Durability and survival are similar after elective endovascular and open repair of abdominal aortic aneurysms in younger patients.

OBJECTIVE: The role of endovascular repair (EVAR) of aortic aneurysms in young patients is controversial. The purpose of this study was to determine the long-term outcomes and reintervention rates in patients 60 years of age or younger who underwent elective open or endovascular repair of an abdominal aortic aneurysm. METHODS: Retrospective review of a prospectively collected vascular surgery database at a university-affiliated medical center was performed to identify all patients who underwent elective repair of an abdominal aortic aneurysm between 2000 and 2013 and were 60 years of age or y

alpha1-Antitrypsin deficiency presenting with panniculitis and incidental discovery of chronic obstructive pulmonary disease.

A 60-year-old man presented to the Emergency Department (ED) with large, painful, indurated plaques on the right thigh, left abdomen, left chest, and right chest, which began without any preceding trauma on the right thigh 3 weeks prior to presentation in the ED. He was initially treated with cefazolin 1 g three times daily as home infusions. When the lesions continued to progress, he was admitted to the hospital and placed on amoxicillin/clavulanate and vancomycin. He had a single episode of fever of 102 degrees F, but his white blood cell count and differential remained normal. An initial bi

Appendicitis in children less than 3 years of age: a 28-year review.

Appendicitis is the most common surgical abdominal emergency in the pediatric population, but is rarely considered in children less than 3 years of age. The goal of this study was to identify the presenting symptoms and signs in this age group and examine their subsequent management and outcome. A 28-year experience of a single pediatric surgeon in academic practice was reviewed; 27 children less than 3 years old (mean 23 months) comprised 2.3% of all children with appendicitis in his series. The most common presenting symptoms were vomiting (27), fever (23), pain (21), anorexia (15), and diar

Postoperative laryngeal symptoms in a general surgery setting. Clinical study.

INTRODUCTION: Vocal cord injuries (VI), postoperative hoarseness (PH), dysphonia (DN), dysphagia (DG) and sore throat (ST) are common complications after general anesthesia; there is actually a lack of consensus to support the proper timing for post-operative laryngoscopy that is reliable to support the diagnosis of laryngeal or vocal fold lesions after surgery and there are no valid studies about the entity of laryngeal trauma in oro-tracheal intubation. Aim of our study is to evaluate the statistical relation between anatomic, anesthesiological and surgical variables in the case of PH, DG or

The Relationship Between BMI and Postoperative Complications Among Colorectal Cancer Patients Undergoing Surgery.

INTRODUCTION: Colorectal cancer (CRC) is common worldwide, and surgery is one of the main treatments. Postoperative complications are a concern. The primary objective of this study is to determine whether elevated body mass index (BMI), the presence of comorbidities, tumor characteristics, and the type of surgery are associated with an increased risk of postoperative complications such as wound infections, pulmonary complications, anastomotic leak, venous thromboembolism (VTE), bowel obstruction, and incisional hernia. The secondary objective is to describe the characteristics of colorectal ca

Transgastric large-organ extraction: the initial human experience.

INTRODUCTION: In laparoscopy, it often is the case that port sites are enlarged for specimen extraction. This leads to higher risk of trocar site complications, such as infection or incisional hernia. Natural orifice surgery (NOTES) is beneficial for minimizing these complications, and this is emphasized when the extracted specimen is of large volume. We have been using transgastric technique for appendectomy, cholecystectomy, and laparoscopic sleeve gastrectomy (LSG). Of these transgastric operations, we focus on the one with relatively large-organ extraction: LSG with transoral remnant extra

A Spine Surgeon's Learning Curve With the Minimally Invasive L5 to S1 Lateral ALIF Surgical Approach: Perioperative Outcomes and Technical Considerations.

BackgroundLateral anterior lumbar interbody fusion (L-ALIF), or oblique lumbar interbody fusion at L5 to S1 (OLIF51), is a minimally invasive approach between the iliac vessels that provide indirect decompression, alignment restoration, and lordosis. While supine ALIF exposure has historically been performed by vascular surgeons, minimally invasive L-ALIF can be performed by spine surgeons familiar with lateral approaches. The L-ALIF is a relatively newer approach, and the objective of the present study was to assess the learning curve of the L-ALIF approach by comparing the pre- and postopera

Vertical banded gastroplasty versus standard or distal Roux-en-Y gastric bypass based on specific selection criteria in the morbidly obese: preliminary results.

BACKGROUND: Predicting successful outcomes after bariatric surgical procedures has been difficult, and the establishment of specific selection criteria has been a subject of ongoing research. In an effort to choose the most appropriate surgical procedure for each patient, we have established a specific set of selection criteria for each procedure based on degree of obesity, preoperative dietary habits, eating behavior, and various metabolic features. METHODS: From June 1994 to December 1998, 90 bariatric surgical procedures were performed at the authors' institution by a single surgeon (F.K.)
