Analogy between laryngeal gesture in Mongolian Long Song and supracricoid partial laryngectomy.

This article reports the results of a multiparametrical analysis of Mongolian Long Song, characterised by multiple ornamentation and shows the similarities between the laryngeal behaviour observed during these ornamentations and the compensatory gesture produced by patients after supracricoid partial laryngectomy. This study includes (1) a physiological analysis of videofiberscopic laryngeal data from a healthy Mongolian singer and from three non-singer French-speaking clinical patients; and (2) an acoustical analysis (fundamental frequency and intensity). For the singer, the fiberoptic analys

Pathophysiology of dorsal displacement of the soft palate in horses.

Dorsal displacement of the soft palate (DDSP) is an intermittent obstructive upper airway condition that occurs in athletic horses during high-intensity exercise. The pathogenesis of this condition is unknown, but may involve epiglottic hypoplasia, malformation, or neuromuscular dysfunction. In this paper, we report on investigations into the pathophysiology of DDSP. In 3 separate experiments, Standardbred horses were exercised on a high-speed treadmill at speeds corresponding to 50, 75 and 100% of maximum heart rate. The upper airway was evaluated by videoendoscopy and measurement of tracheal

Evaluation of the predictive value of tongue height to oral cavity height ratio and anterior neck soft tissue measurements for difficult laryngoscopy in patients with unanticipated difficult airway: A prospective observational study.

BACKGROUND AND AIMS: Effective airway assessment prevents intubation failure. Despite the limited sensitivity of standard tools, ultrasonography (USG) offers promise, especially in predicting difficult laryngoscopies. Our study aims to evaluate tongue-to-oral height ratio (TTOHR) and anterior neck soft tissue measurements on airway USG to predict difficult laryngoscopy. METHODS: This prospective observational study enroled 120 patients posted for elective surgery under general anaesthesia, without anticipated difficult airways. The skin to hyoid bone distance (SHBD), skin to epiglottis distan

Potential Dysphagia Following COVID-19 Infection Due to Cervical Osteophytes: A Case Report.

The epiglottis separates the pharynx from the larynx by flipping upward to prevent swallowed material from entering the airway. Cervical vertebral osteophytes can interfere with this movement, potentially leading to dysphagia. Videofluoroscopic (VF) and videoendoscopic (VE) examinations of swallowing are valuable tools for understanding the pathophysiology of dysphagia and for formulating appropriate rehabilitation plans. A 77-year-old man with pre-existing dysphagia for solid foods was hospitalized for coronavirus disease 2019 (COVID-19). He stabilized with antiviral and supportive therapy bu

Evaluation of the Adequacy of Using the Supraglottic Airway Device (i-gel®) in Cases With Epiglottic Masses: A Mannequin Simulation Study.

BACKGROUND: Epiglottic masses are often asymptomatic, making them difficult to detect during preoperative examinations. Consequently, anesthesiologists may face ventilation difficulties with no apparent cause. Epiglottic masses can sometimes obstruct laryngoscope insertion into the epiglottic vallecula, complicating general anesthesia induction. In such cases, supraglottic airway insertion may be a viable alternative; however, the limited case reports on its use for epiglottic masses make its applicability unclear. Therefore, we test the hypothesis that a larger laryngeal artificial mass could

Design and implementation of botulinum toxin on cricopharyngeal dysfunction guided by a combination of catheter balloon, ultrasound, and electromyography (BECURE) in patients with stroke: study protocol for a randomized, double-blinded, placebo-controlled trial.

BACKGROUND: Cricopharyngeal dysfunction (CPD) occurs in various neurological disorders, especially stroke. The treatment approaches of CPD include swallowing training, cricopharyngeal dilation, botulinum toxin (BTX) injection, and cricopharyngeal myotomy. BTX injection into the cricopharyngeal muscle is effective and safe for some patients with dysphagia, with a success rate between 43 and 100% (mean = 76%). However, well-designed randomized controlled clinical trials are needed to verify its efficacy and safety for the treatment of CPD. The objective of this study is to explore the efficacy a

Soft palate length and upper airway relationship in OSA and non-OSA subjects.

BACKGROUND: The narrowest area of the airway between the posterior nasal opening and the epiglottis is usually located in the retro palatal area. Many consider this the most likely site of airway obstruction during an obstructive sleep apnea (OSA) event. The aim of this study was to investigate the differences in soft palate and airway length between OSA and non-OSA patients. METHODS: In this study, we analyzed the ratio of the soft palate and the upper airway length in 45 consecutive patients. Twenty-five had an Apnea-Hypoapnea Index of more than five events per hour and were classified in t

Dysphagia caused by ventral osteophytes of the cervical spine: clinical and radiographic findings.

