The lumbar disc and its imitators.

A review of the differential diagnosis of the lumbar disc syndrome, based on patients seen and treated during the past 10 years, provides discussion of the unusual cases. A survey of 285 patients treated in a consecutive series from 1964 to 1972 was done, which showed that 253 patients had prolapsed lumbar discs, 20 were treated for spondylolisthesis and 62 (23%) required consideration because of unusual features in association with the disc lesion or because of other pathology. The need for lateral and complete exposure of the nerve root in some instances is reaffirmed.

Role of MRI in Differentiating Various Posterior Cranial Fossa Space-Occupying Lesions Using Sensitivity and Specificity: A Prospective Study.

BACKGROUND: Any abnormal space-occupying posterior fossa lesion may directly involve the vital structures like the brain stem, cranial nerves, cerebellum, vertebrobasilar artery, and venous sinuses, which makes the surgical approach and total excision very difficult. Hence for these reasons, precise evaluation of posterior fossa lesion with MRI is a must to visualize the vital structures, which helps in planning and safe surgery.  Objective: This study aimed to evaluate the added value of diffusion-weighted imaging and magnetic resonance spectroscopy in the localization, extension, characteriz

Integration of intraoperative ultrasound and depth-electrode electrocorticography for resection guidance in epilepsy surgery: technical workflow and feasibility.

BACKGROUND: Complete resection of the epileptogenic zone (EZ) is the strongest predictor of seizure freedom in drug-resistant epilepsy (DRE). However, even in MRI-positive cases with anatomo-electro-clinical concordance, the EZ may not be clearly delineated, complicating intraoperative decision-making. Intraoperative ultrasound (ioUS) provides real-time anatomical feedback, while depth-electrode intraoperative electrocorticography (iECoG) enables electrophysiological delineation of epileptogenic tissue beyond the cortical surface, sampling deep regions not accessible to subdural electrodes. The fetal taenia-tela choroidea complex: a radiologic-pathologic correlation from normal anatomy to Dandy-Walker malformation.

BACKGROUND: The taenia-tela choroidea-choroid plexus complex has been proposed as an imaging marker to differentiate Dandy-Walker malformation from Blake's pouch cyst on prenatal MRI. However, radiologic-pathologic correlation remains limited. OBJECTIVE: To characterize the taenia-tela choroidea-choroid plexus complex in normal and Dandy-Walker malformation fetuses and assess its visibility on routine prenatal MRI. MATERIALS AND METHODS: Nine fetal autopsy specimens with Dandy-Walker malformation and eight gestational age-matched controls with normal posterior fossa were reviewed for taenia-

Endoscope-Assisted Microsurgery in Pediatric Cases With Pineal Region Tumors: A Study of 18 Cases Series.

Background: An endoscope-assisted technique was recently introduced to microsurgery (MS) and may compensate for the disadvantages of MS for deep-seated lesions. This study was performed to identify the effectiveness and safety of endoscopic-assisted microsurgery (EAM) and share our experience of EAM for pediatric cases with pineal region tumors. Method: We retrospectively analyzed the clinical data of consecutive pediatric cases with pineal region tumors treated by EAM or MS from January 2016 to June 2020. These data included the patient population, clinical manifestations, preoperative examin

Scrotal Migration of the Ventriculoperitoneal Shunt in a 1-Year-Old Pediatric Patient: A Case Report and Systematic Literature Review.

Background: Migration of the peritoneal end of the ventriculoperitoneal shunt (VPS) into the scrotum is a rare but recognized complication. Inguinoscrotal migration typically occurs as a result of increased intra-abdominal pressure combined with a patent processus vaginalis. A 14-month-old pediatric patient presented to the emergency department with abdominal pain, vomiting, and swelling of the right scrotum that had persisted for two days. The patient had a history of a head injury that had resulted in a large secondary arachnoid cyst for which a VPS had been placed at eight months of age. Ex

Giant congenital melanocytic nevi: the significance of neurocutaneous melanosis in neurologically asymptomatic children.

Patients with a giant congenital melanocytic nevus can develop melanotic tumors characterized by central nervous system involvement, termed leptomeningeal melanocytosis or neurocutaneous melanosis. Although symptomatic neurocutaneous melanosis is rare, we previously reported distinct magnetic resonance (MR) findings of T1 shortening, strongly suggestive of neurocutaneous melanosis, in 30 percent (6 of 20) of children with giant congenital melanocytic nevi who presented initially without neurological symptoms. The purpose of this study was to determine the incidence of neurocutaneous melanosis

[Pathomechanisms of spinal canal stenosis - upright MRI image gallery].

