Arthroscopic assisted bone grafting for early stages of Kienböck's disease.

Kienböck's disease is known for its difficulty in being diagnosed and treated at early stages; option treatments are few and most of them quite aggressive. The author describes his experience with arthroscopic assisted lunate bone grafting. Three patients with diagnosis of stage I avascular necrosis of the lunate (average age: 45 years), were treated. Before surgical procedure, the patients underwent to a conservative treatment. After harvesting the bone graft from the volar surface of the radius, arthroscopic bone grafting was performed. At an average follow-up of 13.5 months (9-15), all the

Osteonecrosis in kidney recipients: has hypocalcaemia a role?

In a previous study, the musculoskeletal affections among kidney recipients (KR) were reported, with 63 KR suffering from joint affections. We sought to determine the prevalence of osteonecrosis (ON) among those KR with joint affections, its distribution among the affected joints, and possible risk predictors. KR were subjected to biochemical, haematological, and hepatitis serology estimations. Radiographic and magnetic resonance imaging were performed. Relevant data were retrieved from Patient Information System. Fourteen KR were suffering from ON, with an incidence of 22.2% in KR with joint

Inactive disease in polyarticular juvenile idiopathic arthritis: current patterns and associations.

OBJECTIVES: To describe the achievement of inactive disease (ID) and remission in polyarticular juvenile idiopathic arthritis (JIA) and to measure the associations among patient characteristics, imaging results and these outcomes. METHODS: We performed a retrospective cohort study of children with polyarticular JIA diagnosed and treated at Seattle Children's Hospital between 1 January 2000 and 31 December 2006. Each patient's disease status (active disease vs ID) was determined for every clinic visit. Adjusted relative risk estimates were obtained using Mantel-Haenszel methods. RESULTS: One

Serum cobalt and chromium elevations following hip resurfacing with the Cormet 2000 device.

This study was designed to monitor serum cobalt (Co) and chromium (Cr) levels at multiple time points following hip resurfacing with the Cormet 2000 device. Serum samples were obtained preoperatively, at 6 months, 1, 2, and 3 years after surgery. Co/Cr levels (micro g/L) were determined by high-resolution inductively coupled plasma mass spectrometry. Thirty-five subjects were followed. Median preoperative Co/Cr levels were 0.21 and 0.22, respectively. Serum levels following device implantation were increased at all follow-up time points when compared to preoperative controls. Peak levels were

Long-term results of multiple synovectomy for patients with refractory rheumatoid arthritis. Effects on disease activity and radiological progression.

OBJECTIVES: We developed a radical multiple synovectomy (RaMS) procedure designed to reduce disease activity in refractory RA patients involving the excision of all inflamed synovial tissues. In this study we examined the long-term outcome of RaMS in terms of disease activity and articular destruction. METHODS: Forty-two patients with refractory RA underwent RaMS and were followed up for an average of 7.3 years. Clinical findings and radiological outcome were evaluated and compared to a control group. RESULTS: More than 40% of patients continued to maintain improvement up to 4 years after th

Surgical management of calcific tendinitis of the shoulder: an analysis of 26 cases.

Chronic calcific tendinitis of the shoulder has good results after medical treatment in most cases. Around 10% of the patients resistant to medical treatment require surgery. We report 22 patients operated on using an open procedure. The operation included removal of the calcific deposit and an acromioplasty. The patients were rated preoperatively and postoperatively for pain, limitations in activities of daily living, range of movement and power according to a questionnaire and the Constant assessment. Globally the average score increased from 52.2 points out of 100 at the preoperative examin

Conservative open acromioplasty.

Chronic rotator-cuff syndrome with impingement is satisfactorily treated by acromioplasty by both open and arthroscopic techniques, but the Neer operation with deltoid detachment requires a prolonged rehabilitation. Arthroscopic acromioplasty reduces recovery time but is a difficult procedure. McShane, Leinberry and Fenlin (1987) described a conservative open anterior technique with good results and shortened rehabilitation. A similar technique has been used in 25 patients with an average age of 46 years. After a mean follow-up of 24 months, 80% were very satisfied and none was dissatisfied. T

Diagnosis of medial temporomandibular joint disc displacement with dual space anteroposterior arthrotomography: correlation with cryosectional morphology.

This study was undertaken to determine if it is possible to identify medial disc displacement with dual space anteroposterior (AP) arthrotomography. Contrast medium was injected into the upper and lower joint spaces of 11 fresh temporomandibular joint (TMJ) autopsy specimens and arthrotomograms were obtained with an AP projection. The arthrotomograms of five joints showed an enlargement of the medial recess of the upper joint compartment with folding of the disc inferiorly. This was interpreted as an indication of medial disc displacement. Cryosections confirmed medial displacement of the disc

The sore wrist without a fracture.