The purpose of our study was to demonstrate the clinical and radiographic findings in patients with dysphagia and ventral osteophytes of the cervical spine due to degeneration or as a typical feature of diffuse idiopathic skeletal hyperostosis (DISH, Forestier Disease). Since 2003 we encountered 20 patients with such changes in the cervical spine causing an impairment of deglutition. A total of 12 patients had one solitary pair of osteophytes of neighboring vertebrae, 4 patients revealed two pairs and 4 patients had triple pairs of osteophytes. Thirty-two osteophytes were observed totally. A t

Enhanced radiation sensitivity and radiation recall dermatitis (RRD) after hypericin therapy -- case report and review of literature.

BACKGROUND: Modern radiotherapy (RT) reduces the side effects at organ at risk. However, skin toxicity is still a major problem in many entities, especially head and neck cancer. Some substances like chemotherapy provide a risk of increased side effects or can induce a "recall phenomenon" imitating acute RT-reactions months after RT. Moreover, some phototoxic drugs seem to enhance side effects of radiotherapy while others do not. We report a case of "radiation recall dermatitis" (RRD) one year after RT as a result of taking hypericin (St. John's wort). CASE REPORT: A 65 year old man with comp

Hypoosmotic stimuli activate a chloride conductance in rat taste cells.

The oral cavity is subjected to a wide range of osmotic conditions, yet little is known about how solution osmolarity affects performance of the gustatory system. In order to elucidate the mechanism by which hypoosmotic stimuli affect the peripheral taste system, I have attempted to characterize the effects of hypoosmotic stimuli on individual rat taste receptor cells (TRCs) using whole-cell patch clamp recording. Currents elicited in response to voltage ramps (-90 to +60 mV) were recorded in control saline and in solutions varying only in osmolarity (-30, -60 and -90 mOsm). In roughly two-thi

A new method of evaluation of upper airway in patients with obstructive sleep apnoea--computer-assisted quantitative videoendoscopic analysis.

INTRODUCTION: Prospective study to quantitatively examine the static and dynamic changes of upper airways in patients with obstructive sleep apnoea (OSA) by engaging a new inexpensive clinical method which accurately evaluates the morphology of obstructive sites of upper airway. The aim was to minimise the subjective visual estimations and eliminate individual variations of the traditional method of nasopharyngoscopic assessment of upper airway. METHOD: Videoendoscopic (video-nasopharyngoscopic) examinations of upper airways of 15 patients were carried out with a calibrator inserted through t

Indications and results of fronto-lateral laryngectomy using a combined endolaryngeal and external approach.

Patients with laryngeal anterior commissure, cord-commissure, bilateral anterior cord-commissure carcinomas (T1 and T2 N0 M0) were subjected to a new method of frontolateral laryngectomy using a combined endolaryngeal and external approach. The proposed surgical procedure allowed the removal of the anterior commissure and part of one or both vocal cords in a single unit, together with the cartilaginous framework, respecting the integrity of the superior portion of the thyroid cartilage. The internal procedure permitted an accurate delimitation of the posterior part of the laryngeal neoplasm. I

[Fronto-lateral laryngectomy using a combined endolaryngeal external approach].

Functional laryngectomies-whether subtotal (with significant demolition) or conservative (with limited latero-frontal and frontal demolition)-can, at times, lead to significant post operative functional sequelae. Negative outcomes method proposed calls for an internal, endolaryngeal approach along with an external approach. The internal approach makes it possible to carefully delimit the posterior extension of the laryngeal neoplasm. The external approach, on the other hand, makes it possible to remove the anterior commissure and part of one or both vocal cords in a single unit together with a

Magnetic resonance imaging of the upper airway. Effects of propofol anesthesia and nasal continuous positive airway pressure in humans.

BACKGROUND: Anesthetic agents inhibit the respiratory activity of upper airway muscles more than the diaphragm, creating a potential for narrowing or complete closure of the pharyngeal airway during anesthesia. Because the underlying mechanisms leading to airway obstruction in sleep apnea and during anesthesia are similar, it was hypothesized that anesthesia-induced pharyngeal narrowing could be counteracted by applying nasal continuous positive airway pressure (CPAP). METHODS: Anesthesia was induced in ten healthy volunteers (aged 25-34 yr) by intravenous administration of propofol in 50-mg