AIM: Lumbar spinal canal stenosis is a common disease of the elderly patient, with a high prevalence and clinical importance. MRI is the established method of choice for the imaging of spinal canal stenosis. However, there is often a discrepancy between the clinical symptoms and the spinal canal stenosis as shown using MRI in a supine position. In such cases preoperative functional imaging is often warranted. METHODS: In an image gallery three cases of a functional spinal canal stenosis of the lumbar spine are shown. In all three patients a dynamic, positional MRI (upright MRI) was performed.

Cognitive impairment in epilepsy patients and its correlations.

OBJECTIVE: Epilepsy is a severe disease in which seizures play the leading role. Striking clinical manifestations of the attacks take most of the attention of healthcare professionals. Apart from epilepsy itself, it is well known that epilepsy patients may also have psychiatric comorbidities. These disorders, such as anxiety and depression, are mostly thought to be related to epileptic seizures or antiepileptic medications. In clinical practice, cognitive impairment is another disrupted area of interest in epileptic patients. Our study aimed to detect this deterioration and its correlations wi

Neurocutaneous melanocytosis (melanosis).

Neurocutaneous melanosis (NCM; MIM # 249400; ORPHA: 2481], first reported by the Bohemian pathologist Rokitansky in 1861, and now more precisely defined as neurocutaneous melanocytosis, is a rare, congenital syndrome characterised by the association of (1) congenital melanocytic nevi (CMN) of the skin with overlying hypertrichosis, presenting as (a) large (LCMN) or giant and/or multiple (MCMN) melanocytic lesions (or both; sometimes associated with smaller "satellite" nevi) or (b) as proliferative melanocytic nodules; and (2) melanocytosis (with infiltration) of the brain parenchyma and/or lep

Intra-CSF administered recombinant adenovirus causes an immune response-mediated toxicity.

High doses of adenotk were injected into the cerebrospinal fluid of rats and nonhuman primates (Macaca mulatta). Vector administration was followed by ganciclovir administration for 14 days. Despite the absence of clinical symptoms, analysis of the cerebrospinal fluid (CSF) and histopathological examination of the central nervous system (CNS) of the monkeys (3 weeks after vector injection) were consistent with a viral meningitis. Immunohistochemical analysis of the inflammatory infiltrates in the monkeys revealed the presence of T and B lymphocytes, indicating a combined cellular and humoral i

John Howship (1781-1841) and growing skull fracture: historical perspective.

In the late 18th and early 19th centuries, Dr. John Howship, a pioneering British surgeon, described the clinical features and pathophysiology of various surgical disorders of the human body. His critical contributions to pediatric neurosurgery came in 1816 when he first described the features of an important childhood condition following head trauma, what he referred to as parietal bone absorption. This condition as depicted by Dr. Howship was soon to be christened by later scholars as traumatic cephalhydrocele, traumatic meningocele, leptomeningeal cyst, meningocele spuria, fibrosing osteiti

Lumbar radiculopathy in ankylosing spondylitis with dural ectasia.

We present a 57-year-old man with ankylosing spondylitis (AS) and significant dorsal lumbar dural ectasia (diverticulum). An L5 nerve root monoradiculopathy associated with AS has not been previously reported. The quantity and extent of such ectasia is variable but may be related to cauda equina syndrome. Long-term follow-up of these cases may reveal whether or not solitary nerve lesions gradually lead to cauda equina syndrome. We recommend that asymptomatic or symptomatic patients with dural ectasia should be closely observed without need for immediate surgical intervention.

Clinical and magnetic resonance imaging evaluation of facial pain.

The aim of the study was to evaluate facial pain clinically and to determine the frequency with which structural lesions were diagnosed by means of magnetic resonance imaging in a sample of patients with facial pain, including refractory trigeminal neuralgia and atypical facial pain. Fifty-two patients with facial pain were examined clinically, and treatment protocols were adopted for trigeminal neuralgia and atypical facial pain. The patients with atypical symptoms and those who did not to respond to the treatment underwent magnetic resonance imaging to screen for intracranial lesions. Magnet

The success of endoscopic third ventriculostomy in children: analysis of prognostic factors.

OBJECTIVE: The treatment of hydrocephalus in children with endoscopic third ventriculostomy (ETV) has particular features and is associated with different success rates (SR). The aim of this study was to identify putative factors that could influence the outcome of ETV in children. METHODS: Clinical data of 114 consecutive patients under 18 years of age who underwent 116 consecutive ETVs from January 2000 to January 2010 were reviewed. Data were analyzed with regards to clinical and radiological SR. The actual long-term SR was compared to that predicted by the ETV Success Score (ETVSS) model.