The possible causes of wrist pain in a patient with normal roentgenograms are many, and the above discussion of differential diagnosis is by no means intended to be all inclusive. Rather, my intent has been to provide a checklist of some of the more likely causes as well as a systematic approach to the identification of the cause. Many of the tests described are somewhat costly (Table 31-3), and they should not be ordered indiscriminately. Rather, the need for each test is determined by a reasonable presumptive diagnosis, depending on the history, clinical examination, and routine roentgenogra

Magnetic Resonance Imaging in Symptomatic Children With Hereditary Multiple Exostoses of the Hip.

BACKGROUND: Magnetic resonance imaging (MRI) is useful in evaluating nontraumatic hip pain. It provides information about associated injuries like labral/chondral tears or ischiofemoral impingement (IFI). However, in hereditary multiple exostoses (HME) there has been no report about MRI findings in symptomatic children with hip involvement. METHODS: Records of children with HME and hip osteochondromas, who had hip MRI/magnetic resonance arthrography, were reviewed. The presence of chondral lesions and labral tears, as well the presence of IFI, was recorded. IFI was defined as edema or fatty r

Evolution of nonoperatively treated symptomatic isolated full-thickness supraspinatus tears.

BACKGROUND: The natural history of small, symptomatic rotator cuff tears is currently unclear. The purpose of the present study was to assess the clinical and structural outcomes for a consecutive series of patients with symptomatic, isolated full-thickness supraspinatus tears who had been offered rotator cuff repair but declined operative treatment. METHODS: In the study period, twenty-four patients with isolated full-thickness supraspinatus tears that had been diagnosed by means of magnetic resonance arthrography were offered rotator cuff repair and elected nonoperative treatment. The twent

Radiographic and patient factors associated with pre-radiographic osteoarthritis in hip dysplasia.

BACKGROUND: Hip dysplasia leads to abnormal loading of articular cartilage, which results in osteoarthritis. The purpose of this study was to investigate the anatomic and demographic factors associated with the early onset of osteoarthritis in dysplastic hips by utilizing the delayed gadolinium-enhanced magnetic resonance imaging of cartilage (dGEMRIC) index as a marker of the disease. METHODS: Ninety-six symptomatic dysplastic hips in seventy-four patients were assessed with standard radiographs and a dGEMRIC scan. The lateral center-edge angle of Wiberg, the acetabular index of Tönnis, and

Bicipital tendinitis and tenosynovitis in the dog: a study of 15 cases.

AIM: To describe the clinical, radiographic, and sonographic features of 15 dogs with bicipital tendinitis and tenosynovitis, classify them according to cause, and evaluate the long-term efficacy of treatment. METHODS: Dogs exhibiting forelimb lameness with pain localised to the biceps tendon were included in the study. Sonographic examination of the tendon and tendon sheath, and radiographic examination including positive contrast arthrograms of the shoulder joint were performed, and assessed for features consistent with biceps tendon disease. In some cases, synovial-fluid analysis and surgi

Improvement in Functional Outcome Scores Despite Persistent Pain With 1 Year of Nonsurgical Management for Acetabular Labral Tears With or Without Femoroacetabular Impingement.

BACKGROUND: Acetabular labral tears are increasingly recognized as a source of hip pain in a younger active population. Given the significant focus on surgical intervention, there has been limited investigation on the nonsurgical management and natural history of such injuries. HYPOTHESIS: Patients undergoing nonsurgical treatment for symptomatic acetabular labral tears experience functional improvement based on patient-reported outcome measures over the course of 1 year. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Seventy-one patients were identified who had acetabular labral

Imaging appearance and distribution of intra-articular adhesions following open FAI surgery.

OBJECTIVES: To evaluate the appearance and distribution of intra-articular adhesions on direct MR arthrograms (MRA) in symptomatic patients after surgical hip dislocation (SHD) for the treatment of femoroacetabular impingement (FAI). METHODS: All 18 patients (19 hips) who underwent arthroscopic debridement for treatment of symptomatic adhesions after open surgery of FAI between 2003 and 2012 and that had a complete set of pre- and postoperative direct MRA were evaluated. On radial PD-w images, pre- and postoperative osseous alpha angles were measured. Signal intensity and degree of obliterati

A case report of basic calcium phosphate deposition disease mimicking septic arthritis.

We report a case of a 53-year-old man who presented with a diagnostic dilemma mimicking septic arthritis. It is important to consider the diagnosis of calcific peri-arthritis clinically and recognize the hallmarks on radiograph and magnetic resonance imaging as this disease process resolves completely with conservative management like in our patient, and does not require operative intervention.

The Clinical Outcome of Anatomical Reattachment of the TFCC to the Ulnar Fovea using an ECU Half-Slip and an Interference Screw.

We developed an original reattachment technique using a half-slip of the extensor carpi ulnaris (ECU) tendon with a very small titanium interference screw for chronic foveal avulsion of the TFCC. The clinical outcome of 66 wrists with foveal detachment of the TFCC treated by this procedure was examined.A distally based ECU half-slip was harvested, inserted into the TFCC, sutured to the remnant of the TFCC, and pulled out through a 2.5-mm bone tunnel at the centre of the fovea. The ECU half-slip was subsequently anchored to the ulnar fovea with a small titanium interference screw. We evaluated
