Safety of transorally-inserted anvil for esophagojejunostomy in laparoscopic total gastrectomy.

BACKGROUND: To assess the safety of transorally-inserted anvil (TOA) for use during esophagojejunostomy (EJ) reconstruction during laparoscopic total gastrectomy (LTG). METHODS: Between March 2009 and December 2011, 39 consecutive open total gastrectomies (OTGs) and 36 LTGs using TOA for gastric cancer were comparatively evaluated. We investigated postoperative complications, using the Clavien-Dindo classification. To evaluate the effect of a learning period in using TOA for LTG, we also investigated shifts in the patterns of complications and changes in total operation time over the course o

Recurrent cholangitis with and without anastomotic stricture after biliary-enteric bypass.

We recently surgically treated 24 patients incapacitated by recurrent cholangitis after biliary-enteric anastomosis performed for benign disease. Contrary to commonly held dogma, as many as one third of the patients had no evidence of anastomotic stricture indicated by radiologic and operative findings. We identified several other primary and coexistent pathogenetic factors including intrahepatic stricture in 42% of the patients, intrahepatic calculi in 25%, improperly constructed enteric conduits in 13%, and conditions that predispose to bacterial overgrowth in the biliary tree in 17%. Successful management of latrogenic aortoduodenal fistula by intraaortic endovascular stent-graft and duodenal diversion.

Aortoduodenal fistula is usually fatal if not treated promptly. Long-term results are also not always satisfactory. A 53-year-old female patient suffered from massive hematemesis and shock due to an iatrogenic aortoduodenal fistula which was made during a laparoscopic total colectomy. Immediate closure of fistula with duodenal diversion by occlusion of duodenal lumen proximal to the fistula, bilateral truncal vagotomy and pyloroduodenojejunostomy were performed. Endoluminal aortic stent graft was inserted 3 days later. The patient remained disease-free 3 years after operation. With a prompt di

Gallbladder emptying after antiulcer gastric surgery.

It has been shown that truncal vagotomy with pyloroplasty (TVP), but not highly selective vagotomy (HSV), delays the onset, decreases the extent, and changes the pattern of gallbladder emptying. The aim of the present study was to investigate any alterations in gallbladder emptying after a variety of antiulcer gastric surgery, by milk-technetium 99m (99mTc)-dimethyl iminodiacetic acid (HIDA) scintigraphy. After excluding the cases with spontaneous gallbladder evacuation before milk ingestion, there were 26 controls, 41 duodenal ulcer (DU) patients, 22 after HSV (15 prospective cases), 50 after

Evaluation of the learning curve of laparoscopic choledochal cyst excision and Roux-en-Y hepaticojejunostomy in children: CUSUM analysis of a single surgeon's experience.

INTRODUCTION: Laparoscopic cyst excision and Roux-en-Y hepaticojejunostomy is gaining popularity as a treatment for choledochal cyst (CDC) in children. However, the learning curve for this challenging laparoscopic procedure has not been addressed. The aim of this study is to determine the characteristics of the learning curve of this procedure. This may guide the training in institutions currently not using this technique. METHODS: A prospectively collected database comprising all medical records of the first 104 consecutive patients undergoing laparoscopic CDC excision and Roux-en-Y hepatico

Evaluation of the safety and efficacy of esophagojejunostomy after totally laparoscopic total gastrectomy using a trans-orally inserted anvil: a single-center comparative study.

BACKGROUND: Although laparoscopic surgery is frequently performed for the treatment of gastric cancer, laparoscopic total gastrectomy is not widely performed because of its technical difficulty. Since December 2007 we have performed esophagojejunostomy after totally laparoscopic total gastrectomy (TLTG) in more than 110 cases in our institution by using a circular stapler with a trans-orally inserted anvil. We performed a single-center comparative study to evaluate the safety and efficacy of esophagojejunostomy using a trans-orally inserted anvil in patients who underwent TLTG for the treatmen

Metallic stent placement in patients with recurrent malignant obstruction in the surgically altered stomach.

PURPOSE: The purpose of this study was to assess the technical feasibility and clinical effectiveness of expandable metallic stent placement in 196 patients with recurrent malignant obstruction in their surgically altered stomach. METHODS: The 196 patients were treated using five different types of gastric surgery performed for gastric cancer: total gastrectomy (type 1) in 73 patients; distal gastrectomy with gastroduodenostomy (type 2) in 39 patients; distal gastrectomy with a Roux-en-Y gastrojejunostomy (type 3) in 21 patients; distal gastrectomy with a gastrojejunostomy (type 4) in 49 pati

Total laparoscopic esophageal bypass using a colonic conduit for corrosive-induced esophageal stricture.

BACKGROUND: The colon and the stomach are the most commonly used conduits for esophageal replacement in patients with esophageal strictures resulting from corrosive ingestion. The replacement surgeries have traditionally been performed by an open approach. While laparoscopic replacement surgery using a stomach conduit has been previously reported, a total laparoscopic bypass using a colonic conduit has not been previously described. We herein describe the surgical technique and results of laparoscopic esophageal bypass using a colonic conduit. METHODS: Patients with corrosive stricture involv

Closure of small bowel stomas on postoperative day 10.

OBJECTIVE: To find out if early closure of a defunctioning small bowel stoma (day 10) was feasible and safe. DESIGN: Prospective non-randomised study. SETTING: University hospital, France. INTERVENTIONS: During a 42-month period (January 1998-June 2001), all patients with a temporary small bowel stoma were elected for early closure on postoperative day 10 in a non-randomised prospective study. The procedure was considered only if the patient was not taking steroids, was in good condition, and had not developed wound or general sepsis after the initial operation. Other patients' stomas were

Percutaneous endoscopic gastrostomy and gastrojejunostomy in psychomotor retarded subjects: a follow-up covering 106 patient years.

