) spontaneously improved without antibacterial therapy. One month after the fever disap

Nervous system involvement is uncommon in granulomatosis with polyangiitis (GPA), a systemic autoimmune disease with episodes of necrotizing vasculitis. It is usually due to the compressive effect of dural or epidural masses. Spinal hemorrhagic presentation is exceptional. A 41-year-old woman diagnosed with GPA presented with three episodes of acute spinal subdural hematoma separated by eight years and ten months, respectively. The symptomatic debut was pain and paresis in all episodes. On all occasions, a lesion compatible with acute spinal subdural hematoma was diagnosed by magnetic resonanc

OBJECTIVES: Chronic subdural hematoma (cSDH) frequently presents with headache, but the relationship between hematoma size and headache is not well defined. This study aims to determine the relationship between total hematoma size and headache presentation in cSDH patients while identifying a critical size threshold for headache development.
METHODS: We retrospectively reviewed adult cSDH patients diagnosed at our institution from January 1st, 2023, to June 4th, 2025. Patients with a Glasgow Coma Score (GCS) below 14 or a history of ventricular shunt were excluded. Data collected included dem

Acute idiopathic (spontaneous) spinal subdural hematoma (spinal subdural hematomas is a rare neurosurgical emergency, often linked to coagulation disorders, trauma, iatrogenic, and underlying neoplasm, with symptoms worsening gradually. A healthy 34-year-old woman developed sudden paraplegia after back pain. Urgent T1-T2 laminectomy revealed a subdural hematoma, leading to successful evacuation. Eight months postoperative, she walks independently with reduced spasticity and intact muscle strength. The initial presentation may be subtle and symptoms worsen rapidly. Magnetic resonance imaging is

Spontaneous intracranial hypotension (SIH) is associated with subdural hematoma in the supratentorial convexities. We report a patient with SIH who had presented to us with subdural hematoma in the posterior fossa. A 50 year old lady presented with history of sudden onset of lower back ache of 4 weeks duration and daily headaches and neck pain for past three weeks. The headache exacerbated on activity, on sitting and standing position and relieved on supine position. There was no history of fever or head trauma. Imaging of the brain revealed hematoma of the tentorium cerebelli, hematoma of the

Chronic subdural hematoma (cSDH) is an increasingly prevalent condition, particularly among elderly population and is associated with elevated morbidity. The pathophysiology of cSDH involves proliferation of dural edge cells, fibroblasts, and release of vascular angiogenic factors leading to angiogenesis of a vascularized neomembrane perfused by the middle meningeal artery (MMA). MMA embolization is a technique that can be employed as treatment in conjunction with surgery or as a standalone therapy. Our aim is to describe our experience with treating selective cSDH patients with embolization o

BACKGROUND: Even mild head trauma can cause severe intracranial hemorrhage in patients with cerebrospinal fluid (CSF) shunts for hydrocephalus. CSF shunts are considered a risk factor for subdural hematoma (SDH). The management of acute SDH (ASDH) in shunted patients with normal pressure hydrocephalus can be challenging. Addressing the hematoma and the draining function of the shunt is important. To preserve the shunt, we set the shunt valve pressure to the highest and perform hematoma evacuation for ASDH. In this study, we report the surgical cases of ASDH in patients with shunts.
