The outcome of patients with hinge abduction in severe Perthes disease treated by shelf acetabuloplasty.

BACKGROUND: The management of patients with Perthes disease remains controversial. In children with hinge abduction and the potential for remodeling, we have performed a shelf acetabuloplasty, in an effort to contain the hip and allow remodeling. We report our medium-term results in a consecutive series of 27 children with severe Perthes disease and arthrographically proven hinge abduction. This is the first report in the English literature to look specifically at the results of treating patients with hinge abduction in severe Perthes disease by shelf acetabuloplasty. METHODS: Twenty-seven co

[The value of pneumo-arthro-CT in diagnosis after traumatic shoulder dislocation].

Traumatic subluxation of the shoulder in young patients may result in reluxation after further injury. 18 patients with traumatic subluxation and 22 patients with recurrent subluxation of the shoulder (after primary trauma) were examined by arthroscopy and pneumo-arthro-CT. This minimally invasive procedure showed high accuracy in the diagnosis of the typical glenoid margin and humoral head abnormalities. Nevertheless, arthroscopy cannot be abandoned for planning suitable operative treatment.

Niebauer trapeziometacarpal arthroplasty: a long-term follow-up.

A retrospective review of long-term follow-up of Niebauer trapeziometacarpal arthroplasty for treatment of disabling arthritis of the basal joint was performed. Thirty implants in 27 patients were reviewed, with an average follow-up of 9 years (minimum, 4 years). All surgical procedures were performed by the senior author. Eighty-eight percent of the patients were subjectively pleased and would undergo the procedure again. Postoperative subluxation occurred in 83% of the patients. This was not painful unless the prosthesis dislocated. Subluxation increased with time. One case of silicone synov

[Possibilities of shoulder arthroscopy in comparison with magnetic resonance tomography and arthro-computerized tomography].

In the last 15 years the methods of examination of the shoulder have intensively expounded. According to a well documented retrospective study we describe the value of the shoulder arthroscopy and its correlation to MRI and arthro-CT findings. MRI represents nowadays the best way of exploring rotator cuff lesions and of investigating an impingement syndrome. Associated with a contrast medium its ability to clear up an anterior or posterior shoulder instability is now superior to the arthro-CT's. The arthroscopy allows the dynamic exploration of the joint and can be used not only as a diagnosti

Comparison of arthrotomographic and magnetic resonance images of 50 temporomandibular joints with operative findings.

Arthrotomography and magnetic resonance imaging (MRI) were carried out on 50 temporomandibular joints (TMJs) in 48 patients who were being considered for surgery for clinically diagnosed internal derangement. The patients presented over a 4-year period with pain and dysfunction which had failed to respond to conservative management. Open surgery was carried out on all TMJs and operative findings compared with the results of imaging. The clinical diagnosis of internal derangement was confirmed in every case by imaging and at surgery. Arthrotomography over-diagnosed non-reducibility of an anteri

[Shoulder pain and shoulder stiffness: causes, differential diagnosis and therapy:].

Like lumbago, pains and stiffness of the shoulder are a crux medicorum. Here, however, unlike lumbago, promising treatment is possible if a correct diagnosis made. The cardial symptoms are classified for the diagnosis; conservative and surgical treatment of the individual disease pictures are described.

The impact of discography on the surgical decision in patients with chronic low back pain.

BACKGROUND CONTEXT: A reduced frequency of discographies might be the result of increasing concern with long-term effects of discography such as disc degeneration. More knowledge is needed in what patient discography is most likely to influence the surgical decision. PURPOSE: This study was aimed at highlighting how discography affects surgical decisions when performed on one of four different indications in a complicated subgroup of patients with chronic low back pain assumed to be associated with degenerative disc disease (DDD). STUDY DESIGN: Prospective before-after study to analyze how f

Unilateral hip osteoarthritis: can we predict the outcome of the other hip?

OBJECTIVE: The objective of this study was to define, in unilateral hip osteoarthritis (OA), factors predicting the outcome of the other hip. MATERIALS AND METHODS: We examined the anteroposterior radiographs of the pelvis of 95 white patients with unilateral idiopathic (56 patients) or secondary to congenital hip diseases (39 patients) OA. The other hip was free from symptoms (pain or limping) at the initial examination and without radiographic evidence of OA; it was what we call a "normal" hip. Two parameters were evaluated: (1) the type of osteoarthritis of the involved hip and (2) the ran

Long term evaluation of disease progression through the quantitative magnetic resonance imaging of symptomatic knee osteoarthritis patients: correlation with clinical symptoms and radiographic changes.