BACKGROUND: Whether psychomotor retarded persons should be treated by percutaneous endoscopic gastrostomy (PEG) or by surgical gastrostomy combined with an antireflux procedure is controversial. Therefore, the authors investigated the feasibility of a PEG and enteral feeding in these patients. METHODS: Patients referred from specialized institutions for a PEG placement were assessed extensively by a multidisciplinary team. When considered eligible, age and general condition determined the choice of treatment under general anesthesia (group 1) or conscious sedation (group 2). Patients were fol

First Documented Ivor Lewis Oesophagectomy for Oesophageal Squamous Cell Carcinoma in Somalia: A Case Report From a Resource-Limited Setting.

BACKGROUND: Oesophageal squamous cell carcinoma is associated with poor prognosis, particularly in resource-limited settings where access to advanced staging, multi-modal oncology treatment and complex surgical care is constrained. Ivor Lewis oesophagectomy is an established surgical approach for selected mid- and distal oesophageal tumours, but its implementation requires specialised surgical, anaesthetic and peri-operative support. To the best of our knowledge, this is the first documented report of Ivor Lewis oesophagectomy for oesophageal squamous cell carcinoma performed in Somalia. CASE

Complications of minimal-access surgery in children.

Minimal-access surgery (MAS) is rapidly becoming the surgical approach of choice for a variety of surgical disorders in adults, but its use in children remains a relative novelty. Most pediatric surgeons continue to harbor justifiable concerns about the morbidity of this modality owing to the cumbersome nature of the instruments and the technical difficulty associated with two-dimensional views. The purpose of this study was to determine the complication rate and the lessons learned from the use of MAS in performing a variety of procedures in a large series of children. To determine complicati

Spiral enteroscopy-assisted ERCP in bariatric-length Roux-en-Y anatomy: a large single-center series and review of the literature (with video).

BACKGROUND: Deep enteroscopy-assisted ERCP (DEA-ERCP) in post-bariatric Roux-en-Y (RY) anatomy is challenging. Laparoscopy-assisted ERCP (LA-ERCP) and EUS-directed transgastric ERCP (EDGE) are technically easier and faster but are more invasive and morbid procedures. Therefore, we have used DEA-ERCP as our first-line approach, reserving EDGE and LA-ERCP for cases in which adjunctive techniques that cannot be performed through an enteroscope are required (eg, EUS-FNA, sleeve sphincter of Oddi manometry), or DEA-ERCP failures. The 2 main methods for DEA-ERCP are balloon- and spirus-assisted. Cur

[Dying with living donor transplantation].

A female patient with primary sclerosing cholangitis developed a cholangiocarcinoma (Klatskin tumor) at the age of 42 years. It was successfully resected by hemihepatectomy and hepaticojejunostomy. In the next 15 years she had recurrent episodes of bacterial cholangitis and had to be hospitalized several times a year for intravenous antibiotics. At the same time the sclerosing cholangitis progressed and she developed liver cirrhosis. The patient, who was never willing to give up, underwent liver transplantation by receiving the left liver lobe of her daughter (living donor). Postoperatively sh

Pyloric atresia associated with Dieulafoy lesion and gastric dysmotility in a neonate.

In this case study, we report a male infant with pyloric atresia, extreme gastric distension, and a caliber-persistent gastric artery (Dieulafoy lesion) with massive gastric bleeding. After a transverse pyloroplasty and endoscopic hemoclip application to the caliber-persistent gastric artery, very slow gastric emptying developed, which required repeated surgical interventions. Gastroduodenostomy failed to promote gastric emptying. The intraoperative and postmortem histologic examinations of the gastric wall revealed a loss of interstitial cells of Cajal, which possibly explains the extreme mot

Neonatal short bowel syndrome: a cohort study.

BACKGROUND: To date, our knowledge of morbidity and mortality in neonatal short bowel syndrome (SBS) is based on individual case series. Shortcomings of the published literature include long patient recruitment time, selection bias, variable SBS definitions, failure to account for gestational age, and incomplete follow-up. By applying more rigorous methodology, our aim was to determine outcomes of SBS neonates compared with a control group of neonates without SBS. METHODS: A cohort study of all neonates with abdominal pathology requiring laparotomy between January 1, 1997, and December 31, 19

Duke procedure for super obesity: preliminary report with 3.5-year follow-up.

BACKGROUND: Super obese patients pose a major health care problem because their obesity is associated with high risks of morbidity and even death. Neither dietary programs nor surgery results in significant or lasting weight loss. Presented here is a new surgical approach, combined with home infusion therapy, that offers effective weight loss with maintenance for 3.5 years. METHODS: Eight super obese patients (252% +/- 19% ideal body weight; 62.7 +/- 5.6 kg/m2 body mass index) underwent near total intestinal bypass anastomosing the proximal 25 to 30 cm of jejunum to the transverse colon. Management of critically ill surgical patients Case reports.

UNLABELLED: The acute abdomen (AA) still remains a challenging situation for surgeons. New pathological conditions have been imposed to our attention in this field in recent years. The definition of abdominal compartmental syndrome (ACS) in surgical practice and the introduction of new biological matrices, with the concepts of tension-free (TS) repair of incisional hernias, prompted us to set up new therapeutic strategies for the treatment of patients with AA. Thus we reviewed the cases of AA that we observed in recent years in which we performed a laparostomy in order to prevent or to treat a