The objective of this study was to further explore the cartilage volume changes in knee osteoarthritis (OA) over time using quantitative magnetic resonance imaging (qMRI). These were correlated with demographic, clinical, and radiological data to better identify the disease risk features. We selected 107 patients from a large trial (n = 1,232) evaluating the effect of a bisphosphonate on OA knees. The MRI acquisitions of the knee were done at baseline, 12, and 24 months. Cartilage volume from the global, medial, and lateral compartments was quantified. The changes were contrasted with clinical

Failures of open reduction in developmental dislocation of the hip.

Eight patients with the same number of developmental dislocated hips were initially treated with open reduction through an anterolateral approach. The hips then became dislocated again. There were six girls, with a mean age at first open reduction of 13.3 months. We treated the hips with a new open reduction through an anteromedial approach. A constricted anteromedial capsule was always found as the main factor; all had an intact anteromedial capsule, there was an inverted transverse ligament in three cases and a very tight psoas tendon in another three cases. All were reduced, without complic

Treatment of the athlete with multidirectional shoulder instability.

Treatment of multidirectional instability in the athlete continues to be a challenging problem. Nonoperative treatment remains the initial treatment of choice in most athletes. However, some patients will fail nonoperative treatment and might require operative intervention. An open capsular shift procedure is indicated in nonoverhead athletes, most contact athletes and in most revision situations. However, the advances in thermal treatment of the capsule have made arthroscopic electrothermal capsulorrhaphy increasingly attractive as an alternative to the open approach for the primary treatment

Temporomandibular joint disk displacement: arthrographic and tomographic follow-up after 6 months' treatment with disk-repositioning onlays.

Eighteen patients with an arthrographic diagnosis of temporomandibular joint disk displacement with reduction were treated with disk-repositioning onlays for 6 months. Arthrograms and tomograms were obtained before and after treatment. The arthrographic examination after treatment showed that the disk remained in the superior position in twelve but was again displaced in six patients. Three of the patients with disk displacement manifested no clinical signs, such as clicking or locking; two showed signs of locking; and one had reciprocal clicking. The patients with clicking or locking had more

Influence of joint volume on range of motion after arthroscopic rotator cuff repair.

BACKGROUND: Capsular contracture is a well-known etiology in the primary stiff shoulder; thus capsular contracture and resultant decreased joint volume could lead to postoperative stiffness, which is a commonly reported morbidity after arthroscopic rotator cuff repair (ARCR). The purpose of this study was (1) to quantify the joint volume (total joint volume and each quadrant compartmental volume) using computed tomography arthrography (CTA) and (2) to demonstrate the relationship between joint volume and postoperative range of motion (ROM) after ARCR. MATERIALS AND METHODS: Eighty-three patie

Arthroscopic capsulolabral reconstruction of the type VIII superior labrum anterior posterior lesion: mean 2-year follow-up on 13 shoulders.

BACKGROUND: A type VIII superior labrum anterior posterior lesion represents pathologic posteroinferior extension of a type II superior labrum anterior posterior lesion with injury to the insertion of the posterior band of the inferior glenohumeral ligament. No reports in the literature describe arthroscopic treatment of a type VIII superior labrum anterior posterior lesion and its associated glenohumeral instability. HYPOTHESIS: Arthroscopic capsulolabral reconstruction is effective in alleviating pain and restoring stability and function in athletes with glenohumeral instability due to the

Shelf arthroplasties long-term outcome: influence of labral tears. A prospective study at a minimal 16 years' follows up.

INTRODUCTION: Osteoarthritis lesions extent and dysplasia severity (negative vertical center edge [CE] angle) are recognized as unfavorable criteria for the survival of shelf arthroplasties performed for correcting hip dysplasia. Labral tears have recently been described on dysplastic hips, indicating beginning osteoarthritis and worsening the risk of instability. HYPOTHESIS: The labral tears identified in the course of shelf arthroplasty procedures for correction of hip dysplasia carry a predictive value for the survival of this operation. OBJECTIVES: Evaluate this hypothesis at the interme

Critical Findings on Magnetic Resonance Arthrograms in Posterior Shoulder Instability Compared With an Age-Matched Controlled Cohort.

BACKGROUND: Posterior shoulder instability is less common and potentially more difficult to diagnose clinically and radiographically compared with anterior shoulder instability. Radiographic findings including posterior labral tears, increased retroversion, presence of glenoid dysplasia, and increased capsular area are associated with symptomatic recurrent posterior shoulder instability. PURPOSE: This study aimed to determine the prevalence and severity of associated radiographic parameters found on magnetic resonance arthrograms (MRAs) in patients with arthroscopically confirmed isolated pos
